SIXTIETH ANNUAL REPORT OF THE COMMITTEE OF MANAGEMENT. To THE GOVERNORS AND SUBSCRIBERS.

LADIES AND GENTLEMEN —

When last year's Report was compiled your Committee had to admit that the Institution was worse off at the end of the year than it was at the beginning.

Your Committee has again to preface its Report with the admission that financially the year has been worse than its predecessor,^as, notwithstanding the fact thai £102 was withdrawn from the Reserve Fund to reduce the Bank overdraft, the twelve months has closed with a deficit of £720 on the year's transactions. The Bank overdraft was £288 on 30th June, 1912, and accounts still unpaid amounted to £700, making the total debit owiiig £988. The Income for the year from all sources was £6,392, and Expenditure £7/112.

The deficiency has been principally caused by the great increase in number of cases nursed at the Infectious Wards, from Geelong and surrounding districts, the amount contributed by the several Councils being totally inadequate to pay for numbers treated. This question is however dealt with later on in the Report.

Other causes that have militated against the Credit Balance at the Bank have been increased expenditure on salaries and wages, and higher prices for provisions, which item has also been influenced by the course adopted by the Committee of providing a more liberal table for the nurses.

It has been found necessary to increase the salaries of,nurses in charge of wards; also wages of the male and female domestic workers and others, necessitated by the great scarcity of this class of labour. Wages of nurses and servants shows an increase of £400 for past year.

The enlarging of the Nurses' Home over two years ago was the commencement of die Institution falling into debt. The cost of these buildings, £1,000, had to be paid from Current Account. It was hoped at the time that something would be done by the public to raise money to pay this debt. Neither private benefactor nor the public however came to the rescue, and the result is that the Institution's Reserve Fund lias had to be drawn upon, and a considerable sum paid to the Bank for interest annually. Two fixed Deposits, one for £200, and another for £600, shortly clue, will also be withdrawn from Reserve Fund, reducing that account by those amounts. The Fund, which took fourteen years to build up, its interest only being used as a source of income, has thus in one short year been reduced by one quarter of its total. To summarize we may say the income is stationary and expenditure increasing.

Most of the items under "Income" compare favourably with those for preceding year. The Hospital Sunday Fund is increased by £37. The item, "Entertainments," £13, is small. Your Committee's experience of this source of revenue has been unfortunate, as on several occasions, performances organized in aid have resulted in little or no profit to the Institution. Although considerable sums have been drawn from the public nominally for or to aid this Charity, but expenses have'swallowed all or nearly all, and the Institution profited but little.

The total number of patients passing through the Institution was 3,660, Of these Jj,320 were admitted to the Wards. This shows an increase for year of 1,162 patients.

OUT-DOOR PATIENTS.—Number admitted, Males, 850; Females, 490; Total, \J$~ Number of occasions on which they attended, 5,677.

INFECTIOUS DISEASES.—293 patients were treated. They were as follows:-Diptheria, 256; Typhoid, 33 ; Scarlet Fever, 4. 3 deaths occurred from Diptheria; 3_ from Typhoid. This constitutes a record for Diptheria as far as our Annals go. Of these patients Geelong sent 119 ; Newtown and Chilwell, 33 ; Geelong West, 16; Shire of

Bellerine, 18; Shire of Winchelsea, 20; Shire of Bannockburn, 19; Shire of Barrabool, 3; Shire of Corio, 13 ; Shire of Belmont and South Barwon, 7; Shire of Leigh, 4.

Rate of mortality has been 5.3 for all cases; 8.5 for Hospital In-door patients. 16 deaths occurred within 24 hours of admission.

E N D O W M E N T FUND.—Amount to credit of this is £7,685 2s. fid. It should be mentioned that the donors to this Fund have by their wills expressed their intention to leave this money for all time to the Institution, the interest only to be available for maintenance. Your Committee are of opinion that benefactors intentions should be strictly carried out.

The Hon. the Treasurer is reported in the Press recently as saying to a Hospital deputation " that they should spend their Endowment Fund." On it being pointed out to him that this was illegal, he is stated to have said "that a short Bill in Parliament would soon alter that." Your Committee contend that this is not the way to encourage people to leave their money to this Charity for a specific purpose.

OPERATING THEATRE—Instruments cost £190, but included in this sum is £93 for purchase of a new Steam High-pressure Sterilizer of latest design and pattern, for treating all dressings, gowns, instruments, &c, in as short a period as possible. The Theatre is always an expensive part of every Hospital, and ours is no exception. As indicating its necessity, 260 operations have been performed during the year.

The daily average of occupied beds has been 67.2 Hospital, and 74.5 Benevolent Asylum. The average cost per bed was £50 Os. 9d„ which is £7 more than last year.

