Product Defect Module Detected In Description Resolution
Practice Management 150394 RTS 5.8 UD1 Unable to run memorize or elig/ref report. Report
crashes with IRC 8152 When remove the resp_status it
works. Client uses RTS reader which pulls additional
response status. Client uses RTS reader to deposit
additional response status.
Steps to recreate:
1. Log into Practice Management
2. Go to Reports > Scheduling > Appointment Listing
3. Under columns, make sure RTS Resp Status is
selected
Note: There must be a RTS Rep Status over 30
characters.
4. Click OK.
Now, the user will not receive an iRC = 8152
error message when a RTS Response status is
greater than 30 characters. This hot fix has no
impact on other areas of the application. Refer
to the Steps to Reproduce for more information.
Ambulatory EHR 150347 Template Runtime 5.8 UD1 The Clear button does not clear data from the Describe
plan/attempt field under the Risk Assesment panel.
Steps to Reproduce:
1. Create a template with a Note field on.
2. Add a button and add the Assign Note Field trigger
with empty string ("").
3. Open this template, type some string in the Note
field, save, and close it.
4. Reopen this template from the same encounter, click
on the button that has the trigger and then close the
template.
5. Reopen the same template again.
Actual result: The Note field retains the old value that
was typed in step3.
Expected result: The Note field is empty.
The Note field does not save after the Assign
Note field value triggers. This hot fix has no
impact on other areas of the application. Refer

Document Management 149659 Batch Filing 5.8 UD1 In NextGen Document Management, while filing
multiple documents, if the user navigates between
document pages, an error displays.
Steps to Reproduce:
1. Select multiple files and Acquire them through the
File Import option.
2. Select multiple files and open a document type with
a custom field (preferably free text).
3. Select a patient from the Person Lookup button.
4. Go to the viewer and select Move to first page.
5. Go back to the index form and type some free text
on the custom field.
6. Go to the viewer again and select Move to next
page. An object reference error appears.
In NextGen Document Management, while filing
documents multiple documents, if the user
navigates between document pages, it does not
display any error. This hot fix has no impact on
other areas of the application. Refer to the
Steps to Reproduce for more information.
Ambulatory EHR 141272 Menu Items 5.8 UD1 After the UD2 upgrade, alert comments are not tied to a
specific alert and that they are not shown in the Alert
Module or Patient Information Bar. For example,
alerts_mrp_.txt_comments does not display in Alerts
module following the UD2 upgrade, KBM Version
8.3.8.1.
These are alerts that are not tied to a specific alert in
the template. There is no place to store/view general
comments.
Steps to Reproduce
1. Prior to the upgrade, add an alert comment in the
alerts_mrp_ template.
2. Upgrade to UD2 HF01.
3. Verify the general comment from the alerts_mrp_
template. It is displayed as a Chart alert.
General Alert comments are mapped to the
User-Defined Chart alert type. This hot fix has
no impact on other areas of the application.

Ambulatory EHR 150664 Document Generator – Tx Text 5.8 UD2 Even though no immunization has been marked as
Completed, a blank Immunization section shows up in
the document.
Steps to reproduce:
1. In EHR, create a patient record > set the gender to
male and age to 35.
2. Create an encounter for the patient.
3. Open *Intake and set any Specialty and Visit Type.
4. Click on the Care Guidelines link to open the Care
Guidelines template.
5. Go to the Clinical Guidelines panel and click the
Add/Update button on the grid.
6. In the Active Guidelines panel, click in the Condition
field and select Health Maintenance from the picklist.
7. Select No risk indicator from the Risk Male popup.
8. Click Save and Close on the Activate Clinical
Guidelines template to return to the Care Guidelines
template. TD vaccine appears in the CG grid.
9. Highlight the TD vaccine in the Clinical Guidelines
grid and click the Order button on the grid to launch the
Immunizations module.
10. Search for the following vaccines with the exact cpt
and cvx code and mark them as Partially Administered:
• Td – cpt - 90714 (cvx - 113)
11. Click Save and close the Immunization module
window. The Clinical Guideline template refreshes.
12. Close the Clinical Guidelines template and click the
Preview button to generate master_im document.
Actual result: The Immunizations section is shown in
the document even though no immunization has been
marked as Completed.
Expected result: Immunization details should be shown
in the document only if all the immunizations are
marked as Completed.
The immunization macro does not detect the
immunization section in the document when no
immunization is marked as Completed. If the
immunization is marked as Completed, it shows
up in the document. This hot fix has no impact

Ambulatory EHR 150363 Template Runtime 5.8 UD2 Updated the ROWID() function to return current
sequence number on opened extended popup.
Steps to Reproduce:
1. Use ROWID() function without any parameters on an
extended popup.
2. Assign a label or field on populate the ROWID
function.
* Without the fix the rowID will not return anything.
* With the fix the row id will return the current navigated
record on the extended popup
Set all proper field styles when recreating edit
fields when the change field style trigger is run.
This hot fix has no impact on other areas of the

Steps to Reproduce:

1. User selects a dx.
2. Insert – Patient Diagnosis.
3. Various SPs that update that new Patient Diagnosis
record.

This can assign field value triggers or an alert box for
the sake of testing

Diagnosis’ trigger associated to it. Select V22.1 as the
Dx. Save the template.
3. To the same button (E.g.: “Insert Pat Dx”), associate
an assign field value trigger to add a value “Diagnosis
Added” to the edit field. Move the 'Assign field value'
trigger below the 'Patient Diagnosis' trigger using arrow
buttons so that it executes after the 'Patient Diagnosis'
trigger.
4. In EHR runtime, add a medication E.g.: Coumadin.
Stop trigger checking when DUR dialog is open.

