2. Background

The National Laboratories Business Case was approved by the NHSScotland Chief Executives Group on

the 10th April 2018 which recommended an incremental approach to deliver a distributed service model for

laboratories in NHSScotland. The initial focus is on IT connectivity between laboratories and the collection

of standardised data to provide business and clinical intelligence which can be used for multiple purposes.

There is an immediate challenge from the diversity of laboratory IT systems across the 14 Health Boards

that are unconnected, despite significant flows of workload between constituent laboratories. The IT

connectivity project objective is to deliver a solution which will enable the electronic flow of workload

between laboratories. NPEx is seen as an affordable solution delivering many immediate benefits to the

service and will add future value as the service model develops to enable the convergence of IT platforms.

The NPEx solution will also provide a long term link between Scottish service providers and those in the

rest of the UK.

The National Laboratories Programme Team (NLPT) through the IT connectivity project, will work with a

further 7 NHS Boards to implement NPEx in Scotland (4 Boards have already implemented and 3 are in the

process of implementing).

This document is designed to provide NHS Board laboratory services with an overview of how the NPEx

implementation will be managed, along with a sample implementation plan and an understanding of the

tasks and resources required in the initial 2 stages of deployment; Organisational Readiness and

Deployment Initiation. The content has been developed in conjunction with some of the NHSScotland

Boards who have already implemented this solution.

3. What is NPEX?

NPEx is a connectivity solution that can interface different Laboratories Information Management system

(LIMS) between health boards and is relatively simple software, performing a very complex task. NPEx

sends information from the requesting LIMS to the receiving LIMS allowing electronic data prepping of

samples and automatic electronic reporting as soon as results are authorised. This process is displayed in

the diagram below (figure 1).

4. Project Scope

NPEx is already in operation in 4 NHSScotland Boards; NHS Greater, Glasgow and Clyde, NHS Grampian,

NHS Tayside, NHS Lothian and is being implemented in a further 3 boards; NHS Dumfries and Galloway,

NHS Fife and NHS Lanarkshire. The scope of the IT connectivity project is to roll NPEx out to a further 6

territorial boards and the Golden Jubilee National Hospital. At this time the territorial boards include:

The deliverables of the IT connectivity project are to:

Establish a VPN connection in to the NPEx hub

‘Pipe clean’ the connection, using a referral lab partner

Ensure the 7 Boards identified have NPEx capability for Clinical Biochemistry within 12 months

Work with the Boards to complete an expansion plan for connecting with other boards

5. Programme & Project Governance Figure 2 below identifies the governance process for the National Laboratories Programme, including the IT Connectivity Project. The project will be delivered regionally with oversight and management support provided by the NLPT. The NLPT will report progress into the Laboratories Oversight Board (LOB).

The IT connectivity project is part of this programme and will report progress to the LOB via the NLPT as presented in figure 3.

Both an IT lead and Project Manager will be appointed for each region (East, North and West) and will support each of the 7 Boards with implementation. Local laboratory and IT staff will be part of the implementation teams with specific expertise provided from X-Labs (supplier of NPEx) and LIMS interface suppliers.

6. Project Reporting

Throughout deployment weekly checkpoint meetings will take place between the laboratories team, Project Manager, X-Labs and LIMS supplier to update on progress and discuss issues and resolutions. The Project Manager will produce a weekly highlight report from the weekly checkpoint meeting with the implementation team.

The Project Manager will also produce an up to date highlight report in advance of each LOB meeting.

Risks and issues will be captured in the highlight report and escalation will be to the LOB via the IT Connectivity Project Steering Group.

The Project Manager will produce a communication and engagement plan in conjunction with local Board contacts which aligns to the overall programme plan and any regional plans.

8. High Level Implementation plan During the initial planning stages the NLPT have engaged with all boards involved in the roll out of NPEx. The development of the plan has been informed by these discussions.

Phase 1 – Organisational Readiness & Deployment Initiation (this document – pack 1)

This phase will focus on sign off of initial meetings with the laboratories and programme team, obtaining

sign off of Data Sharing Agreement, set up of the required VPN connection, LIMS Interface connection and

recruitment of resources.

