Dear Colleagues,

‘Time and tide wait for none’.Another year has elapsed. New year has started with hope, commitmentand zeal to perform and accomplish. The year 2017 will be a testing and

hectic one because of the dual responsibility of the General Secretary as well as that of theOrganizing Secretary of the 69th Annual conference of Cardiological Society of India to be held atKolkata. The combined power of the wisdom of my predecessors and the bubbly energy of theorganizing team from our CSI -West Bengal Branch will be the source of inspiration and impetus.

Under the able leadership of our mentors as well as the executive committee headed by thePresident of CSI, we have worked over the last few years untiringly to make our society as vibrantand communicative as possible. At the click of a mouse , one can now access not only all importantinformation related to the society's activities around the country and the world and the importantnews that we can use. We are now able to access various publications done on behalf of CSI like,CSI-Newsletter, CSI-Update book, Ten Commandments for prevention of CVD or journals inelectronic form through our website . At the same time the use of the social mediawww.csi.org.inwas also carried out with the aim of proper and timely information.We have forged strong ties withthe international bodies be it ACC, ESC, TCT, PCR or World Heart Federation. We claim with allhumility that CSI is now a globally acclaimed organization. Our relationship with Governmentagencies has just started with baby steps. I am very hopeful that CSI's voice in near future will beappreciated by the Government of India in formulating various protocols and guidelines inprevention of major CV illnesses in India.

CSI with full responsibility, power of advocacy and active participatory role on issues of CVDprevention in India on the cusp of cardiovascular epidemic can only provide better care of thepatients and circumvent the war.

The information like the various sub-speciality councils of CSI, nominations for various orationsand awards will be there in the news letter. I would only request all of you to follow the time scheduleand keep track of our website.

‘A stich in time saves nine.We are yet to achieve two of our ambitious projects: Data collection or Registry formation onvarious CVDs and the other one is the prestigious CSI Book of Cardiology with Indianperspectives. They are also in advanced stages of seeing the light of the day. CSI-NIC data for

cardiac interventions are already available for use by any member in the fraternity. I request all of youto participate in this all important CSI venture of data collection and generation. This will go a longway in the history of practice of Cardiology in India.

‘Where the mind is without fear and the head is held high Where knowledge is

free…’Three important subspecialty national conferences have been arranged this year. The first one thesecond CSI-Heart Failure Council conference has already been held at Bhubaneswar on 10th to12th February, 2017 under the leadership of Dr. U. C. Samal, Dr. S. N. Routray and Dr. S. S. Mishra.The 2nd one will be CSI-NIC mid term Conference at New Delhi on 14th to 16th April,2017 underthe leadership of Dr. K. C. Goswami, Dr. N. N. Khanna and Dr. Rakesh Yadav. The third importantmeeting of CSI-Cardiac Prevent Council will be held at Coimbatore on 25th and 26th June, 2017under the leadership of Dr. S. Shanmugasundaram and Dr. S. Natarajan. All are requested to be partof these conferences so that everyone is enriched with sharing of the knowledge.

‘Ignorance is no bliss’.Kolkata calls all of you to participate in the mega event of the society “the 69th Annual Conferenceof CSI Kolkata’ at the Science City Convention Centre from 30thNovember to 3rd December,2017. The whole atmosphere with most pleasant weather will be full of festive mood. Come, join,interact with the best minds of the subject and also enjoy the culture and cuisine of the city. Thewhole team under the leadership of Dr. P. K. Deb, Dr. Santanu Guha, Dr. A. K. Banerjee, all pastPresidents of CSI, Dr. M. S. Hiremath, President and Dr. K. PresidentCardiological Society of India

As President CSI, it gives me great pleasure to announce the Annual CSIConference on Nov 30th, Dec 1st, 2nd and 3rd 2017, at Kolkata.

Organizing the Scientific program for this important annual meeting hasalways been a great challenge. We had stupendous success at 2016 meet at

Kochi.The Scientific deliberation turned out to be highly rewarding.There were some impact making presentations and highly provocative case based discussions. Bothinternational and Indian Speakers gave some outstanding talks.

The Presidential Conclave where President of ESC, ACC, AHA, and WHF came together was astunning success. There were many exciting lectures in the Side Halls in the field of Echo, Lipids,and Imaging etc. The program worked to precise timing. To me, as Program Chair and my everhardworking team, it was very gratifying to see 2016 meet go so well and we can only get better!!