Ambulatory EHR 145074 Medications Module 5.8 UD1 When a user clicks on the “Prescribe New” button,
there is a delay of 1 to 3 minutes before the Medication
Search window appears. This happens only when a
provider has a large amount (For example, 1500) of
medication favorites saved. This error does not occur
for providers who have saved 20 to 30 medication
favorites. This is due to a query that is being fired off for
each medication favorites and this query executes once
for 5.8 UD1 and twice for 5.8 UD2 for each medication
listing, whenever the Prescribe New button is pressed
and the Medication Search window is opened.
Steps to Reproduce:
1. Open a patient’s record from Ambulatory EHR.
2. Click the Medications Module.
3. Press the Prescribe New button.
The main SQL query is modified to include
PDR BRIEF indicators so that no additional
subqueries are performed for each favorites
medication listing. This hot fix has no impact on
other areas of the application. Refer to Steps to
Reproduce for more information.
Practice Management 145789 Form Templates 5.8 UD2 Selecting the Flat Fee option in the Late Fee Options
Form only zeros out the Percentage display, but it does
not zero out the percentage value in the background,
so the percentage stills gets applied to the late fee.
Steps to Reproduce:
1. Go to File/Print Forms/ Statements and click the
Late Fee button.
2. Select Percentage Setting and set the percentage
box to a percentage other than zero.
3. In the populated percentage box, change the
Percentage Setting to the Flat Fee Setting.
Note: The percentage box zeros out only the display.
4. Click OK. The percentage gets applied to the late fee
instead of the Flat Fee.
Set the Percent Box to 0.00 when selecting the
flat fee. The statement no longer runs late fees
in percentage mode. This hot fix has no impact
on other areas of the application. Refer to Steps

Ambulatory EHR 150256 Template Runtime 5.8.2.74 Whenever a user adds a common assessment, opens
a custom template with the medication summary sheet,
and double-clicks and launches the module, the
Ambulatory EHR crashes.
Steps to Reproduce:
1. Open *SOAP.
2. Click on My Plan.
3. Add Common Assessment.
4. Close the template completely.
5. Open the custom template with the medication
summary sheet.
6. Double click on the summary sheet. The EHR
crashes.
Ambulatory EHR does not crash when a user
does subsequent user actions such as add a
common assessment, open a custom template
with the medication summary sheet, and double-
click and launch the module. This hot fix has no
impact on other areas of the application. Refer
to Steps to Reproduce for more information.
Ambulatory EHR 149134 Other 5.7.x When you have documents or scanned images that go
into multiple view categories and you select one of
those documents or images from one category, the
focus now sets to first category.
Steps to Reproduce:
1. In NextGen File Maintenance > View Categories >
Add a document into several different categories.
2. Log on to NextGen Ambulatory EHR.
3. Open a patient and create a new encounter.
4. Click Documents icon, Click Generate and save the
document.
5. Select the Categories tab.
6. Expand the last category the document is in.
7. Open the document.
8. The focus will now jump up to the first category in the
list of categories the document is in.
When you have documents or scanned images
that go into multiple view categories and you
select one of those documents or images from
one category, the focus now stays on the
selected document or image. This hot fix has
no impact on other areas of the application.

System Administrator 149948 System Administrator 5.8 UD2 When a user tries to change the User password in
NextGen Ambulatory EHR, an unhandled exception
error occurs.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Click Admin.
3. Click User Password.
4. Enter Old Password > Enter New Password >
Confirm New Password.
5. It will update the new password but you will receive
an unhandled exception error.
Actual Result: Password updates but exception error
occurs.
Expected Result: Password updates with no error.
The password updates successfully without any
error, when a user tries to change the User
password in NextGen Ambulatory EHR. This
hot fix has no impact on other areas of the
application. Refer to the Steps to Reproduce for
more information.
Practice Management 149772 Statements 5.8.2.74 The Statements cannot be printed when the printer
toner is low.
Steps to Reproduce:
1. Log on to NextGen Practice Management.
2. Go to patient’s chart that has a balance.
3. Click the Statement icon.
4. Click Print.
Actual Result: User cannot print the statements when
printing from a specific printer.
Expected Result: The User should be able to print the
statement even when the printer toner is low.
The user can print the statements even when
the printer toner is low. This hot fix has no
impact on other areas of the application. Refer

Ambulatory EHR 143272 Document Builder - High Edit 8.x The Document Builder Immunizations macro did not
include the information such as Manufacturer, DateVIS,
VersionDateVIS, CVXcode, and LotNumber.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Click Tools > Document Builder.
3. Open a new document.
4. Click Tools > Macro Builder.
5. Click Create New Macro > Summary Macro >
Standard > Immunizations and add one to the
document.
6. In your macro, Click Edit in the Columns section.
The Macro Column Select dialog box displays.
All the required columns has been added to the
Document Builder Immunizations macro such
as Manufacturer, DateVIS, VersionDateVIS,
CVXcode, and LotNumber. This hot fix has no
impact on other areas of the application. Refer

Ambulatory EHR 148822 Template Runtime 5.8.2.74 In My Plan dialog box, when you click the diagnostic
line items under Today’s Assessments, the screen
flashes for a few seconds before opening the NGKBM
GetDxGroupByReference selection dialog box.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Add data grids with multiple records to display.
3. Set the grids to sort on one more columns in the grid
properties.
4. Open and refresh the grid then there will be an extra
flash compared to the templates. Without grids set to
use sorting.
Delay the template drawing when showing or
hiding the multiple fields to draw after all
showing or hiding is completed. This hot fix has
no impact on other areas of the application.

Ambulatory EHR 145378 Provider Approval Queue 5.8.2.74 HF22 When you launch a New Task from the PAQ tab, there
is a GDI object leak within the Assign To window. 5 –
10 objects are lost.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Click the Inbox icon and go to the Provider Approval
Queue tab.
3. Select the + To Do button.
4. Click Add under assign too.
Expected Result: There should be no GDI object leak
within the Assign To window.
Actual Result: There is a GDI object leak within the
Assign To window.

