REPORTS OF SOCIETIES.MEDICAL SOCIETY OF LONDON.
Nov. 2ND, I868.B. WV. RICHARDSON, M.D., F.R.S., President, in the Chair.
MR. ALFRED COOIER showed a Calculus whlichl he had removed fromthe bladder of a boy, aged I2, by the lateral operation of lithotomy. The point of interest was that the boy had been in the West LondonHospital for seventeen dlays without showing a symptomi of the presenceof the foreign body.
Mr. COOPER also exhibited three Polypi of the Rectum, one of whichhe had removed from a child, one from a lady, and one from a gentle-mani. He had used the ligature in all cases, and all had been mistakenfor hTmorrhoids.
Mir. HENRY SAIITII showed a Stone, weighing five and a-half ounces, which he had removed from a woman's bladder. She had suffered eightor ten years from the disease. I-lc extracted the calculus piecemeal byan incision made through the vagina. The patient had since died.
IMr. SMITH also exhibited a case of Stone from the Female Bladderin which he had performed the usual operation of lithotrity. At thesecond operation there was something difficulty in introducinlg the litho-trite; the cause of obstruction was discovered to be a fragment as 'largeas a small walnut, which had become impacted in the urethra. Thiswas removed by a slight incision, and the patient did well.A discussion then followed on the question of dilating the urethra.
Mr. WALTER COULSON and Mr. B3RYANT were of opinion that rapiddilatation was seldom, if ever, followed by incontinence, whereas slowor gradual dilatation was attended by this distressing sequel.
7The Afedio-lateral O0eration for Zitholoo/,. By HENRY LEE, Esq.-Mr. LEE described the mode of performing the medio-lateral operation, and said it combined the advantages of all the other methods that hadbeen recommended without many of their disadvantages, the urethra inMAr. Lee's operation was entered in the median line, which he consideredensured the success of the operation as far as the surgeon was concerned. The incision was carried round the front and left side of the rectum, soas to leave a sufficiently large external opening. The patient for thisoperation was placed in the ordinary position for lithotomy, and agrooved staff was introduced into the bladder; the skin of the scrotumwas held moderately tight by an assistant, and an incision was made inthe median line of the perin.'eum from before backwards; this shouldextend through its posterior hlalf, terminating two or three lines in frontof the anus. From this point the incision was continued for a quarterof a circle round the front and left side of the rectum. The finger of theleft hand might then be put into the wound, and the rectum passed backwhilst an additional touch or two with the knife separated it still furtherfrom the parts in front. The fore-finger of the left hand was nowpassed into the rectutm, and the knife, with its back towards the bowel, was passed, at the posterior part of the central incision, into the mem-branous portion of the urethra. With the finger as a guide this was donewithl great ease andi certainty. A bistoury or knife with a lpr-obe at itsextremity was then passed into the same opening and made to slidealong the staff into the bladder. The forceps, or any other instr-umentthat might be used, was also introduced more directly into the bladderthan in the ordinary lateral operation. The incision into the prostategland was made from within outward; and this Mr. Lee considered anadvantage. In children, a single incision with the scalpel was sufficient, but in adults, the circular part of the wound shlould be deepened eitherbefore or after the urethra was opened. The operation as a whole was,he thought, the simplest in conception, the easiest in execution, and theleast liable to be attended or followed by any unfavourable complica-tion, of all the operations for lithotomy. Mr. BRYANT advocated theplan that had been taught of using a straight staff. He believed thatalmost any stone of reasonable size could be removed by the lateralmethod. Mr. LEE'S plan might be useful in very large stones.-Mr.WALTER COULSON and Mr. TEEVAN respectively spoke of cutting notdilating the prostate, and exhibited calculi which they had removed. The PRESIDENT gave succinct history of the operation of lithotomy, and showed that, whatever operations were performed, death was inproportion to the age.-Mr. CHARLES HAWKINS said that the mor-tality was greater in private than in hospital practice; the reason being, that hospital patients applied earlier for relief and were more ready toundergo the necessary operative procedure. Private patients, on thecontrary, hoped on until their kidneys and bladders became diseased. In reply, Mr. LEE stated that, doubtless, those who had had much ex-perience in lithotomy could do the usual operation with much precisionand success, but he believed that there were a very large class of sur-

OBITUARY.THOMAS HILLIER, M.D.
