Is Life Too Complicated?
Is your world becoming more complicated? Notice I did not ask if it was less or even the same. I would guess many of the issues we face now stress us by the minute rather than the hours or days our parents knew. Young people may not know much from experience or from memorized information but they can come in handy with the newer electronic devices we are forced to use.
Can your phone do everything an iPad does, everything a game console did, everything a camera does, every-thing a rolodex did, everything the yellow pages did, (I’m just mentioning them one last time because soon they will be gone forever), and find everything in our known universe with google or suri?
The weather, calendar, music, maps, news, stocks, monitor security at home and the office, follow NASA, call a taxi, and last and least games (including a gun app so you can become familiar with all gun sounds), perhaps so we can be better people?
Have you been to the theater and noticed all those phones we are told to close? Have you been to lunch and seen everyone on their phones? Have you been to the dentist and refused to turn it off? Hex on you! Can you even live without your phone for an hour, a day, or longer? “No, Hell No”.
Scientists have suggested strongly that you are on it so much you are getting enough electronic pollution it may give you brain cancer. “No, Hell No”.
You don’t have any personal friends, just electronic friends. You cannot write a real letter on real paper with a tear stain on the paper and read it again 40 years later and remember those times. You cannot hold a conversation with other humans in person.
You don’t know anything because you never learned anything by memory, like a poem, or even diseases you may see and be expected to diagnose without looking it up.
I took a ride on a boat on Sunday. Weather forecast on TV was terrible as usual so I headed out at noon to Canyon Lake. Weather was perfect, lake was empty and with the motor turned off and just drifting with the wind it was clear what a stress free day it was. Sure I had a few items, iPhone, Marine phone, GPS locator and sonar depth finder, but just the basics. I did not turn on the GPS/Sonar because I was in the middle of the lake and could “see” the shoreline and except for a short scan then leaving it on the emergency channel, the Marine phone was silent. I did not get or send any communications. Ahhh!
Do you have a purpose on this earth other than to monitor communications and thereby stress yourself and others? When you have all those gadgets there is usually something needing updating, or fixing. Un-stress and let the artificial fish go without feeding the electronic monster constantly by streaming food. Try to find a friend to “talk” to and maybe you will last longer and “be happy” as an old philosophy used to say. Doing a good job in dentistry provides plenty of stress. Most of us are type “A” characters and we thrive on a little stress so let’s not create more for ourselves by complicating everything.
Just feel the sunshine and smell the flowers and let go of all the baggage that just weighs you down with stress. Have a Fine Texas Day!

Dr. Josh Austin, President, welcomed all to the meeting. The invocation was done by Dr. David Singleton. Dr. Austin announced that SADDS Fiesta Medals were for sale at the registration table for the Membership Cmte. and to see Dr. Potter. He thanked sponsors: Solution Reach, Garfield Refining, BB&T, Mass Mutual, MSA Architecture, Komet and Fortune Management. Dr. Austin asked the SADDS Past Presidents to stand. Included were Drs. Charles Bruchmiller, Glenn Walters, Leighton Wier, Warren Branch, Lisa Masters, Sheryl Beltrane, John Schmitz and Jamie Bone. Dr. Maria Atique presented an award to the Alamo Regional Science Fair Senior Division winner, Miss Sandra Moon from Squire Academy in San Antonio. Dr. Austin thanked Dr. Atique for judging the projects. Dr. Maria Howell presented the American College of Dentists Lifetime Award to Dr. Roy R. Gonzalez. Dr. Austin called the meeting to order at 6:40pm. Motion (3-17) was made by Dr. Glenn Walters and seconded by Dr. Beltrane to dispense with reading of the February 7, 2017 minutes and accept them as printed in the SADDS Newsletter. Motion was approved. Dr. Austin made several announcements including the SADDS Delegation Pre-Caucus meeting scheduled for April 24, 2017 at 6:30pm at SADDS office, the President’s Party
& Installation of Officers on Friday, May 19, 2017 at The Club at Sonterra, the TDA Meeting - May 4, 5, and 6, and the ADA’s new Find-a-Dentist tool. He asked all who hadn’t updated their ADA profile to do so before May 1st. Dr. Singleton read the names of the new members voted in at the March 7, 2017 BOD meeting: Drs. Marcela Alvarez, Marlene Ramos, Frederick Brown, Maria Paz Smith, Angie Sage, Geetika Saini and Shammi Saini. Dr. Lisa Masters introduced the meeting’s speaker Dr. John Schmitz. Dr. Schmitz gave out a few door prizes prior to speaking on “What You Don’t Know About Wisdom Teeth Can Hurt Your Patients”. Dr. Masters announced the course completion code then presented Dr. Schmitz a certificate of appreciation. Dr. Austin gave out SADDS fiesta medals as door prize gifts followed by the remaining door prizes. Dr. Austin announced the next GM meeting will be on Tuesday, June 13, 2017 at the Petroleum Club with Jose Barrera, MD & ENT speaking on “Surgical Management of Obstructive Sleep Apnea” as well as the other upcoming events. Dr. Masters spoke about the upcoming TDA Meeting and to sign up if you hadn’t already. The meeting was then adjourned by Dr. Josh Austin.

