Notary  Automotive  Signing  Checklist  (This  is  to  be  scanned/faxed/pictured  along  with  your  completed  AOI  to  your  Scheduler)  

This  document  is  a  summation  of  your  Notary  Confirmation.  It  does  not,  in  any  way,  replace  your  Notary  Confirmation,  but  serves  as  a  tool  to  acknowledge  your  agreement  and  responsibilities  as  a  Maverick  Notary  Signing  Agent  according  to  the  Notary  Confirmation.    

 This  assignment  will  be  considered  Complete  and  Submitted  for  payment  when  this  Checklist  and  the  AOI  have  been  Uploaded  to  

www.mavsign.com  or  Faxed/Scanned  over  to  your  Scheduler.

After  accepting  the  Auto  Signing  with  the  Scheduler  by  phone,  I  have  done  the  following:  

1. Printed  and  Read  over  the  entire  Notary  Confirmation  I  received  by  email.     Initial  ____  2. Called  the  customer  to  introduce  myself  and  reviewed  Items  to  Collect.       Initial  ____  3. No  appointment  time  was  set.  Customer  knows  I  will  do  this  when  docs  arrive.     Initial  ____  4. Downloaded  and  Printed  2  Affidavit  of  Identity  forms  and  1  Notary  Checklist.     Initial  ____  5. After  the  dealership  documents  have  arrived  to  me  via  Fed  Ex/UPS,  I  have  done  the  following:  

1. Called  the  customer  immediately  to  set  the  appointment  time.         Initial  ____  2. Uploaded  the  appointment  time  on  www.mavsign.com.             Initial  ____  3. Prepared  the  2  copies  of  the  Affidavit  of  ID  according  to  these  documents.     Initial  ____  4. At  the  signing  appointment  with  the  customer,  I  have:
