All applications must be received at CPSP Head office, Karachi, its Regional Offices on or beforethe dates mentioned above. Delays in postal deliveries shall not be considered a valid reason forthe late receipt of application.For further correspondence, please note to mention your name, subject and Examination centrefrom where you are appearing in the examination.

FOR FRESH/REPEATER CANDIDATES

EXAMINATION FEE RS. 15000/-3Examination fee, as mentioned above is to be deposited along with the application form onenclosed challan form in:

United Bank Ltd., CPSP Booth, KarachiDesignated branches of UBL in the country (list enclosed)

or bank draft/pay order payable in the name of CPSP. Postal orders, money orders or Personal Cheques are not acceptable.

REFUND OF FEESeventy five percent of the examination fee shall be refunded to the candidate after deduction of 25% fee forprocessing, subject to:

receipt of application for withdrawal from the examination if received on or before the specified date.rejection of the application by CPSP on account of ineligibility.While applying for REFUND candidate should write his / her NAME, CENTRE, SUBJECT and BANKRECEIPT NUMBER.

In all other cases the fee paid for a particular examination will not be refunded or carried over to nextexamination in case of withdrawl after specified date, absence/exclusion from the examination forany reason. Training must have been completed one month before the date of theory examination i.e.by September 09, 2012.

SUBJECTS FOR OCTOBER 09, 2012 EXAMINATIONS

Candidates who qualify in Theory examination can appear in three out of four consecutive TOACSexaminations (including missed chances) without appearing in theory examination again.A candidate who does not appear in the immediate following TOACS examination after passing theoryexamination will lose one chance. Only two of three consecutive chances will be available to such candidates.

VENUE OF EXAMINATION

Application forms issued for OCTOBER 2012 examination only will be accepted. (Photocopies of the application forms are not acceptable).

PLEASE NOTE THAT SEPARATE APPLICATION FORMS ARE TO BE USED BY FRESH APPLICANTS & REPEATERS.

THEORY: The theory examinations are held simultaneously in thirteen different cities of Pakistan. The candidates have an option tochoose one of these venues for appearing in the theory examination according to their convenience. The choice of the city shouldbe clearly written in the appropriate box in the application form. The CPSP reserves the right to delete a particular centre, or shiftthe candidates from one Centre to another on account of logistic reasons.

TOACS: The College will hold TOACS examination ONLY in one or more cities of PAKISTAN / SAUDIAARABIA depending upon thenumber of candidates eligible to take this examination and logistic facilities available in that city irrespective of the venue at which acandidate appeared in theory examination. Exact Venue of TOACS examination will be communicated after the assessment of theoryexamination papers.
