Please submit the completed form to Karen Limsi via e-mail at Karen.limsi@af.hhs.gov and klimsi@thebizzellgroup.com with a copy to Stacy Cassell at stacy.cassell@acf.hhs.gov at least two business days prior to the target date.

Note that CMC will lightly edit all announcements and correct any glaring grammar, spelling, or punctuation errors. CMC will send the edits to, and questions about, the announcement to the contact persons indicated on the request form for review. Once the text is final, CMC will then send the contact persons a final “proof” copy of the announcement, including any attachments. Once the final version is approved, CMC will distribute the announcement.