Phase 2 – Build Configuration & Testing & Operational Deployment (subsequent document – pack 2)

The initial focus will be on implementing lab to lab communication for clinical biochemistry based on lessons learned from laboratory colleagues who have already implemented NPEx. Clinical biochemistry has a large volume of numerical tests and has been identified as the largest gain for early implementation. A guide based on learning from Boards who have deployed NPEx to date has been developed and is detailed within the NHSScotland Laboratories NPEx Build and Configuration Pack.

Phase 3 – Expansion Planning

The NLPT will work with the Boards to develop an expansion plan relevant to local needs on the successful

completion of phase 2. The plan will outline how each board will expand the further use of biochemistry

and the extension of NPEx to other disciplines (e.g. Haematology, Virology, Immunology etc) following

project handover to Business As Usual operations.

The methodology for deployment is outlined within figure 4 and a high level implementation plan displaying current timelines for the roll out of NPEx is shown in figure 5.

9. Sample Project Plan Figure 6 below provides a sample project plan which includes the detailed task list will be required to complete to ensure a successful roll out of NPEx.

The Project Managers will work with laboratories colleagues in the Boards to agree the implementation plan for their areas. This will be developed using the baseline plan below.

10. Project Resources The NLPT will provide a Project Manager and IT lead for each region to support the stages of deployment including the following:

Local Board laboratory and IT/ eHealth staff will be an integral part of the implementation team and specific expertise will be provided from X-Labs (NPEx supplier) and LIMS interface suppliers as required. The Project Manager will liaise with each board to agree the laboratory staff resource required on the ground. Table 1 below identifies the roles and responsibilities of staff required to achieve the plan with an overview of further resource requirements from a board perspective detailed in section 14.

11. Stage 1 - Organisational Readiness Organisational Readiness (Stage 1) as detailed in the methodology approach is when the Regional Project

Manager will seek to engage with the Board and identify the key contacts associated with the deployment

of NPEx. An initial meet and greet with key stakeholders will provide an opportunity to meet with the Project

Team, X-Labs (NPEx supplier) and provide an overview of NPEx.

To enable sign off of the first stage The Health Informatics Service (THIS) have developed a Data Sharing

Agreement (DSA) that is required to be agreed by the Caldicott Guardian and Senior Information Risk

Owner each of the NHS Boards going through deployment. This document outlines the approach to Data

Sharing in relation to NPEx and further details operational procedures and technical mechanisms to enable

data sharing, in line with organisational policies, procedures and current legislation.

The diagram below details the workflow for these stages and action owners for each step.

1. Engage Board Laboratories Head of Service

The National Programme will seek to engage with the Head of Service for the Laboratory to provide an

overview of the National Laboratories Programme, set up initial meetings and identify appropriate

attendees.

2. X-Labs Meet & Greet & Induction Meeting

The X-Labs Meet and Greet & Induction Meeting will allow Boards the opportunity to meet with the NPEx

supplier and gain an understanding of the platform whilst being able to answer any queries from Boards.

The meet and greet will be facilitated by the NLPT and will further provide an opportunity to share learning

from previous deployments and to discuss the approach and requirements for deployment and share

project management documentation. At this stage the Regional Project Manager will also seek to

understand and document current Board processes in relation to change management and confirm key

contacts in order to outline roles, responsibilities and time requirements for NPEx deployment both within

the laboratories and wider eHealth/ IT Teams. An outline of expected resource and time requirements for

both Organisational Readiness and Deployment Initiation stages is contained within section 14 of this

document.

3. NPEx Data Sharing Agreement

The National Laboratory Programme Regional Project Manager will support the Board in completing an

NPEx Data Sharing Agreement. This document should be agreed and signed off by the Caldicott Guardian

and/or Senior Information Risk Owner (SIRO) for the organisation prior to sign off of the Organisational

Readiness stage. It is expected that senior laboratories staff will assist the Project Manager in progressing

sign off of the Data Sharing Agreement by ensuring the relevant colleagues are briefed and provided with

information as required in order not to delay the sign off of the agreement. Upon sign off the data sharing

agreement should be returned to the Regional Project Manager who will progress as necessary with THIS.