Thus 2017 is expected to be a bigger and much more exciting event. We will have Scientific Programtailored to the needs of a practising Cardiologist's daily requirements. This would have Casescenario, New Drug launch, Interesting images and ECG and continue to have Main Hall flavor ofdidactic lectures. The PG training will continue with the Honoured teachers. I am sure books likeCardiology Update will also continue to add a new dimension of understanding latest inCardiology.

Kolkata, the Heart Hub of India, surely will offer you excellent Hospitality and a great opportunityto interact with Peers and Seniors.

So, mark the dates in your diary. You just can't afford to miss this opportunity to upscale yourunderstanding of Cardiology.

Immediate Past President& Chairman, CSI Fellowship Committee 2017

Dear Colleagues,

At the outset, I want to thank all of you for making my tenure as Presidentof CSI a memorable one which I shall cherish for the rest of my life.

The 68th Annual Conference of CSI at Kochi was a thumping success for the most efficient effortsof Dr. P. P. Mohanan, the Organising Secretary and Dr. M. S. Hiremath, the then President Electand Scientific Chairman. We were honoured by honouring Dr. S. C. Manchanda and Dr. J. P. Daswith the prestigious CSI Life Time Achievement Awards.

My Presidential Oration titled “Management of STEMI: Past Present and Future. An IndianPerspective” culminated in publishing the CSI Position Statement on STEMI Management inIndia. My mission and vision are to propagate this to every nook and corner of Indian Healthsystem. You will be happy to know that we are going to publish the CSI Position Statement on HeartFailure Management in India in the 69th Annual Conference in Kolkata this year.

The Mid Term NIC, 2016 took place in Hyderabad with flair and flamboyance under the leadershipof Dr. N.N. Khanna, Scientific Chaiman and Dr. J. Shivkumar Rao, Organising Secretary. The FirstJoint National Conference of CSI Cardiac Rhythm Council and CSI Congenital Heart DiseaseCouncil took place in Patna in November, 2016 under the respective Chairmanships of Dr. S.Ramakrishnan and Dr. Rakesh Yadav with the most befitting back up support by the OrganisingSecretary Dr. Ajay Kr. Sinha.

The Indian Heart Journal (IHJ) is going from strength to strength under the dynamic leadership ofDr. Sundeep Mishra. We have come out with two sister journals of IHJ namely Indian Heart JournalCardiovascular Case Reports (online) and Indian Heart Journal Intervention. The World HeartDay was also celebrated with usual élan both at the CSI Head Quarters and at the branch levels.

I am confident that CSI will perform according to its international reputation under the ableleadership of Dr. M. S. Hiremath, the President of CSI. I shall try my best to make the CSIConvocation Ceremony, 2017 a descent show. Stay tuned with us!

Hony. Editor Indian Heart JournalCardiological Society of India

Dear Friends,

You will be happy to note that Indian Heart Journal has achieved a newmile-stone. For the first time in history the number of citations receivedby IHJ crossed 200 in year 2015, it was actually 249. You might rememberthat bit was exactly a decade ago, in year 2006 in fact that we wereanywhere near, i.e. 196 citations. But why look at only citations; the SJRhas nearly doubled compared to the year 2009 and we are now very near ourpeak SJR rank ever, which was achieved in year 2004. As a matter of fact, for the first time in morethan a decade IHJ has exceeded even JAPI in impactfulness (research gate) and also the journal rank(SJR). Traditionally, world over the medicine journals are always rated much higher than specialtyjournals. For example impact factor of JACC is 17.759 but that of Lancet is 45.217 or NEJM is59.558. This is because the medicine journals always have a broader reach and wider acceptance. Inthis context IHJ may be a stand-alone journal from India whose impact is more than major medicinejournal of the area. All this really means is that IHJ is now not only firmly back on track but is lookingtowards new landmarks.

Another milestone is the launch of a new open access publication, , a third in theIHJ-Interventionfamily of Indian Heart Journal publications. IHJ-Interventions will feature cutting-edge newresearch in the field of Interventional Cardiology. It will:

1. Place a high priority on publishing the best work, making the submission andpublication process as straightforward and flexible as possible for authors and rapidpublication.

2. Publish a broad range of research study types: Original Research, BriefCommunications, Reviews, and Opinion pieces, Images, Case Reports and Letters.

3. Be a forum for highlighting timely new research ideas for the interventionalcommunity as also for featuring novel research in interdisciplinary andmultidisciplinary areas.

4. Available both online and print version, all articles in IHJ-Interventions will be peerreviewed, fully open access: immediately and freely available to read, download andshare.

