Hazardous Observation Steel Fabrication

1. Insufficient leadership and commitment from the management, safety culture does not exist, which can result into accidents and many other problems, that can ruin the company in no time.

2. Workers employed in this company are not using PPE properly and also some equipments/tools have not provided to them by their employer,which can result in serious personnel injury, acute discusses because of noise, vibration and dust.

3. Drinking water is available but date no mentioned at cartridge of filters. It can result in that some workers/persons may avoid to drink because of the expiry date not mentioned on the cartridge of filters.

4. Housekeeping is poor Housekeeping of the entire fabrication is very bad/poor. Metal pieces are laying all around the floor. It can result in trip and fall of workers may result in severe injury.

1-Management should lead with examples as role model. 2-Set realistic targets and Objectives.3-To review the output progress Quarterly as per ILO-OSH 2001.

5. There are unclear walk ways and No designated escape routes nor Assembly point. During the emergency situation, if there is no clear understanding of Assembly point, may cause delay in rescue of someone who is injured.

6. Some workers doing work in high temperature recording as almost 50 degree above Celsius and for the period of 7 to 8 hours. And some workers not using helmets because of heat. It can result in head injury dehydration to workers because of heat. Skin burning because of heat.

7. First-Aid equipment is in good condition and some of workers also leaving training certificates in first aid.Ambulance is also provides for emergency situations. Providing all the basic requirements.

8. Condition of Washrooms And Toilets is very good. Everything is cleaned.There is very less chance of spreading bacterial infections or diseases etc.Slip and Trip chances are very low.

1- Walk ways should be Clearly Marked and Designated which should lead to Assembly Point.2-Drills/Training exercises must be carried out at least Once in Six-Months

1-Provide Adequate rest intervals to Workers.2-PPE provided to workers Should be Light in Weight.3-Provide workers rest Shelters and Drinking Water.4-Train workers to drink water frequently to Avoid Dehydration.

9. Electrical Connections warning 220/440v are not properly secured. Cables are joined with insulation tapes, Ex-proof glands not installed near flammable gases. It may result in electric shock, Electrocution. Injury from exposure to Arcing. Fire or Explosion.

10-Welding work was carried out near the storage of flammable liquids (Diesel, Petrol).It is considered as unsafe act, and have potential to Cause Fire and Explosion. Damage to equipment and property. Serious personal injuries or Fatalities.

11. Lifting gear accessories e.g. Slings and Shackles are not Clarified.No colour coding is used.It could result in load fall, Inspection of Critical Components are Vital for any lift and Certification by Third-party is Must.

1- Immediate replacement of all damaged glands with Ex-proof glands.2- Cables to be replaced.3- All junction Boxes should be secured.4- High Voltage area should be marked as ‘Danger High Voltage’.

1-Stop the work.2-Shift the work location at safe place.3-Provide training to workers of Hot work and Fire fighting.4-Ensure the new work location is safe and proper housekeeping is there.

1-Remove the expired or out of service parts.2-Third-Party should called for inspectOf all Slings, Shackles with Colour Coding.

12. Only two Fire-Extinguishers available for entire area.Extinguishing agents would be inadequate in result of fire.

13. Workers carrying out the grinding work which creates Dust,But dust Masks and other respiratoryEquipment is not provided to them.It could result in Lung diseases and other respiratory disorders (asthma, emphysema, bronchitis etc),through Inhalation of dust particles. These are Chronic Work diseases.

14. Soft slings were damages to wear out.It could result in falling of load as the SWL may have been lose due wear and tear.

15. There is No Induction training to the visitors in office premises. It could result in anxiety in case of Emergency as Visitors are not briefed on where the Assembly point is, Not sure to escape routes. They could get trapped.

1. Immediate procurement of Fire Extinguishers as the whole Fabrication yard is carrying out Hot-work and Fire Fighting equipments are not Sufficient.

1. As per ILO-OSH 2001,all type of PPE including dust masks and other respiratory equipments should be provided to workers by Employers (free of cost).

1. Discard the soft slings immediately And cut them into two-pieces to avoid 2. Third-Party inspection should be done for the soft slings.

1-All visitors, workers must be given a proper induction training of escape routes, Assembly point that during emergency they could easily use them.2- Types of Emergencies must be explained.

16. Not specific smoking area is Provided at Workplace.Some workers smoking while carrying out the work, While there are electricity cables and other flammable liquids are there.It can result in Fire, Explosion. Serious injuries, loss of life. Damage to property or equipment.

17. No building Fire layout plan, Emergency escape routes are not designed with proper Signage.It could result in chaos during drill and Emergency situations.

18. The work of steel lifting is carried out at fabrication yard by using crane and auxiliary hoist’s safety latch not functioning properly.There is also no maintenance checklists for vehicles at Workplace.It can result in equipment damage, fatality of rigger. Accidents can occur due to the improper Maintenance of Vehicles.

1. There should be specific smoking area provided to workers at workplace.2. Provide training to workers of Fire and Safety.3. Also provide Supervision as necessary.

1. Station bells and other Emergency plan specially Fire, flooding, Earthquakes etc, must be displayed.Proper signage should be there because Signage play vital role during any Black-out situation.

19. In the Dining room, there is proper cleanliness and room is well equipped and well-maintained.All facilities including A/C, microwave , refrigerator, clean wash room and tissue papers etc are Provided.

20. There is no proper segregation and disposal of Waste in the workplace. Hazardous waste is being mixed with the General waste.It could result in the spreading different types of life taking diseases including Malaria, Dengue , Cancer by the exposure to wastes.

21. Observed that there is poor Light in the area where the forklift is carrying out the lifting work of steel.It may lead to an Accident due to the poor lightning in the area.It may cause injury and damage to equipment. Poor lightning is a health hazard, too much or too little light strains eyes and may cause eye irritation and headaches.

Good Practice

1. Inform the Manager to take the action to segregate and dispose the waste.2. Provide separate skips and with colour codes for hazardous waste and general waste.3. Supervisor should ensure that there is proper segregation and dispose of the waste.

1. Provide proper local Lightning in the workplace for the safe operation.2. Workers should be provided and wear High Visibility Jackets.3. Regular inspection of the workplace should be there.

22. Manual Handling : Observed that some workers are used to lift heavy steel material/weight inside the workplace and also with wrong posture. This can result in back Injuries, arm injuries and may also can cause damage to the material that they used to lift.

23. Worker was using hammer for the straightening of steel continuously for two hours and having no brake.He was also not using Noise protection ear plugs/Earmuffs. This can result into arm injury, his sense of touch can be damage. Joints Injury and swelling. The person can also face the problems of Tinnitus, Permanent deafness.

1. Provide electric tugs to Workers to make the work easy.2. Ensure proper posture are adopted in manual handling through supervision.3.Also provide training to the Workers about the Right Postures while lifting the load, or directly use electric tugs to avoid any type of Injury.

1. Need to provide break of 5 to 10 minutes after every 40 minutes.2. Provide the worker Earmuffs or plugs.3.Also provide good quality of Gloves to wear to reduce vibrational effects.4. Provide proper training to the workers to make them aware of the hazards Noise and Vibration.