4. Laboratory Workflow Mapping

The introduction of NPEx should aim to work alongside any current send away test systems already in

place. Existing systems/processes will remain in place for rare tests, those not currently on NPEx e.g.

further biochemistry and wider disciplines and as contingency for any NPEx downtime. As part of the

Organisational Readiness stage the Project Manager will arrange to attend the laboratory to map the

current send away test process and sample reception procedures. This process mapping will assist in initial

identification of benefits to be gained from NPEx which should include improved data quality and reduction

of staff time. A baseline audit of current processes should be carried out by the labs staff before

implementation; this will then be re-audited post operational go-live.

Workflow Mapping for Sending Laboratory – a ‘walk through’ of the system currently in place for

biochemistry send away tests. The Regional Project Manager will facilitate this process with the

laboratory staff and be able to identify current processes that may change with the introduction of

NPEx.

12. Stage 2- Deployment Initiation

Upon sign off of Organisational Readiness, Deployment Initiation will commence and will require input from

IT/Networks Teams and eHealth. The deployment initiation stage is outlined within figure 9 with further

detail provided on each of the steps below.

5. Start Up Meeting

A Start Up Meeting will be arranged by the Project Manager and will require representation from the Board

Laboratories Team, Board IT/ eHealth, NPEx supplier X-Labs and the appropriate members of the National

Programme Team. The Project Manager will confirm progress made so far and look for the Board

representatives to update in regards to Change Acceptance Board (CAB) processes and required VPN

configuration.

6. IT Change Acceptance Board/ Change Advisory Board (CAB)

Each local Board should submit any required CAB documentation as per organisational process; the

Regional Project Manager can assist with this. As above the Regional Project Manager will document the

local change control procedure at the induction meetings in order to ensure documentation is submitted in

line with project timescales and organisational processes. In order to ensure the process, timescales and

resource availability are fully understood from the outset it is beneficial to have eHealth/ IT representation in

attendance at Start Up and Induction meetings. Throughout Deployment Initiation the requested change

should be submitted to and approved by the local NHS Board CAB (if necessary).

7. VPN Tunnel Configuration

A VPN tunnel is required to be set up to allow the laboratories LIMS to communicate messages to NPEx.

NPEx advise that they connect to NHS Boards using a VPN rather than just using N3 due to extra security

as although the N3 is secure for the NHS it can still be compromised so the best course of action is to

encrypt the traffic from end to end to ensure no PID is sent unencrypted. The VPN interface will require to

be set up by IT/ Networks specialist/s within the Board in collaboration with X-Labs and The Health

Informatics Service (THIS). In order to enable the VPN connection the Project Manager will supply VPN

documentation to be completed and returned to X-Labs. Following on from the return of this document a

call will be set up between the IT/ Networks lead and suppliers in order to set up and ‘ping test’ the VPN

connection.

To ensure an understanding of current IT facilities and pre-empt any issues with the facilitation of the

required VPN tunnel it is beneficial to include IT/ eHealth colleagues at initial meetings. This will also assist

in understanding resource availability and capacity to implement the change and will ensure the timeline for

completion of work is realistically scheduled.

Please note: NPEx requires messages from NHS Boards to be sent from a 10.xxx.xxx.xxx IP address. If

NHS Boards utilise a 192.xxx.xxx.xxx IP address the Board will be responsible for configuring the set up of

the VPN to NAT from a 10.xxx.xxx.xxx IP address.

8. LIMS Interface

Conduits are required to be installed into the LIMS demo environment in order to allow data to be

transferred from a sending laboratory LIMS through NPEx and to then be received by the receiving lab

LIMS and vice versa. The conduits require to be installed by the LIMS provider at a time agreed by the

Board. Although it is not usual for any downtime to be required for installation of the conduits the labs team

should be aware of this work being carried out and time mutually agreed. All NHS Scotland Board LIMS

require to be set up to facilitate both outbound’ requests out, results in, ‘incoming’ requests in, and results

out. This may require additional licences to be purchased from the LIMS provider or the Board may be able

to utilise existing dormant licences. If additional licences are needed the Regional Project Manager will be

able to facilitate discussions with the LIMS provider and ensure these are available in line with project

timescales.

interface and raising any queries with the LIMS vendor at this early stage should reduce the need for

patches to the interface later.
