This directory is for people who live in the following counties in Washington State: Adams, Clark, King, Spokane, Whatcom, and Yakima.

This directory is current as of 08/21/2015. Some network providers may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. To get the most up-to-date information about Vision Services Plan’s network providers in your area, you can visit www.healthfirst.chpw.org or call our Customer Service Department toll-free at 1-800-942-0247, or (TTY Relay: Dial 711) between 8:00 a.m. to 8:00 p.m., 7 days a week.

Community HealthFirst™ is an HMO plan with a Medicare contract. Enrollment in CommunityHealthFirstMedicare Advantage Plans depends on contract renewal.

Section 1 – IntroductionThis directory provides a list of Community HealthFirst™ Vision Services Plan network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.

The “network providers” listed in this directory have agreed to provide you with your vision health care coverage. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network providers, please see your Evidence of Coverage for detailed coverage information, it may cost more to get care from out-of-network providers.

While you are a member of our Plan, you must use network providers to get your covered services except in limited cases such as emergencies or urgently needed out-of-area care. With the exception of emer-gencies or urgent care, it may cost more to get care from out-of-network providers.

After you receive urgent or emergency care, make sure you inform your PCP and Community Health-First™ Customer Service at 1-800-942-0247 (TTY Relay: Dial 7-1-1). You should do this as soon as possible and ideally within 48 hours of receiving care. For details on urgent and emergency care benefits, please refer to your plan’s Evidence of Coverage booklet.

Obtaining Vision Health Care Services Out-of-NetworkYou must use plan providers except in emergency or urgent care situations. If you obtain routine care from out-of-network providers, neither Medicare nor Community HealthFirst™ will be responsible for the costs.