H O S P I T A L S A T U R D A Y A N D SUNDAY.—£502 was subscribed, £80 of which was collected in the streets on Saturday by the Nurses and the following Honorary Collectors: Salvation Army Lasses, Miss Dot McManus, Masters Jack, Mederic, Guillaume and Miss Christine Fraillon. To the Nurses and Honorary Collectors your Committee's grateful thanks are now tendered. It is hoped that more willing volunteers will present themselves next Sunday Collection, and emulate these kind children in th eir efforts to help the sick and helpless. Although a number of Churches continue to divide their collections between the Hospital and Ladies' Benevolent Society, your Committee still hopes that the several Religious Denominations who do this, will eventually see the justice of the Hospital's contention, that Hospital Sunday Colleetions should be given entirely to the Hospital.

O L D - A G E P E N S I O N E R S A N D T H E B E N E V O L E N T ASYLUM.—TheBenevolent Asylum is fast becoming, instead of a Home for the aged, a Hospital for pensioners who are too ill and weak to linger outside, and who are then brought here by the Police, or the people with whom they board, to die. We have to bury them, if they have only been resident a day, the Old-Age Pension Department refusing to contribute anything towards a pensioner's funeral, and should the Commonwealth Old-Age Pension Department agree to pay any special case's pension to the Institution, it is deducted by the State Government from their Annual Grant, and thus an actual loss is further thrown upon this Institution. It is the opinion of your Committee that in many cases the granting of a pension does the recipient more harm than good. These poor people, with no one to look after them, sink into dirt and poverty. They would live longer, and be happier, in any properly conducted Asylum.

I N F E C T I O U S W A R D S . — T h e s e have continued very full during the whole year, 293 patients having been admitted; each one stayed on an_ayerage 23 days. The totally unexpected numbers seeking admission quite upset the calculations on which the various compromises as to payment were come to with surrounding Councils, and your Committee has had to advise all the Municipal Bodies that in future a minimum charge of £2 5s. will be made for every patient admitted. This amount was fixed after an exhaustive inquiry by your Committee as to what was the actual cost of each patient treated, Hospital authorities not undertaking to collect any money from patients.

As the General Hospital has had to bear the burden of these cases, there was nothing else to be done, if the Institution is not to drift steadily towards bankruptcy. The Government gives no grant towards upkeep of Infectious Hospital, so if Councils do not pay the cost of same, the Hospital has to. It is satisfactory to be able to report that nearly all the Councils have seen the justice of the Committee's decision, and have agreed to the terms indicated. No Council is obliged to send their patients to us, and in the event of any Council not agreeing, we shall have to absolutely refuse to treat their residents in the Infectious Hospital.

C O N S U M P T I V E P A T I E N T S — Y o u r Committee is strongly of the opinion that in every district there should be erected by the Government, Sanatoria for the proper nursing and care of consumptive cases,where patientscould be treatedinthemost up-to-datemethod, and also isolated. At present these cases have to be nursed here in the midst of surgical and medical patients, often lingering for months at a time. We cannot give them the special expert treatment they should receive, which should be given early in the disease. Recently on several occasions patients suffering with this complaint in an advanced stage, have had to be admitted after being discharged from the recognised Government Sanatorium as incurable ; having been turned adrift, helpless, on the community in the most dangerously infectious stage of their complaint.

N E W C H A R I T I E S B I L L . — T h e Government has for some time contemplated getting a new Hospital and Charities Act passed, which was brought up by the Hon. W. A. Watt in July, 1911. This Bill, if carried as proposed, will bring under the jurisdiction of a Board of Charity practically all Charitable Institutions. Most of these Charities are successfully managed by Committees, who have had very extensive experience of their needs, and of the best methods of affording relief. In many cases the Secretary and Members of Committee have acted continuously for very long periods, and have acquired knowledge and experience which could be gained in no other way.

At a Conference held in Melbourne, at which representatives of this Hospital attended, they, with a large number .of delegates, objected to one Board as then proposed, to govern Metropolitan and Country Hospitals, but joined in the opinion that, while not objecting to a Board to govern Metropolitan Institutions, a second Board should be also appointed to manage Country Charities.

The Board will consist of three members—a Chairman at £800, and two members at £600 each. Your Committee cannot see what advantage the Board can claim over the present system. The powers proposed to be given to it are very extensive indeed. Your Committee is of the opinion that whatever purpose it is intended to serve, it will not help the Charities, or advance their welfare in any way. The Melbourne Institutions are protesting against the Bill, and asking Country Charities to join them.

N U R S I N G STAFF.—Dur ing the past year several vacancies have occurred on the nursing staff. These have been filled by the appointment of the following as sisters :— Nurses Arnold, Ilett, and Peddle, trainees of our, hospital; and Nurses Symonds and

Mason, trainees of the Melbourne Hospital, latter has also had a course of training at the Infectious Hospital at Fairfield, and is now in charge of the Infectious Wards. Fourteen of our Nurses went up for their final examination, and thirteen were successful in passing. It is with deep regret that we have to record the death of Nurse L. L. Hill, one of our former trainees, and subsequently a sister in charge of Ward 1. She was a splendid character, and one of whom we were justly proud. Her loyalty to the Institution was unswerving and constant. She was loved by everybody with whom she came in contact, and her early death was the cause of bitter grief to all.