From The President
From the President... Joshua A. Austin, DDS FAGD
Time flies when you are having fun, so they say. That has certainly been true over the past year. My time as President of the San Antonio District Dental Society has felt like possibly the fastest year of my life. Last May, when I was inducted as President, it seemed like we had infinite time to accomplish the goals and objectives of the Association. As the year comes to a close, one can’t help reflect on the past year.
My first goal going into this year was to reformulate a new strategic plan for the associa-tion. With the help of now former TDA executive director Mike Geeslin, we are very close to having that wrapped up and finished. The new strategic plan should be in place for the new Board of Directors by the end of May. Thankfully, Mr. Geeslin was able to help us with some of the final touches before his departure from the Texas Dental Association in early May.
Another goal of this year was to have a successful year of General Membership meetings. I feel we have successfully accomplished that yet again. We had a great December Holiday party with a visit from TDA President Dr. Rita Cammarata. In February, we topped that with a visit from the American Dental Association President Dr. Gary Roberts and we invited the dental seniors & ASDA students to meet him. We have had great topics for the education portion of our meetings and attendance has been great! I believe, deep in my heart, that coming to General Membership (GM) meetings is one of the best things you can do for yourself and for your association. For that is where you can most closely experience what organized dentistry can offer you. That is where relationships and friendships are sparked and fostered.
If you didn’t make it to a General Membership meeting this year, make it your goal to come to one soon. You will have a great time and you won’t regret it.
My last goal of the year was to foster and promote some young leadership within the San Antonio District Dental Society. With a vibrant and active Committee for the New Dentist, San Antonio District Dental Society has become a
model component for young dentist activity. In addition, our incoming Secretary Treasurer, Dr. Christina Meiners, is consid-ered a new dentist. We couldn’t be more excited to have her start the executive leadership ladder. The San Antonio District Dental Society also now has the Texas Dental Association New Dentist Leadership award winner in Dr. Andrea Janik. The San Antonio District Dental
Society might lead the state in new and young dentist activity and leadership!
Looking forward, the future is bright. We have great executive leadership in the pipeline. Dr. Vicki Vickers will be a spectacular President of this Association. The other officers and directors are committed, energized and focused on continuing the excellent experience that is San Antonio District Dental Society membership. Fortunately, this isn’t good bye for me. I plan on continuing to be an active, involved member in this Association.