A final piece of activity of IHJ now and in future is to develop Practice Algorithms which will benefitIndia and other less industrialized countries around the world in contrast to West based guidelines.Treatment algorithms for Stable CAD are the first; those for STEMI are on the way, and those ofheart failure, dyslipidemia, and hypertension are soon to follow.

We are going to have the CSI-NIC Mid Term Meet at Hotel The Ashok, New Delhi from April 14 to

16 2017 under the stewardship of Prof. K C Goswami as the organizing Chairman, Prof. Rakesh

Yadav as the organizing secretary and Prof. N N Khanna as the chairman of CSI-NIC. This is the

largest annual conference of interventional cardiologists in India.

We would also have Cardiac Prevent 2017 on 24th and 25th June at Coimbatore, Tamil Nadu under

the leadership of Prof. Sanmugasundaram

69th Annual Conference of CSI would be organized at Science City, Kolkata from 30th November

to 3rd December, 2017 with Prof. Saratchandra as chairman of scientific committee and Dr Mrinal

K Das as organizing secretary.

ACC Annual Congress 2017 at Washington, will have ACC-CSI Session on 17th March, 2017. IHJ

“Morphing thru Time” Session, on Sidelines of ACC 2017, Washington DC, would be held in

Newseum” Washington DC on Friday, 17th March 2017, 7PM onwards.

We are proud to announce that there would be joint sessions in Euro-PCR, ESC Annual Congress

and TCT in 2017, to name few.

India (CSI) to be held at Kolkata from 30th November to 3rdDecember, 2017 at the Science City

Convention Centre, Kolkata, I welcome you all to participate in the mega event of the society and

Rheumatic Heart Disease is a disease of “have-nots”

RHD affects up to 20 million people across the world, out of which 2million are children. It is responsible for approximately 500000 deaths annually worldwide. Indiahas approx 1 million patients with RHD. Even a conservative estimate of the incidence of RFsuggests that at least 50,000 new episodes occur every year.

The younger age of onset (juvenile RHD) seen in India is a special feature of both public health andclinical importance. In the 21st century, rheumatic fever (RF) and rheumatic heart disease (RHD)are neglected diseases of marginalized communities.

Although RF and RHD have been almost eradicated in areas with established economies, migrationfrom low-income to high-income settings might be responsible for a new burden of RHD in high-income countries.

The World Heart Federation (WHF) and its Working Group on RF and RHD unites global experts,combines their experience and enthusiasm, and provides a platform for RHD control.

The WHF institutional strategic goal - a 25% reduction in premature deaths from RF and RHDamong individuals aged <25 years by the year 2025.

Incidence of rheumatic heart disease (RHD) - a chronic heart condition caused by rheumatic feveris much higher in India than what is presently believed. Just months after TOI reported how RHDamong Indian children could be 20 times more than what is believed, a study presented at the WorldCongress of Cardiology in Dubai in 2012 said RHD is significantly under-treated in India. It isironic that rather innocuous “sore throat” should extract such a high price from the host.

Control of preceding infections and their squeal is both cost effective and inexpensive.Rheumaticfever licks the joints but bites the heart. Clinical profile of ARF is changing; adherence to revisedJones criteria may lead to under diagnosis among Indian children with polyarthralgia and overdiagnosis in clinically diagnosed carditis. To avoid morbidity, mortality, heavy expenditure onsophisticated treatment with balloon and valve replacement, an early precise diagnosis of ARF andsecondary prophylaxis is very important and ECHO is a very simple, noninvasive, reproducible anduseful investigation in this endeavor. ECHO has enhanced our understanding of thepathophysiology of ARF and chronic RHD and is commonly used in the evaluation of patients withacute rheumatic fever.

In India, rheumatic fever is endemic and remains one of the major causes of cardiovascular disease,accounting for nearly 25-45% of the acquired heart disease. The annual incidence of rheumaticfever is 100-200 times greater than that observed in developed countries and fluctuates between100-200 per 1,00,000 children of school age (from 5 years to 17 or 18 years depending on thestudy). India is in the phase of 'epidemiological transition'. On one hand there is a substantialburden due to RHD for the treatment of which enormous money is spent, on the other hand thegovernment resources are scarce to treat and prevent the disease.

The time has come to stop blame game. When the helpless child with severe carditis with CHF isbrought to the hospital; the cardiologist blames the clinician for referring the child late; but thephysician blames the illiterate parents and the parents in turn blames the government for poor socioeconomical conditions and unhygienic living conditions. It is high time we stopped blaming eachother in India and utilize the modern facility like ECHO to make early and precise diagnosis to treatthe children with carditis appropriately in time.