STAFF.—Drs. M. Jacobs and J. B. Be]] filled the positions of Senior' and Junior Residents respectively ftfr the greater part of the year. At end of MarchTSrTJacobs retired according to the rules, and Dr. Bell took his place as Senior. Dr. J. Connor was elected to the Junior position. No other changes of any importance have occurred.

MEDICAL STAFF.—Drs. F. J. Newman, A. W. Marwood, T. J. M. Kennedy and G. McCallum were re-elected by the subscribers at last Annual Meeting, with Drs. R. G. McPhee and H. T. Elvins as Hon. Anaesthetist and Skiagraphist respectively. Drs. McPhee and Elvins also take Out-patients four days in each week. Drs. J. Small and P. A. Croker continue on the Staff as Honorary Consulting Surgeons. Thanks are tendered to these gentlemen for their services. With the exception of Drs. Small and Croker they now retire, but offer themselves for re-election.

GIFTS.—The following are gratefully thanked:—Messrs. Humble & Son, supplying Ice free all the year, and use of Cool Store ; Queenscliff Fishing Co. and Fishermen, Fish; Ladies' Flower Mission for supplying cut flowers throughout the year; to the Press for its kind help at all times ; and to the Proprietors of the following papers for supplying their respective journals:—"Australasian," "Argus," "Age," "Leader," "Weekly Times," "Geelong AdVertiser," " Geelong Times," "Ballarat Star," "Advocate," and " Worker." Thanks are tendered to all who have in any way assisted to support or amuse the patients.

CHRISTMAS DINNER.—The usual generous response to our appeal was received at Christmas time. A very ample and varied repast was set before all who were able to partake of it. Cigars and a liberal supply of tobacco were also smoked with relish. All who contributed are gratefully thanked.

BOARD OF MANAGEMENT.—Geo. Martin, Esq., J.P., was re-elected President at the last Annual Meeting ; Dr. J. Small and J. Nicol, Esq., Vice-Presidents; A. W. Butt, Esq., Hon. Treasurer. Mr. Butt resigned as Treasurer owing to his removal from Geelong. Mr. M. B. Young, Manager of the Colonial Bank, was elected by the Committee to fill the position. He now retires, but is eligible for re-election, and offers himself accordingly. Messrs. W. Higgins, J. Purdie, W. A. Bradley, W. Humble, and Rev. E. Harris were re­elected as members of the General Committee. The following members of Committee now retire by effluxion of time, in accordance with the Rules. They are all eligible for re-election, and offer themselves accordingly, viz.:—Messrs. H. B. Brown, W. C. Kernot, E. L. JuUien, G. E. England, T. A. Dickson, and Rev. F. W. R. Newton.

During the year 12 meetings of the General Committee, 50 of the House Com­mittee, and 12 of the Finance Committee have been held; also 7 Special Meetings.

The Committee earnestly hopes that the future will show an extensive increase of interest and income from private sources, otherwise it will be impossible to maintain the high standard the Hospital has always endeavoured to work up to. The State Aid yearly becomes less, and unless more money is received from private sources, the Institution must fall behind, even while the City of Geelong, to which it belongs, is advancing by such rapid steps. Soliciting help, in money or in kind, which our generous friends have so splendidly vouchsafed, in our special times of stress. Our need is before you. The cause of charity is to aid the sick, helpless, and suffering. Our desire is to carry on the work committed to us efficiently. We rely confidently on the public to come now to our aid, and relieve the burden of debt—nearly £1,000.

Admission of Paying Patients.—HvK-hA-W N o . 59.—Paying Pat ients may be admi t t ed at t h e dis­cretion of t h e H o u s e C o m m i t t e e , w h o shal l fix the weekly ra te to be charged to each pa t ien t , and in t h e event of an o p e r a t i o n b e i n g necessa ry , m a y fix a fee therefor , and may requ i re the same to be paid in advance ; a n d every person a d m i t t e d on t h e g r o u n d of emergency , to the benefi ts of the Ins t i tu t ion , and able in t h e e s t ima t ion of t h e C o m m i t t e e , t o pay , shal l be required to do so at such r a t e as t h e Commit tee may d e t e r m i n e . Payment of Maintenance.—All p a t i e n t s will be expected to pay towards t h e actual expenses of thei r ma in tenance , so far a s t h e i r c i r cums tances m a y enable t h e m ; and all" p roper ty found on Pat ients w h o m a y die in t h e Hosp i t a l wil l be a p p r o p r i a t e d to defray the expenses of t h e m a i n t e n a n c e and bur ia l . T h e C o m m i t t e e of M a n a g e m e n t shal l a p p o i n t a Sub-Commit tee (with t h e concur rence of t h e S u r g e o n ) which sha l l h a v e power to d i scha rge any p a t i e n t in case of h is refusal to p a y ; and shal l have the fur ther power to r e q u i r e (if n e c e s s a r y ) , P a t i e n t s to m a k e a declara t ion before a Jus t ice as to the i r m e a n s .
