SUBJECTIVE DATA
Patient stated that he usually eat fast foods and instant noodles and that he usually drinks 4 – 5 glasses of water a day
Patient verbalized “I think I’m overweight”
Patient is currently having a diet because he wants to loose weight
SUBJECTIVE DATA
Patient verbalized “I would like to know how to loose weight in a healthy way”
Patient stated “I don’t like eating sea foods especially sardines”
Patient stated “I want to look good”
OBJECTIVE DATA Patient looks a bit big from the normal Weight: 200lb Height: 117.8 cm Body Mass Index: 28. 7 = OVERWEIGHT

NURSING INTERVETIONS
1. NUTRITION MANAGEMENT:- tell the patient the best food diet and foods to eat
R: Healthy Living Pyramid and the Food Plate- Eat a lot for Breakfast (Omelet with Avocado, Broccoli and few slices of hot pepper)
R: Eating a heavy breakfast gets the metabolism going and prevents binge eating
1. NUTRITION MANAGEMENT:- Eat moderate lunch (Almonds, Chicken Breast, Low fat yogurt)
R: Eating moderately for lunch would help in maintain digestion through out the day- Eat a few for Dinner (Stir fries with vegetables)
R: We eat a few for dinner because there is minimal activity in the evening for digestion
2. INTAKE OF WATER- tell the patient that 8 – 10 glasses of water is needed in a day
R: 8 – 10 glasses of water must be taken to remain hydrated
3. EXERCISE- suggest cardio and strength training (treadmill run for 5 – 8 minutes daily)
R: Exercising helps burn fats to loose weight
4. SLEEP WELL PATTERN- tell the patient that he needs 6 – 8 hours of sleep and a normal wake up time of 7, 8, or 9 am
R: Sleeping well keeps the body well rested for one to be able to do the activities that may help burn fat
5. MENTAL FITNESS- tell the patient to think that he can loose weight
R: A motivated patient would be eager and willing to loose weight
EVALUATION
GOAL MET:- the client understood the ways on how to loose weight in one hour- the client verbalized:
1. “I need to eat the foods according to the Healthy Living Pyramid and the Food Plate, eat a lot for Breakfast, Moderate for Lunch and a few for Dinner”
EVALUATION
2. “I need to drink 8 – 10 glasses a water in a day to remain hydrated”
3. “I need to exercise by running a treadmill for 5 – 8 minutes”
4. “I need to be able to have 6 – 8 hours of sleep a day”
5. “I need to be well motivated, I can loose weight!”