Ambulatory EHR 148693 Workflow Module 5.8.2.74 In Clinical Tasking tab, when you select the Chart
button from the All Tasks and then re-open the
workflow the patient at the top of the task list is
highlighted.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Click the Inbox icon.
3. Select the Clinical Tasking tab and click the All
Tasks filter.
4. Highlight a patient task in the list and select Chart.
5. Re-open the Inbox. The same patient in the list is
highlighted.
7. Select a different filter from the list of filters.
8. Highlight a patient task in the list and select Chart.
9. Re-open the Inbox icon.
The focus is now on the first patient in the filter task list.
Expected Result: When a filter is applied, the Workflow
module should highlight the previously selected task
upon re-opening.
Actual Result: When a filter is applied, the workflow
module highlights the top task upon re-opening.
In Clinical Tasking tab, when you select the

Ambulatory EHR 148083 Template Runtime 5.8.2.74 The Pain Management panel in the Health Promotion
Plan dialog box when accessed from the pain plan link
in vital signs freezes the NextGen Ambulatory EHR
when you enter the quality of pain.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Click Add in vital signs panel to open vital signs pop-
up.
3. Select the Pain Plan link.
4. Enter fields in Pain Management panel. (Pain scale,
Method, Location, Onset, and Duration).
5. When user save and close the duration pop-up, the
quality pick list auto opens and the template does a
refresh that causes a big white space in the panel.
(Trying to select the quality of pain from the pick list
moves the user out of EHR and the EHR session
freezes.)
The Pain Management panel in the Health
Promotion Plan dialog box when accessed from
the pain plan link in vital signs does not freeze
the NextGen Ambulatory EHR when you enter
the quality of pain. This hot fix has no impact on

Practice Management 147561 Billing 5.8.2.74 HF7 Claim edit tasks are being generated when the check
box Create tasks during claim edits is not selected in
Enterprise Preferences dialog box. The tasks are
created after an encounter is saved and Claim Edits
are run in NextGen Practice Management.
Steps to Reproduce:
1. Log on to NextGen Practice Management.
2. Click Admin > Preferences > Enterprise and clear
the Create tasks during claim edits check box.
3. Create a claim edit task type in NextGen File
Maintenance.
4. Open your claim edit library that is in use for your
practice, turn on claim edit 173.
5. In the task types column, right-click and select Open.
Select your claim edit task you just created in step 2.
6. In NextGen Practice Management, create a new
encounter for a test patient and select a provider.
7. Add a new charge for the encounter with a different
provider.
8. Click the life saver icon on the main toolbar of charge
entry to run claim edits.
9. Note that the edit 173 fired off in red and should
state you need to correct the difference in providers.
10. Click Work Log on the main toolbar.
11. Select Claim Edit Task Types, click find.
12. Note that a task has been generated for the
encounter.
Claim edit tasks are only generated when the
check box Create tasks during claim edits is
selected on the Enterprise Preferences dialog

File Maintenance 142923 Master Files - Practice 5.8 UD2 The old data field Guar_Name of the Version 5.8,
printed the first name and then the last name on the
Budget Letters. The new data field Account Guarantor
Name in the Version 5.8 UD2 prints the last name with
a comma and then the first name.
Steps to reproduce:
1. In File Maintenance, open the account letter to edit.
2. Add the Account Guarantor Name data field and
save the letter.
3. Log on to NextGen Practice Management and open
a patient's account.
4. Select the letter icon, select the letter that was
edited, right click and preview. The account guarantor’s
name displays with the last name and then the first
name.
Actual Result:
The old data field Guar_Name in the Version 5.8
printed the first name and then the last name on the
Budget Letters. The new data field Account Guarantor
Name in the Version 5.8 UD2 prints the last name with
a comma and then the first name.
Expected Result:
The Guar_Name field should display the name in the
First Last Name format.
The Account Guarantor Name First Last field is
added in the data repository under the Account
Information section for the budget/demand
account letter for printing the Guar_Name in the
First Last Name format.
Also, the Guarantor First Last Name is added in
the data repository for demand encounter
letters for printing the Guar_Name in the First
Last Name format. This hot fix has no impact

Ambulatory EHR 146023 Communication Component 5.8 UD1 When some text from a workflow is copied and pasted
into Communication Component template, if that text
has some special characters in it, the
Infragistics2.grid.v10.3 crashes and the Red X displays.
Steps to reproduce:
1. Log on to NextGen System Administrator and make
sure that your user has an assigned provider to access
the workflow.
2. Open the Workflow.
3. Select any workflow document and type a comment
on the right side.
Note: Keep the comment text to be short but with
several lines of text. Make sure you use the Enter key
to create a new line.
4. Create a task for yourself using this comment.
5. Open a patient and open a template with the
Communication Component.
6. Create a new call as Other.
7. Fill in the Call Info and select Communication.
8. Click on the Comments textbox.
9. Open Inbox and click on the task that is created
earlier.
10. Select the all the text that was created in step #3.
11. Copy the text and close the Inbox.
12. Navigate back to the Communication Component
template, paste the copied text to the Comments
textbox, and click Add. The Red X displays instead of
the grid.
Expected Result:
The application checks the pasted text for invalid
characters and replaces them before displaying on the
grid.
Actual Result:
When some text from a workflow is copied and pasted
into Communication Component template, the
Infragistics2.grid.v10.3 crashes and the Red X displays.

Ambulatory EHR 149247 Template Runtime 8.3.8 In the Intake – Cardiology template, under the Reason
for Visit grid, if you select the Absent check box for
CAD, CHF Symptoms, Arrhythmia Symptoms, Vascular
Symptoms, and Valvular Symptoms and when you
clear the check box, it does not clear the data in the
grid but retains the data in it.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Select the Specialty as Cardiology and Office
Consult for Reason for Visit.
3. Select the Absent check box for CHF Symptoms and
clear it.
4. Notice the data in the grid doesn’t clear out upon
clearing the Absent check box for CHF Symptoms.
Expected Result: Data should be cleared out from grid.
Actual Result: Data still remains in the grid on clearing
the check boxes.
In the Intake – Cardiology template, under the
Reason for Visit grid, if you select the Absent
check box for CAD, CHF Symptoms,
Arrhythmia Symptoms, Vascular Symptoms,
and Valvular Symptoms and when you clear the
check box, it clears the data in the grid. This hot

Practice Management 137186 Reporting 5.7.2.118 HF60 The Payments with Reason Code By Line Item report is
not pulling in qualifying encounters based on report
parameters.
Steps to Reproduce:
1. Log on to NextGen Practice Management.
2. Go to Reports > Daily > Line Item > Reason Code >
Select Payments.
3. Set up Parameters
4. Click OK.
5. Report Results.
The Payments with Reason Code By Line Item
report is now pulling in qualifying encounters
based on report parameters. This hot fix has no
impact on other areas of the application. Refer