WE: deeply regret to have to record the untimely death of this highlyesteemed and accomplished physician. Dr. Hillier's health had beenfailing lately; and, acting under the advice of his medical friends, hewas preparing to pass six months in a warm climate. A sudden shockwhich he received from the death of a near relative seemed greatly toaffect him; and severe cerebral symptoms supervened, ending fatally.
Dr. Hillier was known not only as an able clinical student of skin-diseases and diseases of children, on which he has made many valuablepublications-one quite recently-but as a highly accomplished practi-tioner in all branches of medicine, an earnest and enlightened sanitarian, and a man of eminently scholarlike, refined, and amiable character. He graduated as B.A. of the University of London in 1849, and M.D.in 1855. He was a Fellow of University College, and Physician tothe Children's Hospital and to the skin department of University CollegeHospital. His unwearied labours as Medical Officer of Health of St.Pancras laid that parish under a heavy debt of gratitude to him, but illrepaid by its authorities. His untimely death has caused deep and generalgrief in the ranks of the profession in London.

In thisthere is certainly some mistake, as the number of medical students who were mem-bers of the Medical Society amounted last year to not less than forty-five. Thenumber of those who commenced medical study in the University in that year wastwenty-five. The number of entries seems, therefore, probably equal to that ofany of the provincial schools, and more than half those in London.
I am, etc., BERESFORD N. EARLE,Late Secretary of the Cambridge University Medical Society, Trin. Coll. Camb.
PREVENTION OF INFANTICIDE.SIR,-Can you inform me whether or not there is in Paris (or any other town in
France), an institution, similar to that in Naples, where, in order to prevent infanti-cide, mothers can place their infants, without letter or recommendation of any sort, without even divulging their name or relationship to the child. I am, etc., LEX.P.S.-Is the system of "baby-farming" carnied out to the same extent in Paris
as in London?
NOTICES of Births, Marriages, Deaths, and Appointments, intended forinsertion in the JOURNAL, should arrive at the Office not later thanIO A. M. on Thursday.
F. LucY, F. R. C. S. (Poole).-We do not feel at liberty to give any further advice onthe subject, than the ordinary rules of personal responsibility will of necessity havesuggested to our correspondent.
THE CALENDAR OF THE ROYAL COLLEGE OF SURGEONS.SIR,-I am indebted to a friend for a sight of the new Calendar published by the
College of Surgeons. It is a book excellently got tip, and ftull of interesting matter, and such as shotuld be in the hands of every member of the College. Till now, Iwas unaware of the existence of such a book; but I am told it may now be pur-chased for a shilling. I venture to suggest that it may be sold at the College; orstupplied throiigh the post to any one wvho wvill forwvard eighteen postage-stamps,wvhich would suffice to pay the postage and for the trouble of sending it.Hereford, Oct. I868. I am, etc., A COUNTRY FELLOW.
A MIEMBER Liverpool).-The examinations forthe Fellowship of the College will takeplace on the 2 Ist, instanit.
MR. GARRAWAY (Farnhamrn-The infamous circulars enclosed have before becn no-ticed. In the present state of the Medical Act, we know not how to combat thepractices of these miserable spermatorrhcea quacks.
A. It. C. (Salop). -We cannot see any strong objection to complying with the wish, tinder the circtumstances stated. Our impression is, that such an arrangement is notso very unusual as our correspondent implies.
J. L. shotild consult the work of the author on Uterine Snrg-ery for the therapeuticdetails which he asks. Dr. Marion Sims is at the present moment in New York.