Professor Bongani Mayosi who is one of the pioneers in RHD globally rightly quoted

“RHD fell through the cracks of healthcare: in combating Ischemic heart disease and newpandemics of HIV-AIDS”

Experience from several regions of the world suggests that incorporation of a strategy of primaryantibiotic prophylaxis into a comprehensive program for disease control can reduce the incidenceof rheumatic fever and rheumatic heart disease and can be successfully implemented in resource-poor settings across the world and should be a key component of any rheumatic heart diseasecontrol program.

Not only the cost of treatment is phenomenal, the morbidity and mortality is also enormous. Hence“Prevention is better than cure” is very apt for rheumatic fever. We will seek the guidance of stalwartsin the field of RHD from our country. Together we will make it possible.

Dear colleagues,

Best wishes for a healthy, peaceful and prosperous New Year.

Cardiological Society of India (CSI), the most prestigious association of practisingcardiologists of our country, has its exclusive subspeciality council for PreventiveCardiology with the aim of focussing our thoughts and intensifying our efforts toprevent cardiovascular diseases (CVD) among the public. Facing a huge epidemic of

cardiovascular diseases in our country, it is the responsibility of each and every one of us to effectively promote theconcept of CVD prevention among our patients and the public at a large. It should be the priority of allcardiologists, both interventional and non interventional, to identify the barriers for implementing CVDprevention and formulate plans to overcome them. To implement this huge task of CVD prevention, we need toput our efforts together to identify those at risk, favourably modify their risk factors and make the enirepublicaware of the importance of adoption of healthy lifestyle.

With this in mind, the Preventive Cardiology Subspeciality council of CSI conducts an international meetingonce in two years to exchange our thoughts and augment our efforts to prevent CVD. In addition to propagatingthe scientific information among the physicians, the council serves as the nodal organization to offer suggestionsto the policy makers of our nation. The present convention, the third of its kind, will focus on the theme of “ CVDprevention in India – the challenges and solutions”. We plan to have the meeting conducted in Le Meridien, atCoimbatore on 24th and 25th of June 2017. Attending this meeting will certainly enhance our awareness andunderstanding of current issues of prevention. Though topics of general interest like diabetes, hypertension,lipids, obesity etc are discussed in every meeting, this convention will elaborate on finer details and recentdevelopments in these fields.

International experts in this filed are participating to propagate the current concepts of Preventive Cardiologyamong us. , President Elect of World Heart Federation, , President Elect,Dr. David Allan Wood Dr. Paul DendaleEuropean Association of Preventive Cardiology, of European Society of Cardiology,Dr. Antonio PellicciaDr. David Colquhoun, Clinical & preventive Cardiology council, The Cardiac Society of Australia and NewZealand and from USA have already consented to be the International Faculty.Dr Prakash DeedwaniaRenowned researchers and clinicians from our country are also taking part as National Faculty .

This conference is very likely to be different from others in the respect that Learning and Sharing will be on thebasis of group discussions, round table meets, resolving controversies, hands on workshops on CVD screening etc.Cardiologists, both clinical and interventional, Internists, General Practitioners, Nurses, Physiotherapists,Dieticians, Occupational Therapists, Psychologists, Exercise Physiologists and Exercise Professionals are theones who would benefit the most out of this meeting. It will also be extremely useful for DM/DNB fellows as thecurrent curriculum does not focus on CVD prevention, epidemiology, design of clinical trials and biostatistics indepth. Being the future of Cardiology in India, they should utilize this opportunity to update their knowledge ofpractice of preventive medicine and to improve their powers of doing research.

Echo has evolved from a tool of curiosity to, now a need for physicians.

This not so long journey has been a roller coaster ride. Now having reached a stage wherein 3D echo

has achieved a `routine to use' status we are still faced with challenges.

The penetration of this extremely useful modality is poor. A tool which should be available to every

physician is limited by lack of availability, training or even law !!!

The efforts should be consolidated towards bridging this gap so as to make this equipment and

expertise available to all who need.

Dear all ,

Here is wishing you all a happy New Year !

Non invasive cardiac imaging has undergone a quantum leap with the the adoption of CT perfusion

and CT FFR in routine day to day studies. It has transformed a purely structural study into that of a

combined structural and functional imaging study. The addition of prognostic data that is emerging

adds to the flavor of using these tools as a one stop shop in the evaluation of patients with suspected

coronary artery disease and appropriately risk stratifying them in a cost effective manner.

