Nursing Diagnosis

Fatigue r/t increase insulin resistance by the cells® An imbalance in blood sugar is the main cause of diabetes-related fatigue.  Cells use glucose - sugar - for fuel. The hormone insulin controls the distribution and use of glucose in the body.  In diabetics, due to poor production of insulin or high insulin resistance, meaning the receptors within the surface of the cells do not take in insulin, the glucose is not properly utilized by the cells; instead, it's floating around in the bloodstream, where it can't be used as energy.  As a result, diabetic people will feel drained.

Goal of Care

After my 8 hours of care, Mrs. J will be able to improve sense of energy as evidenced by:

a. decrease fatigue level to 0-4;

b. presence of strong hand grip and;

c. ability to answer questions or communicate with high spirits;

Nursing Interventions

1. Assess vital signs.

2. Determine presence/degree of sleep disturbances.

3. Obtain client’s description of fatigue (i.e. lacking energy or strength, tiredness, weakness over length of time)

4. Ask client to rate fatigue (1-10 scale with ten as the highest and 1 as the lowest).5. Assist with self-care need like ambulation if cannot tolerate.6. Avoid or limit exposure to temperature and humidity extremes.7. Provide diversional activities like open and jovial communication.

8. Encourage to drink fluids.9. Avoid drinks containing caffeine.10. Instruct to breathe more evenly, deeply and slowly.11. Provide a sound and comfortable environment conducive for resting. Limit noise and bring the curtains down to prevent light from getting inside. Provide adequate clothing.12. Stress the importance of sleep.

Evaluation

“GOAL MET” @ 07/26/10 7 amAfter my 8 hours of care, Mrs. J was able to improve sense of energy as evidenced by:

a. fatigue level of 3;b. strong hand grip;c. open to questions and answers patiently,

and initiates communication to student nurse: “Basig makauli nami karon ba. Maka-stress man gud pud diri sa hospital,” said while smiling.