Practice Management 149064 Reporting 5.8.1.28 Some of the ICD-10 codes do not export from the Daily
Charge report. When you Export to Excel the value was
in date format instead of ICD-10 codes such as Z01.01,
Z01.21 and so on.
Steps to Reproduce:
1. Log on to NextGen Practice Management.
2. Load the ICD-10 diagnosis library.
3. Create a test encounter and add a test charge for
that encounter.
4. Select the Diagnosis code of Z01.21.
5. Click Save.
6. Go to Reports > Daily > Charges.
7. Select the Diag 1, Diag 2, Diag 3, and Diag 4
columns.
8. Click OK.
Note that the Z01.21 diagnosis is listed correctly.
9. Click the Export to Excel button and click Save.
10. Open the file in Excel, and note that the Z01.21
diagnosis code has been changed into a time date
format.
Actual Results: The Z01.20, Z01.21, Z01.30, Z01.31
ICD-10 diagnosis codes export into excel as dates.
Expected Results: The diagnosis codes should be
exported to Excel as they are.
When you Export to Excel the Dialy Charge
report, the Diag columns displays the ICD-10
codes as it is. This hot fix has no impact on

Ambulatory EHR 148478 Workflow Module 5.8.2.74 In NextGen Ambulatory EHR, the new Inbox or PAQ
introduced in 5.8 UD2 upgrade executes a continuos
refresh at the specified user preference for the first time
even when the window is closed. This causes
excessive executions of the respective stored
procedures.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Click the Inbox icon.
3. Click the Clinical Tasking tab.
4. Click the Settings icon and select Preferences.
5. In Task Preferences dialog box, select Every
Minute(s) option.
6. Close the Workflow module.
7. Run a SQL trace and look for the "EXECUTE
ng_get_todo_tasks" to run.
8. With the fixed version, the tasking stored procedure
will only run if the Workflow window is currently visible.
In NextGen Ambulatory EHR, the Inbox or PAQ
does not refresh when the Workflow dialog box
is hidden. This hot fix has no impact on other

NextGen Share 145677 Share Provider Search 5.8UD1 The NextGen Share provider search overwrites the
contact data for a provider in File Maintenance >
Master Files > Providers. This includes information
such as name, address, fax, phone, email, and
taxonomy code.
Steps to Reproduce
1. In File Maintenance > Master Files > Providers,
create or modify a provider’s contact information such
as Name, Fax Number, Address, or Phone Number.
2. In NextGen Share directory, search for the provider
that you modified or created in File Maintenance.
3. Select the provider from the NextGen Share
directory.
When a provider is created or modified in File
Maintenance, the NextGen Share Provider
Search component has been modified to not
overwrite any provider data created or modified
in File Maintenance (File Maintenance > Master
Files > Providers).

component has been modified to prevent the
error message "An item with the same key has
already been added." This message was
occuring when activating the search screen and

organization have the same Direct address and
added to the user’s Favorites list. This hot fix

2. After Refill request has been received for patient;
Launch the Workflow Module.

5. Select deny option then select "Denial - new
prescription to follow.
The Comments label has been renamed “Non-
Clinical Notes to Pharmacy" and the alert
message, “Notes to the pharmacy should be
non-clinical” now displays in the Send

1. Set the medication practice preference to "eRx:

2. Prescribe a brand medication such as Medrol (Pak)
4 mg oral tablets, dose pack.
3. Erx the medication.
4. The send medication's name is
METHYLPREDNISOLONE 4 mg TAB DS PK.
5. Surescripts expects the name to be
methylprednisolone 4 mg tablets in a dose pack.
The code was modified to resolve the defect.

Steps to Reproduce
1. On the Encounter Lookup dialog box search for an
encounter.
2. Select the encounter, right-click, and click Balance
Control.
3. On the Balance Control dialog box click the Prorate
button.
When the encounter has a discount, then the date error
occurs.
When clicking on the Prorate button on the
Balance Control dialog box, if there is a
discount on the encounter, the discount is
reversed and a new discount is created with the
current date. This hot fix has no impact on other

PM 147500 Statements 5.8 UD2 When printing statements in batches with more than 30
statements, the system freezes and displays the error
message, "Insufficient memory to continue the
execution of the program."
Steps to Reproduce
1. In Practice Management, click File > Print Forms >
Statements.
2. On the Statement Options dialog box, select a batch
of statements equal to or larger than 30.
3. Click Print.
When a printing statements in batches with
more than 30 statements, the statements print
successfully. The system does not freeze and
an error message is not displayed.

Practice Management 147689 Statements 5.7 When an account meets the criteria to reset the
statement counter, it does not reset to zero.

In Practice Preferences Statements, do the following:

2. Select the check box Acceptable % of self-pay
balance and enter the applicable percentage.
3. Preview a statement and check the balance due.
4. Apply a payment to the charge that meets the criteria
for the statement counter.
5. Apply an adjustment for a patient with a discount for
making a timely payment.

Preferences Statements to reset the statement
counter, the statement counter resets to zero.

1. Open EHR, select a patient>go to the Orders
Module.
2. Order a Lab.
3. Add results for that order. Then select a collection
date time and a result and save and close the module.
4. Open the module again. Then select the same order
and enter the results for it again by keeping the same
collection date time (make sure it matches the one you
inserted before) and select Save and Close.
5. Go to the Results tab.
6. If you don't see the results, click Refresh and notice
the results.
7. Notice different results each time you change the
view results by selection for the entered collection date
time.
Note: The Results tab should always display the most
recent results for the patient.
Code changes were made to display the most

Practice Management 147713 Statements 5.8.2.74 When running batch statements in preview and then
clicking the print button, the statements print. When
you
are prompted to confirm if they printed OK, you get
errors and the statement print date does not update.

1. Log into Practice Management.
2. Go to File > Print Forms > Statements.
3. Click the Preview button on the Statement Options

7. Bill the encounter.
8. View the Claim – The first charge line will show
the authorization or referral, but subsequent
charges are blank.

range, but a separate line for each CPT code on the
same authorization or referral. All will be blank.