An unprecedented ransomware attack has wreaked havoc on the world starting May 12, 2017. Ransomware is a type of malicious software that infects a computer and restricts users’ access to the computers data until a ransom is paid to unlock it. News accounts indicate that the WannaCry ransomware was stolen from the National Security Agency, and then obtained by criminals, who distributed the ransomware via email to victims. News accounts indicate that this ransomware targets a vulnerability in the Windows operating system. Microsoft released a patch to fix the problem in March, 2017. Computers, which do not have up-to-date security patches in place, particularly up-to-date Windows Updates, and computers that use old Windows operating systems that are no longer supported by Microsoft are at great risk. Victims of these attacks receive demands to pay $300 or more in Bitcoin to unlock their devices, with the ransom increasing over time. News accounts indicate that this attack has hit large companies as well as governments, includ-ing Britain’s National Health Service, the Russian Interior Ministry, and FedEx. Britain’s National Health Service had to delay surgeries. Once WannaCry infects a computer, it spreads and infects all computers on the network. In a stroke of good fortune, especially to computer users in the United States, a 22 year old British researcher found a “kill switch” to halt the spread of this variant of ransomware, but warned that the hackers could program around the “kill switch” and continue attacking. Healthcare providers are particularly attractive targets to these cyber criminals, because healthcare providers are so heavily dependent on their computer systems, use electronic medical records, and their work is a matter of life or death.
Given this significant risk, dentists should immediately verify with their IT and HIPAA consultants that the following and other prudent measures are in place:
Windows security patches are up-to-date on all computers, serv-ers, and devices on their networks and that frequent automatic updates are occurring.
Security patches for all software programs are up-to-date on all computers and are being updated.
Anti-virus programs are installed, running, properly working, and being updated.
Data is being regularly and safely backed up to a location other than the computer network. Encrypted backups are advisable.
Dentists should also work with their IT and HIPAA compliance consult-ants concerning updating their HIPAA risk assessment – to be sure that this risk is addressed and mitigated. A 2014 case is illustrative. On December 8, 2014, the US Department of Health and Human Services, Office of Civil Rights (OCR), released a bulletin entitled “HIPAA Settle-ment Underscores the Vulnerability of Unpatched and Unsupported Software”. In that case, an Alaska community mental health services organization agreed to pay $150,000 to the federal government due to a malware caused breach of electronic protected health information of 2,743 individuals, as a direct result of failure to regularly update with available patches and running outdated, unsupported software.
OCR gives this advice concerning prevention: Ransomware can be delivered via email by attachments or links within the email (as well as
other hacking methods not involving email). Attachments in emails can include documents, zip files, and executable applications. Malicious links in emails can link directly to a malicious website the attacker uses to place malware on a system. Be aware of the following (and train your staff): Only open up emails from people you know and that you are
expecting. The attacker can impersonate the sender, or the computer belonging to someone you know may be infected without his or her knowledge.
Don't click on links in emails if you weren't expecting them - the attacker could camouflage a malicious link to make it look like it is for your bank, for example.
Keep your computer and antivirus up to date - this adds another layer of defense that could stop the malware.
Recently, an Austin medical practice was the victim of a ransomware attack. In that case, the medical practice indicated in its March 22, 2017 HIPAA breach report to the OCR that 279,663 individuals were affected by the breach.
Dentists should consider obtaining cybersecurity insurance.
If you are the victim of ransomware, immediately contact your IT consultant, your health law attorney, the police, and your insurance carrier. Also, OCR advises: If your organization is the victim of a ran-somware attack, please contact law enforcement immediately. We recommend organizations contact their FBI Field Office Cyber Task Force at www.fbi.gov/contact-us/field/field-offices immediately to report a ransomware event and request assistance. These professionals work with state and local law enforcement and other federal and inter-national partners to pursue cyber criminals globally and to assist victims of cybercrime. Victims are also encouraged to report cyber incidents to the United States Computer Emergency Readiness Team (US-CERT) via email to [email protected] and the FBI's Internet Crime Complaint Center at www.ic3.gov.

Student Spotlight
Marineé moved from New York to San Antonio to pursue her dream of dentistry and is a fourth year dental student at UTHSCSA. Prior to starting dental school, Marineé’s first career was in engineering: she worked as a Product Development Engineer for a dental implant company where her love for dentistry grew. Marinee received her bachelor's degree in Bioengineering from Lehigh University and her Master's degree in Bio-materials Science from New York University. As a Master’s degree candidate, Marinee was a finalist in the IADR Arthur Frechette Prosthodontic Group Award and she won the Master’s of Science Research Award at NYU for her work on comparing the failure modes of two implant-supported oxide-ceramic FPD systems.
