Your donations at workYour donations at workMy name is Mark, I grew up in Guilford, and by the age of 14 Iwas an addict
I was adopted, I grew up in Guilford and by the age of 14, I was baling addicon. I
started out smoking pot, then bingeing on alcohol, and eventually added cocaine. For
the next 30 years, my only purpose in life was to drink and do drugs. You may or may not
be surprised to learn that for most of that period, I was technically
homeless. Id actually worked out a prey good system to keep
my self off the streets. I would go down to the hospital in New
Haven, tell them I was suicidal and theyd check me in for a few
days, and somemes a few weeks. That way I had a warm place
to sleep and people to take care of me. Being suicidal wasnt a

complete lie. I do struggle with major depression and have aempted suicide in the past in fact, in 1996 I
almost succeeded. I purposely overdosed on cocaine and pills (downers mostly), I wound up in a coma, had
a stroke and spent the next three years in a wheelchair. Somehow though, even that didnt stop me from
geng high. That didnt happen unl much later, in fact not unl October 4, 2010. I was back in the inpaent
unit in New Haven, doing my thing, planning to get drunk as soon as I was released, and my doctor came in
to speak with me. He basically told me that the jig was up, that I couldnt connue checking myself into the
hospital, that I needed to get help once and for all and start taking care of myself, or start living on the streets
more permanently. He wanted me to try Antabuse, (used for the treatment of alcohol abuse) and I said, No.
Being the good doctor that he was, he walked away, but his words resonated with me over the night, and the
next morning something in me changed. I wanted to try to get sober and I asked him for help. I started theAntabuse that morning, and I havent touched a drink or illegal drug since.
I was discharged from the hospital and admied to the HARP program at Harbor Health. I was immediately
enrolled in an intensive outpaent program (IOP) for people just like me who were baling to stay sober,
and struggling with mental illness. I spentfive days a week here (at Harbor Health) three in the clinic, and

even after you graduate, youre not forgotten.

two up at OPTIONS where I worked with my case manager, housing specialist, and
parcipated in recovery support groups.
I graduated from the IOP program, but I am not alone. I can pick up the phone any
day of the week and someone at Harbor Health is available to me. Thats what I
truly cherish, even aer you graduate, youre not forgoen.
I would be lost today without Harbor Health. They dont just handle addicon, they
cover all aspects, mental health, financial issues, housing, family issues. They even
brought my Mom in for a couple of my sessions to help us get back on track.
Harbor Health helped me rent a co
age in Madison so now I have a place to call home. They help manage myfinances, because money is a trigger for me. It feels good to pay my bills. I used to look down when I passed a
mirror, but now I look up, when I look in the mirror I feel proud and its an amazing feeling.
Im not saying that recovery is easy. I have a sponsor, I go to meengs and my roune is very important to me.
Im looking forward to my one-year anniversary this October, and I think down the road I may go into recovery-
related work.
Out of all of this Ive learned that no situaon, no illness and no disease cant be dealt with in a posive way,
on a daily basis. Its okay to ask for help, and once you do, youll feel proud too.
When I look in the mirror I feel proud and its an amazing feeling.

Looking back on the last year, we are so grateful for all that youve helped us provide.
Thanks to your generosity and support, more than 1,500 of your friends and neighbors
have access to crical mental health, substance abuse and community support services,
right here in their local communies.
We all know that the landscape of healthcare is ever changing, and we here at Harbor
Health Services do all that we can to evolve as the system advances and develops. There
are a lot of excing endeavors on the horizon for the next year, most importantly, our
upcoming merger with Birmingham Group Health Services in Ansonia.
The merger provides a fantasc opportunity for us to expand and enrich our services, provide treatment and
support for more people and extend the reach and impact of our mission. Over the next 18 months we will
integrate programs and services throughout our 40 town service area and will grow to treat more than 13,000
individuals and families. We thank you for your connued support during this important me and look forward
to working with you as we expand our horizons.

Programs & Services
Clinical Services:HHSI provides a wide range of comprehensive psychiatric and substance abuse treatments. Treatmentrounely includes assessment, diagnosis, prescripon and administraon of psychotropic medicaons, groupand individual therapy, psycho-educaon, and reviews of medical health history. Counseling services in
Spanish are made available through CommuniCare and the CT Lano Behavioral Health System. Overall last
year 1,572 individuals received 19,048 clinical services at HHSI.
Crisis Services:HHSI, through CommuniCare, provides both on-site and mobile crisis services, 24hrs/7 days per week, forthose clients who experience severe or crical mental health episodes. Walk-in crisis services are available at
our outpaent clinic, Mon. - Fri., 8:30 am - 5 pm. Mobile crisis services are also available during these hours for
persons who are homebound. Aer hours, weekend and holiday crisis services are provided through the South
Central Crisis Service (SCCS). Over the last year, HHSI provided 445 crisis services to 209 individuals.
Community Support Services:The Community Support Program is designed to assist adults with severe and persistent mental illness and/or

