Instructions for the use of logbook
Aim of the logbook
The purpose of the logbook is to provide one source of evidence for the Ob-
stetrics and Gynecology scientific council that you have attained the desired
level of competency required for licensure. It is the place where you are go-
ing to document experiences and operations you performed during your
training.
The logbook is divided into several sections. These instructions will help you
to complete those sections correctly.
Personnel information
Please fill in all your personnel information required . This will help the
Egyptian Fellowship Administrators to process your logbook during scien-
tific council evaluation yearly and finally before sitting for the final exam.
Your personnel photo should be attached to the logbook and you should sign
the personnel information page
Outpatients clinics
You will attend outpatients’ clinics related to antenatal care, family planning,
gynecology and ultrasound. Other outpatients clinics relevant to the specialty
should also be recorded. For each outpatient session please record the follow-
ing information:
1. The type of the clinic attended (antenatal, family planning, etc)
2. The date
3. The location (which hospital or center)
4. The number of cases you managed during the session whether inde-
pendently or under supervision
5. Signature and clear name of attending consultant

Obstetrics and gynecology inpatients data
For each training year, there are tables to enter summary of clinical data rele-
vant to inpatients (obstetrics and gynecology). For each patient, please fill
the following information:
Name, hospital number, provisional/ final diagnosis , summary of diag-
nostic workup, summary of management provided and the final outcome
of the case .
Please note that examples for varieties of patients you are expected to see
are available at your curriculum (page 53&72).
Outpatients and ward procedures
There are tables in your logbook to report outpatient and ward procedures
(whether obstetrics or gynecology). For each procedure please fill in the fol-
lowing information:
Name and hospital number of the patients, diagnosis, name of performed
procedure, level of trainee participation and trainer name and signature
The key for trainee participation in procedures and operations is:
O: Observer
A: Assistant
P: Perform the procedure or operation (either supervised or independent)
Operative logbook
The requested surgical operations , the minimum number to be registered
in your logbook and your expected level of participation are available at
your curriculum (page 76-80).
For each year of training, you will find tables to register the obstetric and
gynecology operations and your level of participation.
In each table , please fill in the following data: name and hospital number
of the patient, date of the operation, the diagnosis, the name of operation,
your role in the operation, name of the first surgeon and your trainer name
and signature.
Academic activities
Academic activities that must be documented in the logbook are
lectures, journal clubs, morbidity and mortality conferences, and work-
shops or other conferences attended.

Workshops and conferences tables are the place where you will record your
CME activities whether inside or outside the training center. Any attended
activity must be signed by the workshop or conference organizer/coordinator
Annual summary table
At the end of each training year, you are requested to provide documented
summary of all operative activities you participated in as assistant or first
surgeon. The tables are present in the last page of each year log and should
be signed by your trainer and educational supervisor
Assessment of logbook activities
1. Your trainer will assess your logbook weekly for completion and provide
feedback
2. Your educational supervisor will assess your logbook monthly or every
two months, provide verbal or written feedback and counter sign
important activities
3. The examination committee of the council will revise your logbook:
A) Annually before your progress from one year of training to another
B) At the end of training before the final exam
To be noted that unsatisfactory completion of the logbook would lead to
delay of training progression. Unsatisfactory logbook at the end of training
will prevent you from entering the final exam
Important Notice:
It is your responsibility to maintain accurate and completed logbook and to
regularly update your records. Shall you meet any difficulty; you must
contact your trainer or your specialty administrator at the Egyptian
Fellowship Board.
