• Purpose: assist primary care providers to prescribe in a safe and effective manner

• Applies only to chronic non-cancer pain• Only 11 pages

1. In general, the total daily dose of opioid should not exceed 120 mg of oral morphine equivalents

2. If considering going above 120mg, obtain a pain management consultation

3. Assess function regularly, for example using SF-36, Oswestry, QuickDash
