    APPENDIXA-4HEALTH INSURANCE

    Effective January 1, 2010, * the Board shall provide each bargaining unit Memberand his/her enrolled dependents with an insured Anthem Blue Cross/Blue Shieldhigh deductible Health Savings Account which replicates the PPO plan coverageprovisions in effect December 31, 2009.     The HSA deductible

    The percentile reimbursement is 80th percentile out of networkThe Board of Education will make a deposit each plan year to the participant's HSaccount in the amount of 75% of the applicable deductible value. During the firsyear, the Board of Education will make its deductible deposit on the first day of thpolicy year. In subsequent years of this Agreement, the Board will make itsdeductible deposit as follows: one-half the value on the first day of the plan yearwith the balance to be transferred in monthly installments.The Board shall maintain the present PPO for the limited purpose of providingbenefits to exempt BargainingUnitmembers.     The Board of Education of the Town of Norwalk , Connecticut hereby agrees toemploy... , ........and ... .., """""""'"'''' (to whom the term "teacher"hereinafter refers) hereby agrees to serve, under the direction of the Superintendent of Schools as a teacher in the public schoolsof said Town, for the school year beginning July 1, 20 , subject to the conditions stated below.

    In accordance with the provisions of the salary schedule contained in the group contract covering the above period, betwthe Board of Education for said Town and the representative teacher organization, the Board has voted and hereby agrees to paysaid teacher, and said teacher hereby agrees to accept for service during the above stated period, and annual salary of $...........in... .., ..periodic installments, payable as follows: .Bi-weekly... .beginning """20 , and subject to required deductions for the State Teachers Retirement Fund and the United States Withholding Tax, andother agreed to deductions which the teacher may in writing authorize.

The teacher may resign for good reason by submitting at least thirty days' notice at any time except duringhe month of August, during which month, unless the contract has been terminated by mutual consent or Board action, theeacher will accept employment with no other Board of Education in Connecticut; provided that in the event no group contractor the ensuing year has been signed by August first, limitations on the teacher's resigning shall not be binding until such antract has been signed.The teacher may, upon written request filed with the Board after receipt of any notice from the Board of its intention toerminate or not to renew this contract, be entitled to a statement, within five days of said request, of the reason or reasons foruch termination or failure to renew.     1. Workshop Presenter - Prepares to lead a group of teachers on aninstructional/curricular topic. A teacher may be asked to share his/her expertise omay be asked to lead a workshop on new curriculum. The presenter may be givenmaterials to use or may have to develop his/her own. Presenters may be required toplan an agenda, prepare materials and structure the day through a variety oactivities including whole or small group "instruction".Presenters shall be paid at the contract rate according to Appendix A-6, Category XVIWhen more than one presenter is leading a workshop, the stipend will be split or proratedconsistent with past practice.2. Workshop Facilitator - Maintains discussions or pre-planned activities. For example,when teachers come together to share information, one person agrees to direct thediscussion. A workshop facilitator may also playa more active role but will havebeen provided training and all materials necessary for the workshop.     High School Department Chairpersons may use their release time for any additionalpreparation required to work with their departments.High School and Middle School Subject Area leaders and Middle School Teamleaders who are trained and/or given materials to use when leading a workshop wilnot be paid for preparation. If additional preparation time is required, teachers shallbe notified in advance of the number of hours they shall be compensated. Teachersmay also obtain advance approval for additional preparation time from the AssistantSuperintendent for Curriculum and Instruction.Literacy Specialists who have received training and/or been given materials to usewhen leading a workshop, will usually not be paid for preparation.