3. Click on Scheduling.

1. From Patient's Chart, go to the Clinical
History/Notes tab.
2. Click on Appointments on the left side of screen
under Topics.
3. Right-click on one the appointments and select
Recall.
4. From the Patient Recall window, right-click on one
of the appointment and select Print > 1st Letter.
5. Close and go back to the Patient Recall window.
6. Highlight the next Appointment, and right-click and
select Print > 1st letter.
7. In Recall Plans under PM Master, search for Recall
Plans you just printed in PM for sample Patient.
8. Click Open, notice the information under Mailing
Information.
9.Click Ok close.
10. Click on Letters under PM Master.
11. Find Recall from the drop-down menu for Letter

3. Open *Soap, select specialty and visit type.
4. Open *Chronic Conditions.
5. Select one of the problems. If prompted to map an
ICD code, select one.
6. Type something in one of the text boxes. Select
Add/Update. Notice the text box does not clear.
7. Select a different problem.
8. Without making any changes or clicking anywhere
on the template, select a different problem.
The template fields flash, but the newly selected
problem details do not load the template. The User
must select the item again in order for the details to
appear.
The users are now able to select and edit a
problem from the Problem Description list on
the left pane of the Chronic Conditions template
the first time they select it. This hot fix has no

2. Search and select a provider from the share
directory.
3. After returning to the referral template, re-open
Provider Search.
4. Click the star icon to favorite the provider.

search window. The provider’s star icon is not Yellow.
6. From the recently used view, click the star icon to
the provider. The error displays.

click and select New. The Encounter Insurance
Selection dialog box displays
4. Double-click the Authorization. The Insurance
Maintenance displays.
5. Enter an encounter Authorization and click OK.
6. Double-click the Referrals, enter an encounter
Referrals and click OK.
Expected Result: When you open the Insurance
Maintenance dialog box, the Authorization and
Referrals tabs will have a pushpin and the information
will be populated in each tab.

memorized report and then hover over a patient. The

Ambulatory EHR 146354 Workflow Module 5.8.2.74 HF1 Double-clicking on assigned task in the Inbox does not
prompt you to accept the task before editing patient’s
chart. When a task is sent to multiple users or a group,
user double clicks on the task and now the chart opens.
The user is able to work the task without having
accepted the task (multiple users can now work the
same chart simultaneously). In 5.8 UD1, the user would
be prompted to accept the task, and the task would
drop out of all other users' workflow task area.
Steps to Reproduce:
1. NextGen Ambulatory EHR > Inbox > New > assign to
two other users.
2. Log on as the other users to confirm new task is in
Inbox.
3. Without accepting the task, open task by double-
clicking the task.
4. Both users can open the task and edit the patient
chart simultaneously.

This will warn users if they are opening up a patient

To fix this, a new user preference was added to

automatically accept it and go to the chart. The
preference defaults to not set (unchecked). If
the preference is not set, the application
prompts them if they want to accept first when
they double click on a task to accept. An option
was also added to Accept and Chart just like
the Accept and Complete option. You can only
Accept and Chart if the task has a patient

Template > Practice tab > Ngkbm Config.
2. Scroll down to the Telephone Call Tasking section
and confirm that "Attach Telephone Call template" is
NOT selected. Save the template if a change was
made.
3. Open a patient's chart and create a new encounter.
4. Open the Telephone Call template and select a
Specialty.
5. Select Medication Management.
6. Under Tasking, select Send & Close then select a
user, click Add, and click OK.
7. Save the telephone call template and close it.
8. Open the inbox.
9. Double click on the new task.
10. On the Medication Management Popup, click
"Telephone Call Summary".
11. Click "X" to close both popups.
Actual Results: NextGen Ambulatory EHR becomes

user and also has the Always prompt for recipients
option enabled, the task does not automatically get
sent to the original user indicated (i.e. the user does not
automatically populate in the Task Recipients grid. In
UD1, the original user would be included in the grid,
and then there was an option for additional recipients to
be added.
Steps to Reproduce:
1. Log on to Template Editor and save a new test
template.
2. Add a new button to the template.
3. Right-click on the button and select Triggers, and
then select Add New.
4. Confirm that the event is set to Button Click and the
action is Add New Task.
5. Set the Due Date field .
6. Click on the ellipsis button next to Assign To and
select a user.
7. Check Always prompt for recipients.
8. Click OK to save all popups and save the template.
9. Log on to NextGen Ambulatory EHR and open a
patient's chart.
10. Open the new template and click the button with the
tasking trigger.
Actual Results: The user from step 6 will not be
included in the "Task Recipients" grid and the task will
not be sent to that user unless they are specifically
selected in the Select Task Recipients window in

2. Open chart/clinical history/recalls.
3. Add new recall plan.
4. Right-click recall plan to stop.
5. Recall plan disappears from chart.
6. In previous versions the stop sign would show.
7. Same as above on appointment edit book.
8. Report can be run for inactive recalls.
Actual Result: Recall plans have been stopped and you
are not able to see any of them in the Clinical
History/Notes.
Expected Result: When you stop a recall plan, the
recall plan should not be removed from the Clinical
History/Notes tab of the patient chart. There should be
a stop sign icon in front of the stopped recall plan.
Whenever we stop a Recall Plan for a Patient,

3. Click Diagnosis Codes.
4. Pulls up Diagnosis Code list, and then double-click
Default Diagnosis Library.

The diagnosis code library has been updated
so that it will no longer load all the diagnosis
codes in the library upon initially opening a
library for edit or when a diagnosis code type is
selected in the search > filter section of the
form. Now when editing a diagnosis code library
one will have to perform a search in order for
the data to load in the grid. A label was added
at the bottom right of the grid that will indicate
how many codes are loaded in the grid out of
the entire library (xxxx of yyyy) where x is the
number of items in the grid and y is the number
of items total in the library that match the type

Practice Management 144861 Patient Chart 5.8.2.74 HF2 After upgrade to version 5.8.2.74, when entering
foreign addresses, the system does not retain any
alpha character in the Zip Code field.
Steps to Reproduce:
1. Open patient demographics.
2. Select Jamaica as the Country from field. This may
need to be added in File Maintenance > Master List >
Countries > Open Jamaica> Select Show in PM > Click
OK.
3. Type in zip code JMAAW08, and then press the Tab
key.
4. Type in Kingston for the City.
5. Select FC (other) for state.
6. Click OK.
When a foreign address that does not exist in
the Zip Codes master file is entered, the on-the-
fly form will pull in the country that was added
with the new zip code. This way the on-the-fly
form does not automatically assume the ZIP
code is a US ZIP code and therefore will not
remove the alphabetical characters. The hot fix

appears requesting the required fields to be completed
before data will be saved.
Expected Results: The 9-digit ZIP code should
automatically save without requesting additional
information because the majority of 9-digit ZIP codes
will be different, unless the new providers are living at
the same residence.