The role of imaging in structural heart disease cannot be overemphasised as we had a plethora of

speakers in the recent CSI meet at Kochi elaborating on the importance of a good imaging study

prior to intervention especially in complex cases.

We move ahead this year by partnering with CSI prevent at Coimbatore in June for the first time and

we hope to get cardiologists interested the benefits of utilizing imaging tools in day to day practice.

We are also hopeful of joining hands with other societies like the pediatric and heart failure societies

where imaging plays a small but tangible part in their practice. We are hoping to hold small

workshops with the help of local CSI chapters throughout the year to enable practicing

cardiologists to understand the nuts and bolts of imaging in an easy manner.

Oration are as under :

i) The nominee must be a member of CSI for minimum continuous period of 5 years. He must be of

Indian origin and have worked in India.

ii) Must have had Academic/Research experience of not less than 5 years.

iii) He must have a total of at least 15 publications in the field of Cardiology, in journal of repute and

also Indian Journals.

5. The proposer must submit a written proposal seconded by another member.

6. The proposal must be accompanied by –

i. Complete Bio-data.

ii. List of publications as the first author.

iii. List of publications as the joint author.

iv. Three of the best publications.

NOTICEFor all the members of the CSI

1. All CSI branches (city and State branches) shall be under the administrative control of the

parent body.

2. Branches must follow the Constitution of the CSI with regard to membership, elections,

functions of the office-bearers and aims & objectives.

3. Each branch should have its own articles of association and rules and regulations

registered with the registrar of societies. This is needed as the composition of the

executive committee and the scientific committee will depend upon the existing strength

of the members and is likely to be at variance with that of the parent body.

4. Branches should use correct logo and stationery in conformation with that approved for

the parent body.

5. New branches to be created should follow the state-wise pattern (pre-existing branches

would not be disturbed). City branches should have affiliation to the state branch and the

state branches should be directly responsible to the parent body. Zonal branches, if

formed, must have membership from contiguous states and union territories and not from

one state/city only. It is desired that correct nomenclature of the branches be followed to

avoid confusion and overlap.

6. Branches should have regular elections at least once in every two years. It would be the

joint as well as separate responsibility of the Secretary and the President of the outgoing

executive committee to send the notification regarding newly elected office bearers to the

parent body. If there is no election for two or more consecutive terms, the branch shall

stand dissolved and all money/assets owned by it shall vest with the parent body till

alternative arrangements are made. Bankers of the branches must be given a copy of this

resolution at the time of opening of the branch or when the office bearers change.

7. The election of the branch body should be direct with all members offered an opportunity

to participate. Number of office bearers would not exceed 10% of the current membership

with a minimum of four office bearers. There should be no provision for nomination

although the articles of association should mention the method of filling of any interim

vacant post. All such decisions should be ratified by the general body with a copy to the

parent body.

Guidelines for the BranchesCardiological Society of India

8. Each branch should have its own bank account, PAN, TDS number and 80G certificate.

This should be communicated to the parent body as soon as it becomes available.

However, the parent body shall not share any financial liability. All tax matters will be the

responsibility of the branch. No proceedings would be permissible against office bearers of

the parent body (CSI) for the lapses of the branches.

9. Audited accounts of the branch should be submitted every year to the HQs. These will be

shown as annexures in the annual report of the CSI parent body. Only the portion of the

money which are physically transferred to the accounts of the HQs shall form part of the

latter's accounts. Failure to submit audited accounts for two or more consecutive terms

without a valid and acceptable reason shall result in dissolution of the branch. The names

of auditors and bankers must be communicated to the HQs. The HQs shall reserve the

right for external audit in case of complaints, if any.

10. A branch must periodically (once in six months) submit a report of its activities to the

Headquarters.

11. Branches shall enjoy complete financial autonomy with regards to the funds collected by

them. However, wherever a large capital expenditure of more than 5 lakhs is contemplated,

prior permission of the HQs should be obtained. Each branch should make a provisional

budget in advance and seek approval from its own general body.

12. Branches are expected to optimally utilise the resources and it is desirable to send a

periodic report about it to the HQs.

13. All disputes and disagreements related to any branch shall have final arbitration by the

national executive committee of the CSI.

14. A member either ordinary or Life Member of the Central CSI could be ordinary or Life

Member of the Branch. Others could only be Associate member of the branch.

15. To open a branch, the required number of members should be 25.