Marineé also is an active leader in organized dentistry. In addition to being an active member in ASDA she saw the need for creating a network of female dental students and dentists to connect. She rejuvenated the AAWD (American Assn. of Women Dentists) chapter in 2015, which lead to the chapter receiving “The Phoenix Award” at the ADA meeting in Denver, CO in October. Dr. Mary Martin, President of AAWD National, also recognized Marinee as “Student Leader of the Year” for her work with the AAWD San Antonio Chapter. Marinee has served as ASDA’s (American Student Dental Association) event coordinator where she created the fund-raiser, Dr. Dodgeball, a dodgeball tournament where students compete in teams comprised of their GPG’s to raise money for a different charity each year. This year, all of Dr. Dodgeball’s proceeds benefitted the Dr. Keith Hill Memorial Scholarship.
Marinee enjoys volunteering her time and has done so at many events throughout the year, notably SADDS’s Dental Day at the Zoo, Haven for Hope / SACDC’s Veterans event, SACDC’s Serving our Heroes event and more.
Marineé was the recipient of UT Health San Antonio’s SEED Award, given to students who demonstrate record of exceptional motivation and commitment to serving the profession, the community, and the School of Dentistry and is currently serving as Presidential Schol-ar Ambassador for 2016-2017, where she represents the dental school at all UT Health San Antonio events.
Academia has been an interest of hers as she has been involved with the Teaching Honors program since her first year of dental school. Most recently she has been working on developing dental education videos through her ADEA Academic Dental Careers Fellowship Program research project, which aims to create multimedia content for dental students, by dental students. Her passion for dentistry is apparent across all aspects: academic, clinically, and organized and it is clear she will continue this trend going forward.
In her free time, Marineé devotes herself to her alma mater, Lehigh University, where she served as Young Alumni Trustee on the Board of Trustees, Director-at-Large on the Board of Directors, and Member of the Young Alumni Council. Lehigh recently recognized her hard work with the Lehigh Alumni Award, given once a decade to an alum who dedi-cates their life to giving back to the university in various capacities.
Marineé is interested in multiple aspects of dentistry and will pursue an AEGD in Greenville, NC this July. She hopes to spend the year explor-ing the state and all it has to offer before she decides where she’d like to ultimately practice. She looks forward to combining her love for dentistry with her passion for teaching and service as a proud alum of UT Dental San Antonio’s Class of 2017.

COURSE CONTENT The concept of the surgical placement of multiple dental implants at diverse angles is based on the idea of maximally utilizing the patient’s available bone while gaining more favorable distribution within the arch. This simple principle has given back hope to numerous patients that otherwise would have to undergo additional bone grafting procedures with the associated additional cost, longer treatment times and the use of removable dental prostheses. This course is designed for clinicians, staff and dental technicians who want to expand their treatment options for state-of-the-art, implant-based fixed
prosthetic dentistry for complete arch indications, providing a practical guide to the rehabilitation of the edentulous arch. The speakers will address a variety of treatment concepts and material options, mainly focusing on the All-on-4 protocol & immediate function. This lecture will provide a complete overview of the All-on-4 treatment and use of tilted implants covering the fundamental diagnostic, surgical, laboratory and prosthetic aspects required for the immediate fixed rehabilitation of full arches.

Nancy Avellar of the San Antonio Conservation Society was the featured speaker with ‘Missions Accomplished’ - the story of the San Antonio Missions from their founding in 1718 to their designation as Texas’ first UNESCO World Heritage Site. Chairman Kristen Huriega and Programs Chairman Mary Balderas for a year of delicious lunches and truly interesting programs.
The Alliance to the TDA convention and annual general meeting is May 4th and 5th. This event is extra-special to the San Antonio Alliance because we will watch our own Patty Taylor be inducted as President of the ATDA. We are so proud of Patty and so confident that she is the perfect person to take on this important role. Our delegates to the ATDA convention will be Diana Bruchmiller, America Gies, and Chita Harris.