the first 5 digits of US ZIP codes when checking
to see if information for the ZIP code exists.

decimal point, the decimal point seems to be moved
over one unit to the left when mapping these codes to
ICD-10 codes. For example, E878.2 is mapped to
ICD10 Y83.2, but when looking at the load file, it shows
the code as E87.82. It also gets mapped this way in the
Diagnosis Codes library. This appears to be happening
for all of the E ICD-9 codes that have 4 characters
before the decimal.
Steps to Reproduce:
1. Log into File Maintenance.
2. Click on Libraries > Diagnosis Codes.
3. Open a diagnosis codes library that has no
mappings.
4. Search for ICD-9 code E878.2 and verify it exists.
5. Search for E87.82 and verify that it does not exist.
6. Close the library.
7. Right-click on the library > GEMs Update.
8. Find the GEMs file, and then click Load.
9. Once results show in the Mappings, search for
E878.2 and it will not find the code.
10. Search for E87.82. Note that it finds this code.
11. If you click on import to load the mappings and then
check the library for ICD-10 code Y83.2, you will see
that it is mapped to E87.82 and not mapped to the
actual ICD9 code of E878.
Modified code to account for the "E" codes to
correct the issue with the decimal appearing in

Steps to Reproduce:
1. Log on to NextGen File Maintenance.
2. Click Master Files > EHR > Enterprise.
3. Select the Enterprise which has atleast 2 practices
and click the Preferences button, and select the Show
DUR Across Enterprise check box .
4. Log on to NextGen Ambulatory EHR.
5. Select a patient. On the file menu, click Modify
Patient Information and clear the Enterprise chart
check box.
6. Click the Medication module and click Prescribe New
medication, which can be used for checking the DUR
Interactions.
Once the medication is moved to Active status, close
the patient.
7. Select another Practice within the same enterprise
using File > Change Enterprise/Practice.
8. Open patient lookup in the second practice, and
open the same patient for whom the medication was
prescribed previously.
9. On the file menu, click Modify Patient information
and verify the Enterprise Chart is cleared.

8. Wait for more than 1 minute. The status does not
refresh till you switch tabs and then come back to the

Steps to Reproduce:
1. Log on to NextGen File Maintenance.
2. Click Master Files > EHR > Enterprise.
3. Select the Enterprise which has atleast 2 practices
and click the Preferences button, and select the Show
DUR Across Enterprise check box .
4. Log on to NextGen Ambulatory EHR.
5. Select a patient. On the file menu, click Modify
Patient Information and clear the Enterprise chart
check box.
6. Click Medication module and click Prescribe New
medication, which can be used for checking the DUR
Interactions.
Once the medication is moved to Active status, close
the patient.
7. Select another Practice within the same enterprise
using File > Change Enterprise/Practice.
8. Open patient lookup in the second practice, and
open the same patient for whom the medication was
prescribed previously.
9. On the file menu, click Modify Patient information
and verify the Enterprise Chart is cleared.

2. Select a Patient.
3. Click the Medication module and click Prescribe
New.
4. Search for a medication and add the same to the
cart. Click Select.
5. Enter all the fields and click Accept.
6. Click the Procedure module and click the Procedure
lookup icon.
7. Select a Procedure and click OK.
8. Manually enter an ICD-10 code in the Diagnosis
fields, which should not be present in patient chart and
also should cause an interaction with the medication.

codes with the ICD-10 Mappings even after an ICD

then the ICD codes should not be removed from the
Encounter Dual Coding pop-up.
Steps to Reproduce:
1. Log on to NextGen Ambulatory EHR.
2. Select a patient.
2. Create a New encounter and click the Diagnosis
module.
3. Click the Add ICD button and add an ICD code.
4. Click Tools > Encounter Dual Coding and notice
mapping between ICD-9 and ICD-10. Click OK.
5. In the Billing ICD tab, delete the ICD 9 code.

This is a DBA change for the Provider Approval Queue
to improve performance. Test the EHR documents,
EHR images, orders, DM images, HIE documents,
Crystal Reports, and patient notes are displaying
properly in the Provider Approval Queue for pending
and rejected items.
In order to test, check how much times does the
ng_provider_approval_queue stored procedure takes
before and after the change.
In the Provider Approval Queue stored
procedure , replace dynamic sql and @table var
with #temp tables in order to avoid parameters
sniffing and sp execution plan and add indexes
to improve the performance. This hot fix does

Practice Management 143436 Statements 5.8.2.74 Clients on UD2 are having exporting issues with
statements. Special characters are creating extra
columns in the data file and is causing files unable to
be
processed. EDI has found the following information:
1. Special characters are creating additional columns
when exported. Thus pushing data within the columns
over, resulting in data being printed in the wrong
description fields.
2. The additional columns exceed the amount of
columns
able to be processed per format type. Files are no
longer
able to be processed by EDI.
3. The special characters are found in the charge
description. Due to the special characters in the charge
description, the export cuts the descripion into one or
more additional columns.
4. After going into the charge descrition within the
client's
PM, the known characters at this point are comma,
quotes, and pound sign.
5. As new files are caught, EDI is working with the
clients
to have them manually change the charge description
and create new files. This only resolves the issues
when
the charge is newly added. For patients that already
have the charge in their chart, they will not be able to
process by EDI. Clients are forced to print those
patients
demand statements, but still printing in invalid columns.
6. Clients that are not on UD2 but did the ICD10 HotFix
have also reported an issue.
7. The majority of charge descriptions causing the
issue
are reason codes that are IDC10 related. For example:
CO140. The desription in EPM has a (#) that is
seperating the field into 2 columns.
The code was updated to look for a delimiter in
the data field and headle it, before adding the
wanted delimiters in the export file. This hot fix
has no impact on other areas of the application.

Location field.
7. Observe the number of displayed results and if there
is an alert that not all or the results could be displayed.

