NHS North Tyneside CCG— Hydration Monitoring Solution
Working together to maximise the health and wellbeing of North Tyneside Communities by making the best use of resources
North Tyneside Clinical Commissioning Group are working with 18 Nursing Homes in North Tyneside to develop a mobile hydration monitoring solution (Hydr8) to raise the
quality and safety of residents care.
The project aims:
To raise awareness of the importance of ensuring that nursing home residents are adequately hydrated.
To accurately complete and communicate fluid balance status for all identified residents.
To accurately monitor the number of residents receiving sub cutaneous fluids.
To automate the recording of fluid balance maximising the use of accessible technology.
To enable care home staff to see the cumulative totals for each patient at a glance and be alerted when residents appear to be falling behind optimum levels.
To enable GP/ named clinician to access real-time information on residents.
To reduce hospital admissions with dehydration from nursing homes.
To release time of nursing home staff to care and lead.
This collaborative development lead by North Tyneside Clinical
Commissioning Group includes stakeholders from Nursing Homes, North
Tyneside Local Authority and Nine Software.
This innovation is supported by the Academic Health Science Network, the
delivery and design branding aspects will be delivered by North Tyneside
CCG in partnership with Nine Software Company Limited