Dental Health Chairman Karen Cox shared some of her experiences from visits to local elementary schools this year. In partnership with the city of San Antonio and MetroHealth, Karen and other Alliance members visited five underprivileged schools. Her focus has been on reaching out to pre-kinder through 1st graders with a message about oral health and good habits. In addition to her puppet show, Karen has devised fun activities to reinforce good choices.
For example, the children are asked to sort various food items into either a ‘happy’ sack or a ‘not-so-happy’ sack. Such a great way to get the children’s attention!
Fun fact: Karen is ‘taking her show on the road’ this spring with a visit to El Buen Pastor daycare in Austin.
The Smiles of Hope Dental Clinic is scheduled for October, but to the Alliance that’s just around the corner. Once again Kristen Huriega is stepping up to coordinate breakfast and lunch for all volunteers on October 6th and 7th. If you would like to donate funds toward this effort, or better yet, are willing to contact local food vendors for a donation, please contact Kristen. Karen Cox and Chita Harris presented outgoing

In a final nod to our year of growth, the 2017-2018 officers were installed in a floral-themed ceremony. Each new officer was given a flower whose meaning was tied to their office and its duties. The Alliance is lucky to have creative and thoughtful ladies to design such a neat ceremony.

This 2 weekend (4 days) course will provide a framework for the treatment of the complex interdisciplinary dental patient. The first step in the process is the diagnosis, followed by the treatment plan. The next step is the coordination and sequencing of the treatment plan with the specialists. The final step is the provision of the restorative dentistry. It is the purpose of this course to provide a step-by-step system to allow the restorative dentist to complete each step in the process.

In Memoriam
Cyrus Jack Payne, of San Antonio, TX, born on July 20, 1928, suddenly passed away on the morning of Tuesday, April 4, 2017. He was a loving father, grandfather, great-grandfather and uncle. His greatest joys were his family and his church.
Born in Sweetwater, Tennessee, Jack grew up in Dayton, Tennessee, where the famous Scopes trial was held. After graduation from high school, he joined the US Marine Corps; he credited the Corps boot camp with changing his life’s direction. Jack spent a brief time with t he US Army and later attended Middle Tennessee State University where he excelled in football and academics. Prior to graduation, he was offered a commission in the US Air Force for jet pilot training. After Jack completed pilot training, he was stationed at the Aberdeen Proving Ground, MD where he flew a variety of aircraft including B-17’s, P-51’s and B-26’s among others.
Jack’s flights with the Air Force brought him to San Antonio. While touring downtown he found himself at the same bus stop as an old high school sweetheart from Tennessee, Sidney Margaret Potter. They were married on Groundhog Day, February 2, 1952. While on their honey-moon in Las Vegas they witnessed testing of the Atomic Bomb. He flew F-86 Saber jets in Okinawa during the Korean War.
After leaving military service, Jack enrolled in the University of Tennessee Dental School at Memphis. While there, he and Sidney welcomed their only child, Polly Payne Noble. Jack practiced general dentistry for many years in San Antonio before he and his family returned to Memphis to attend the Master of Science Orthodontic Program.
While maintaining an active orthodontic practice in San Antonio, Jack loved teaching and being a mentor to students at the University of TX Dental School, Houston. When the UT Health Science Center in San Antonio opened, Jack transferred to that campus to continue his teaching career. In 2000, Jack retired from his practice and devoted even more time to another of his great passions-golf. He had a very dear and close group of friends from Tennessee with whom he traveled to Scotland and annual trips to Doral in Florida.
He is preceded in death by his parents, Carter Duncan Payne and Bessie Leming Payne, siblings: Docs Ellis Payne, Erskine P. Payne, Dana Anderson Goldston and Frances Payne Hackney and his precious baby great-granddaughter, Scarlett McNeel Noe. Jack is survived by the love of his life for 65 years, Sidney Margaret, his daughter Polly and her husband, Charles H. (Hank) Noble III, granddaughter Meg Noble Noe and her husband, William Noe, grandson Charles H. Noble IV, and great-grandson William Gray Noe Jr., brothers-in-law Robert Potter and his wife, Diane, Alan Potter and his wife Xuan, along with many much-loved nieces and nephews. A Memorial Service was held on Saturday, April 8, 2017, 2:00pm at McArthur Park Church of Christ, 1907 N.E. Loop 410, San Antonio. A private graveside service was held.
