STEP 1:
PDA
CERTIFICATE
TEMPLATE
✓ Upload a PDA Provider logo (optional).
✓ Upload the Responsible Party’s signature (required).
✓ Upload other approvals (optional).
✓ Click the orange button ‘Submit Request’.
✓ PDA staff will review for approval. Turn-around time is approximately
one week.
✓ The RP will be contacted if adjustments are needed.
STEP 2:
EMAIL THE
PDA CERTIFICATE
Individual or Bulk Distribution?
PDA Certificates must be distributed
to the participants within two
weeks of the course completion.
Individual Distribution
✓ Complete all required fields.
✓ The PDA points and
recertification category will
automatically be
displayed at the bottom.
✓ Click ‘Email Certificate” and
the PDA Certificate will be
automatically emailed to the
attendee.
Bulk Certificate Distribution
The bulk distribution option allows the PDA Provider to
email multiple participants at one time. Each
attendee will receive their individualized NCCAOM®
PDA Certificate of Completion via email.
The first step is to enter all participant information into
an Excel spreadsheet called the NCCAOM® PDA
Attendance Record.
PDA ATTENDANCE RECORD
▪ This specific Excel spreadsheet MUST BE USED and NOT MODIFIED.
▪ Multiple courses may be entered in the spreadsheet by changing the course number.
▪ Enter attendee information and save to your computer.
▪ The maximum limit is 500 attendees per upload.
Bulk Certificate Distribution
✓ To upload the PDA Attendance Record
information into the system, click on the center
box (# 2) “Upload Attendance Record Template”.
✓ To send all the certificates, click on the box (#3) at
the right, “Email Certificates”.

Green ribbon icon:
Certificate emailed successfully
Envelope icon:
Saved copy of the PDA Certificate.
May resend certificate if a duplicate
is requested.