6. Select a patient of your choice
7. Navigate to a template housing the NextGen Share
Provider Directory search template component.
8. Click the Template button to launch Share Provider
Directory Search.
9. Search for a provider by provider name in the Name
field.
10. On the search results, click the provider name to
select the entry.
11. Launch Share Provider Directory Search, clear
Share check box and select MY EHR check box under

1. Log on to NextGen Ambulatory EHR.
2. Select a patient.
3. In a new or existing encounter, open the Home Page
template.
4. If needed, complete the Specialty/Office Visit
information.
5. Navigate to the File > System/Practice Templates >
Practice tab and open Ngkbm Immunizations.
The template or NextGen Ambulatory EHR displays the

Ambulatory EHR 141964 Document Builder - Tx Text 5.6.9.12 The client would like to add KBM macro's for user
credentials. NextGen System Administrator has a field
for user "credentials"and often the medical assistants
and nurses need their credentials on the documents
they create. This would affect all users, auditors and
other outside sources that review any NextGen Clinical
documentation that contains information created from
the "current user" function in the Template Editor.
Business Reason: Current functionality allows to pull
the "current user" into data fields on a template. Adding
the ability to concatenate the credentials data to the
"current user" data would be valuable because in many
cases it is a med-legal requirement to list the license
information (RMA, CMA, RN, etc.) with a user's name.

3. Clicking this button will display the ICD-10 mapping
screen that is used in other parts of Ambulatory EHR.
4. If the code is already ICD-10 then no mapping is
performed.
5. If the code is ICD-9 then the mapped ICD-10 code is
displayed in case of 1:1 mapping.
6. If there are multiple mappings, this screen will allow
user to pick one ICD-10.
7. Once the mapping screen is closed, the order gets
updated with the mapped ICD codes.
8. Saving the order will update mapped diagnosis to
ICD-10.
If encounter is locked, order cannot be saved. User will
have to unlock the encounter.
The Orders Module now enables a user to run

replacing the selected link, it will add a new link to the
list. This will cause confusion to the user as they will
believe they are following the workflow to update a
saved ICD-9 to ICD-10 (or vice versa) link, when
instead this feature will retain the old link and add the
new modified link. In this scenario, the feature should
discard or overwrite the old ICD-9 to ICD-10 link when it
is updated.
Steps to Reproduce:
1. Access NextGen Ambulatory EHR and open a
patient's chart.
2. Open the Dx module and add the ICD code 982.4 to
the patient's chart.
3. With that encounter selected, open the Encounter
Dual Coding dialog box (Tools > Enc Dual Coding).
4. Click the button in the middle of the first row.
5. Select any ICD-10 code from the list on the diag
search screen.
6. Click the OK button on the search screen and on the
Encounter Dual Coding dialog box.
7. Go back into the Encounter Dual Coding dialog box ,
click the middle button and select a different ICD-10
code to be linked to 982.4.

A button on the Encounter Dual Coding dialog box will
appear blank (without a label) when re-assigning a

1. Open a patient’s chart and select an encounter.
2. Assign the following ICD codes to the patient’s chart
under the selected encounter: 982.4 and T52.3x3a
3. Open the Dual Mapping window and map the code
T52.3x3a to 982.4 (i.e. ICD-10 > ICD-9). Please be
sure not to map the ICD-9 code to an ICD-10 code.
Expected Result: The middle button will appear with a
left facing arrow indicating an ICD-10 to ICD-9
mapping.
Actual Result: The middle button will not have any
display on it (i.e. blank middle button).

consistency the reverse mapping should be automatic
as well.
Steps to Reproduce:
1. Find an ICD-10 code that maps to only one ICD-9
code. (i.e. S42.341K)
2. Access NextGen Ambulatory EHR and open a
patient's chart.
3. Select any encounter and open the Problems
Module.
4. Under the ICD Billing List tab, proceed to add the
ICD-10 code (S42.341K) to the patient's chart.
5. Click to view the Encounter Dual Coding screen
either via the button on the Ambulatory EHR toolbar, or
from Tools > Enc Dual Coding.
Expected Result: The ICD-10 code (S42.341K) is
displayed on the Dual Coding screen with the
associated ICD-9 code (733.82) linked to it.
Actual Result: The ICD-10 code is displayed, but the
ICD-9 code is not. The user must click to link the ICD-9
code to the ICD-10 code. In this scenario, the ICD-9
code should automatically be linked to the ICD-10
code, requiring no user intervention.

Ambulatory EHR 142233 Document Builder - Tx Text 5.8.2.74 After updating to 5.8 UD2, when there are other
application window open along with the NextGen
Ambulatory EHR application and a user generates a
document, the Ambulatory EHR application goes into
the background and other open application come to the
foreground. The user has to then click the EHR icon on
the task bar to bring it back to the foreground.
Steps to Reproduce:
1. Access NextGen Ambulatory EHR
2. Open a patient.
3. Open other multiple applications.
4. Generate a document in Ambulatory EHR. The
document goes in the background along with NextGen
Ambulatory EHR and other applications come to the
foreground.
The application has been updated so that the
NextGen Ambulatory EHR application remains
in the foreground when a document is
generated. This hot fix has no impact on other

Steps to Reproduce:
1. Access NextGen Document Management.
2. Print any document from the application.
Actual results: When printed from NextGen Document
Management, documents print in reduced size scale.
Expected results: When printed from NextGen
Document Management, documents should print in the
correct size scale.
The application is updated to fix the issue. This
hot fix has no impact on other areas of the

Patient Sync 141851 Patient Sync 5.8 UD1 During the Core synchronization stage the Diagnosis
Search Tables on the remote application are over
written by the Populate Search Tables SQL job on the
remote database. The NextGen Remote Patient Chart
Synchronization client application is not properly
disabling the Populate Search Tables SQL job on the
remote database. The tables are being truncated on
the remote database, but it is not preventing the SQL
job
from executing. The SQL job executes, resulting in an
empty search tables.
The NextGen Remote Patient Chart
Synchronization Client application has been
modified to automatically disable the Populate
Search Tables job on the remote workstations
during the Core synchronization process. This
hot fix has no impact on other areas of the
application.

