HIEs offer distinct advantages to other models for an infrastructure to enhance public health capabilities.
q HIEs are good at matching patient identities, combining data sets, and managing patient consent
q As not-for-profits governed by stakeholders and regulated by states, HIEs are a more trustworthy place to manage sensitive health information than either government or for-profit businesses.
q HIEs are experienced at managing health data and protecting privacy and have long lived under strict rules, carefully developed by the community, for the use of patient data.
q A not-for-profit HIE is a good vehicle for deploying public investments, more adroit than a government agency, yet responsive to the policy choices and priorities of public health leaders.
q HIEs are closely tied to states, which are the key governmental body for managing public health.
q An infrastructure which is used all the time will be ready when needed, but one that is usually idle may be difficult to spin up in an emergency. Consider natural disasters, wildfires, hurricanes, etc.

Critical Role of HIEs During COVID-19 Response
Combining Multiple Data SetsHIEs are very effective at combining multiple data sets at the identified patient level, a task which is made more accurate as additional data sources are added. During COVID this capability has been important for gaining insights about race/ethnicity and comorbidities, and will be important as treatments and vaccines are evaluated.
Projects Requiring CooperationHIEs are good at projects requiring cooperation among healthcare organizations, state agencies, and other stakeholders. Most data interoperability requires meaningful cooperation to work well.
Putting Grant Dollars to WorkHIEs are non-profits and are effective at putting grant dollars to work, often sharing the outputs across states. HIEs are on the frontline with state and local stakeholders, coordinating discussions among communities to ensure the right information is flowing to those who need it, as quickly and effectively as possible
Compiling Data from the FieldHIEs are When public health leaders need to collect data, HIEs are a good vehicleboth for compiling data from the field and making information available back tothe credentialed healthcare providers who would benefit.

q Issue guidance to state, local and federal public health agencies that incentivizes the agencies to partner with HIEs as their primary data source when possible
q Designate HIEs as public health entities/public health data utilities, and enable direct funding via CDC grants or other funding vehicles.
q Provide incentives to finish the buildout of regional and statewide HIEs to cover all population areas, including rural and underserved communities.
q Encourage CMS colleagues to prioritize bidirectional exchange via HIE in payment policies.