substance use disorders in achieving and maintaining rehabilitave and recovery goals. Services address the
mental health/substance abuse treatment needs of individuals as well as provide financial, social, educaonal,
vocaonal, residenal, jail diversion and other treatment and support needs of individuals. In the last year
1,343 individuals received 8,255 community support services.
Residential Services:Residenal services provide clients with 24/7 on-site support. The Harbor
Apartment and Respite Program (HARP) is an agency owned/managed apartment
complex that offers services in a cluster apartment model to individuals who can
benefit from staffsupervision 24 hours a day while living in their own apartment.
Harbor House is an eight bed, connually staffed adult psychiatric community
residence that provides a safe, structured, and suppor
ve environment in thecommunity in which residents with serious and persistent mental illness are assisted
in achieving their highest degree of independent funconing and recovery. In 2010, HHSI provided residenal
services for 67 individuals, who benefied from 23,345 services.
Tobacco Cessation:The tobacco cessaon program began in 2010 thanks to a grant awarded to CommuniCare by the CT
Department of Public Health. The program includes tobacco cessaon counseling and nicone replacement
therapies/medicine for nicone dependent individuals. The program links clients with Nicone Anonymous

and the CT Quit-Line to help support their recovery. To date, 176 HHSI clients have enrolled in tobacco
cessa
on services, of those enrolled, 95 percent reported a history of failed quit a
empts, and 62 percent ofthose who completed the program report either reducing their tobacco use, or complete cessaon.
Health Management Strategies for Recovery:Our on-site primary care program, Health Management Strategies for Recovery
(HMSR) is funded by a grant from the Substance Abuse and Mental Health Services
Administraon, and is a CommuniCare iniave. HMSR has been increasingly
successful in providing services to our community members.
188 clients are currently enrolled in the HMSR Program, and are assessed by the
nurse care manager on an average of every two to three months. Over the last year,
the nurse care manager provided 278 assessments. 104 clients, who had no access
to a primary care doctor, are now receiving on-site primary care services through our partnership with the
Cornell Sco Hill Health Center.

In AugustCongresswoman Rosa L. DeLauro (CT-3) visited CommuniCare toparcipate in a ribbon cung ceremony to celebrate our $2 million federalprimary care grant. The funds are being used to integrate primary care into
each CommuniCare partner agency clinic: Harbor Health Services in Branford,
Birmingham Group Health Services, of Ansonia, and Bridges, and A Community
Support System, of Milford.
On October 1, we began providing oversight and support for the Town of NorthHaven counseling services.
In October,OPTIONS held its first ever art show. More than 50 people gathered at the James BlackstoneMemorial Library to celebrate and enjoy the arsc talents of 10 of our clients.
We celebratedour 30th anniversary in November at our Annual Meeng. State Representave Patricia M.Widlitz presented us with an official citaon from the Conneccut General Assembly, introduced by herself,
Representave Heinrich, Representave Reed, and Senator Meyer congratulang us on this important

milestone. The citaon stated, You have improved the quality of life for many
people in the Shoreline Community. You have demonstrated a remarkablecommitment to the highest quality of behavioral healthcare while working
with individuals through all phases of recovery. You are a fine example of
collaboraon with both individuals and families in ways that help them achieve
their full potenal.
Thanks to a grantfrom Alcoa Howmet of Branford, we launched our new websitein January 2011.
Our second annualGirls Night Out Fashion Show Fundraiser was a great success, itraised $5,000 and drew a crowd of 175 women to Terminal 110 in New Haven on
March 18. Guests enjoyed a fabulous fashion show by Ella, where She shops
of Guilford, hair by Echo Salon, makeup by Jennie Fresa Beauty Library, and
fabulous jewelry by Amy Leiner. Terminal 110 served delicious food & drinks.
Guests also enjoyed bouque shopping, raffles and a silent aucon.

OPTIONS celebratedMental Health Month in May with a picnic and awarded SueSearles with the Volunteer of the Year Award. John Lezia donated a brand new
computer and printer in honor of Sues accomplishments and in memory of his father
Carlo Lezia.
Our 5th AnnualRoad to Recovery Gala and Aucon was held June 3 at AmarantesSea Cliffand was a tremendous success raising more than $20,000 to support our
programs and services. The Gala also gave us the opportunity to recognize Charloe
Maei for her hard work and volunteerism, and State Representave Patricia M. Widlitz as our Community
Partner for her years of connuous support and effort on behalf of our clients and our organizaon.
