PORTFOLIO GUIDELINESDefinitionStudent portfolios are collections of work that represent the student’s achievements and may be used to document and evaluate specific performance outcomes.
PurposeThe use of the student portfolio in the Nursing Department will serve three purposes. The primary focus of the portfolio is for the students to collect and select samples of their work; and then reflect upon the merits of that work. The portfolio serves as a secondary function which is to present a visual representation of the students’ achievements. This visual may be advantageous for academic assessment, employment, scholarships and awards. The third focus of this portfolio is to reinforce learning as a lifelong process.
Assignment DescriptionIt is important to think of the portfolio as a dynamic document rather than a finished product. This implies that it will continuously change. Files may be included or removed at any time. The primary contents of the portfolio will include an Identification/Cover Page, Plan of Work/Career Goals, Health Information, Awards and Examples of Student Outcomes by course or grouping.
Student ResponsibilitiesStudents are required to maintain a “Presentation Portfolio”. The portfolio will be submitted, for review, to the instructor at the end of each clinical course. Ideally, your portfolio is your own personal document that should be designed using your own style. However, there are some guidelines that will help provide consistency across the program and between students. Students will be given background and instructions regarding the portfolio requirement during the new student orientation.
1. The portfolio may be enhanced at any time that the student wishes to replace a designated activity.
2. Portfolio submissions should include a variety of formats such as written materials, presentations, accomplishments, awards, etc.
3. The portfolio process begins in NUR 111- Nursing Foundations and continues through NUR 219-Nursing Issues.
4. The CAP Test Results Form and Clinical Skills Checklist must be updated by the student at the end of each course.
5. A Self-Assessment Form and final course competency (evaluation form, exam score sheet, checklist, etc.) must also be included for each course entry by the student.
6. The Portfolio Assessment Form will be reviewed and completed by the faculty at the each of each clinical course for evaluation.
Portfolio Format1. 3-ring quality binder with dividers for each course.2. All written work must be typed, unless otherwise directed.3. Cover page with essential identification information.4. Follow Guidelines of Portfolio Content Checklist.5. Remember you are not perfect, show growth and maturity as you progress through the
program.

The Department of Nursing requires that all students maintain a portfolio throughout the program. Contents shall be maintained in a 3-ring binder to hold items in a neat and organized fashion.