Eugene Tokar, DDS April 9, 1929 ‐ April 5, 2017
Dr. Eugene Mory Tokar passed away on Wednesday, April 5, 2017, following a brief illness. He was born in Philadelphia, PA, on April 9, 1929, to Philip Tokar and Rebecca Bleichman Tokar. He attended Camden High School in Camden, NJ, and entered Optometry School in Philadelphia, PA, in 1947. Upon graduating he moved to San Antonio, TX, where he married his beloved, Jimmie Louise Weaver, on October 13, 1957, at the Menger Hotel. Unsatisfied with optometry, he enrolled in the University of Texas Dental School in Houston. He graduated in 1964 and moved to Floresville, TX, where he practiced dentistry for 51 years, retiring at the age of 86.
Dr. Tokar was a best friend to each of his five grandchildren. Despite his age, he never became an old man and was interested in all forms of technology, current events, and popular culture. He was active on social media and considered himself a lifelong learner. A trip to Norway with his niece and grandson was planned for the fall.
He was preceded in death by his parents, wife, and son, Steven Tokar; sister Florence Saunders; and brother Henry Tokar. He is survived by his daughters Janet Tokar and Carol Pavliska; son-in-law Jeffry Pavliska; grandchildren Ellen Pavliska (husband Cody Bates), Joel, Jules, Camille, and Jasper Pavliska. A celebration of his life was held at Vinyard Funeral Home in Floresville on April 22, 2017, at 1:00 pm. Memorial donations may be made to the San Antonio Metropolitan Ballet (www.sametballet.org) or Big Bend Conservancy (www.bigbendfriends.org).
Lee Lloyd Harris, DDS March 24, 1934 ‐ May 7, 2017 Lee was welcomed into Heaven on May 7. He was the beloved husband of Chita and the father of Justin Harris [Marti], Stephen Harris [Linda], and Daniel Harris [Laura]. He was blessed with nine grandchildren and many family members and friends. Lee was born in Kilgore, Texas on March 24, 1934 to Hazel and Raymond Harris. His parents preceded him in death. He is survived by his sisters, Averil Bradford and Ramona Cooper [Bud], and his cousin, Tom Harris [Alene]. Lee's motto was "Keep Smiling". He graduated from Baylor University's School of Dentistry and the U.T. Dental Branch in Houston. In addition to his orthodontic practice, he was on the faculty of the U.T.H.S.C. Dental School. His charitable heart was evident in his volunteer involvement in the Optimist Club, Youth for Christ, Trinity Baptist Church, Methodist Hospital's Blue Birds, and dental organizations. The celebration of Lee's life was held Monday, May 15 at 2:00 PM at Trinity Baptist Church, 319 East Mulberry. Sleep disordered breathing (SDB) is a spectrum of syndromes comprised by snoring, upper airway resistance syndrome (UARS), and obstructive sleep apnea (OSA). OSA is a syndrome: it associates complaints and test findings. Sleep apnea syndrome consists of periods of apneas (cessation of airflow at the nose or mouth for > 10 seconds) and hypopneas (reduced respiration with desaturation terminated by arousal). OSA can be defined by the number of apneas and hypopneas per hour or apnea-hypopnea index (AHI).
Prevalence: A common sleep disorder, OSA incidence has been reported as 24% in males and 9% in females by attended polysomogram in 602 undiagnosed state employees. Most patients fail to be diagnosed; in fact, 93% of females and 82% of males with moderate to severe OSA have not been identified.
Significance: The profound effects of SDB upon the cardiovascular and respiratory systems, and neuro-cognitive function have been documented. The Sleep Heart Health Study and the Wisconsin Sleep Cohort have demonstrated a strong association between SDB and hypertension. Treatment options, both non-surgical and surgical, will be discussed.