Ambulatory EHR 142055 Crystal Report 5.7.3.16 A new report is required to capture all prescription
deletion events.
Steps to Reproduce:
1. Verify that a new report Prescription Audit Events is
available in the Distribution Reports folder:
2. Log on to NextGen Ambulatory EHR, and then open
a patient chart.
3. Access the Medication module, and then click
Prescribe New.
4. Search for a controlled medication and add it to the
patient chart.
5. When the medication is active, perform the following
actions on the medication in the mentioned order:
a. eRx
b. Fax
c. Print
d. Delete
6. After all the actions mentioned in step 5 are
performed, on the File menu, click Reports > Crystal
Reports.
7. Click Report Setup.
8. Setup a new report for Prescription Audit Events,
with All Encounters for All Patients.
9. Navigate back to the Report Setup window, and then
click Preview, with the report (created in step 8)
selected.
10. Enter all the necessary parameters and the click
OK.
Expected Results: The generated report should display
all the actions performed on the controlled medication.
A new report named Prescription Audit Events
is added under the Medication Reports folder.
This report will have the ability to display the
deleted medications. This report will also
display medications that have been Canceled,
eRx, Faxed, and Printed. This hot fix has no

box Remove charge diagnoses when voiding a charge
is selected, then the charge is processed and voided,
and the diagnosis ICD-9 or ICD-10 code remains on the
Patient Chart >
Encounters tab >General sub tab in the ICD-CM
Diagnoses code field.
Steps to Reproduce:
1. Login to Practice Management (PM).
2. Access Admin>Practice Preferences> select
Remove Charge diagnoses when voiding a charge
option, and click OK.
3. Access any patient and create a new encounter that
has Insurance for this patient.
4. Access the charges; and add any charge with a
diagnosis.
5. Save the charge.
6. Bill the encounter and generate a claim.
7. Access posting and create an active batch.
8. Right click on the encounter in the encounter list on
Patient chart and click Add Transaction.
9. Add the transactions and Save.
10. Access the active batch, balance it and post the
payments.
11. Access the Charges dialog box of this encounter
and click Void to void the charges and click OK on the
pop-up message dialog box.
12. The system strikes the charge line item and the
charge is voided.
13. Access the General sub-tab of Encounters tab on
the Patient chart.
14. Review the ICD-CM section.
Result: No Diagnosis codes should display; but
currently they still display even after voiding the charge.

2. Look up a patient.
3. Create a new encounter.
4. Add a NextGen Ambulatory EHR image to the
patient's chart.
5. Make sure that the image is setup to be sent to the
PAQ.
6. Close the patient.
7. Look up a different patient in NextGen Ambulatory
EHR.
8. Log on to the PAQ.
9. Select the NextGen Ambulatory EHR Image that was
sent to the PAQ.
10. Select the printer icon and print the image.
Actual results: The active patient in NextGen
Ambulatory EHR prints in the image
footer.
Expected results: The active patient in the PAQ should
print in the footer.
When a user attempts to print a NextGen
Ambulatory EHR image from the PAQ , the
correct patient name now prints in the footer.

2. Do not assign the user to a provider.
3. Access NextGen Ambulatory EHR.
4. Open the Inbox.
Expected Result: No error message displays.
Actual Result: The following message displays: The
provider to which you are linked is not enabled for the
PAQ.
This error message should only display when the
user clicks the PAQ icon, not the Workflow (Inbox)
icon.

Practice Management 140850 ERA 5.8 UD1 The ERA import posting report displays the error
message, Not posted due to Ineligible Policy Number.
This error is due to a code change that was needed to
address problem number 54594. An additional code
change is needed to incorporate separate encounter
payer matching options.
A code change was made to implement the
previous encounter payer matching logic along
with the new logic that exists in hot fix 117 to
avoid the surplus of Ineligible Payer errors. To
differentiate between the two methods, a new
column was added (sys_info.era_matching_ind)
which is set to Y for clients reporting the original
issue in problem number 54594. For all other
clients needing this latest hot fix, no database
update is necessary. This hot fix has no impact
on other areas of the application.
Ambulatory EHR 140249 Orders Module Not Version
Specific
A new script is needed to update pending vaccine
orders to use an ICD-10 code instead of the associated
ICD-9 code.
A script has been provided to update the
diagnosis code to the ICD-10 code of Z23 for
pending vaccines created within the given date
range and for all inventory and favorite
vaccines. The script can be run the night before
the mandate date to use ICD-10 codes. This
issue has no impact on other areas of the
application.
Note: The script is released as a stored
procedure.
The name of this stored procedure is:
ng_imm_update_icd. When the HF package is
installed, this SP will be created in the
database.
Ambulatory EHR 140261 Document Generator – Tx Text 5.8 UD2 Lab Results Summary macros are not displaying data
on a generated document when the macro is on a line
with no other text.
Steps to Reproduce:
1. Add a lab results macro to a document.
2. Generate the document on an encounter that
contains lab results data.
Result: No lab results data appears on the document.
Documents now display lab results correctly
when the Lab Results Summary macro is used.
Changed the blank logic to recognize Lab
Results Summary macros as 'not blank' when
they have data. This hot fix has no impact on
other areas of the application. Refer to Steps to

Ambulatory EHR 140910 Communication Component;
Workflow Module
5.8 UD2 When a user completes a telephone call using the
Complete status in the Communication Component, all
pending telephone call tasks for all patients are
completed. The parent_detail_id is not changing, which
is causing all tasks to update together.
Steps to Reproduce:
1. Use the new Communication Component to create a
new call (Call #1) to a patient.
2. Use Add & Task to send a task to a recipient (users
or groups, both have been tested)
3. Use the new Communication Component to create a
new call (Call #2) to a different patient.
4. Use Add & Task to send a task to the same
recipient.
5. Recipient: Mark the call chain complete using the
Communication Component Call Status screen.
Change the Status field to Complete.
Results: Both tasks in the recipient's Clinical Tasking
inbox are marked as complete. Call #2 is marked as
complete in the call chain within the Communication
Component. Call #1 does not show any change in
status within the Communication Component.
Completing a call through the Communication
Component no longer completes all of the tasks
associated with calls in the Workflow module;
only the task associated with the completed call
is marked completed. This hot fix has no impact
