The current era of anti-immigration has heightened realistic fears participants have about their ability to stay in the communities where they are raising their children, and is forcing a conversation about what may happen in an age of mass deportation. This unknown future has cast a shadow over the developmental processes of these families, and highlighted the desperation that sent them migrating in the first place. This CE presentation (for beginning to advanced practitioners) will share relevant theory, research and clinical practice of portable wellness practices to support and stabilize these families.

Karen is a somatic psychotherapist serving migrant families, and core faculty at Palo Alto University?s MA Counseling program. She served the foster care and migrant communities of Santa Cruz County for 15 years, including work in juvenile detention diversion programs addressing

disproportionate minority confinement. Karen is currently Clinical Coordinator at Family Connections in Redwood City, a First 5 parent-involvement preschool serving the migrant community of the peninsula.

As a Clinical Traumatologist and Compassion Fatigue Therapist, she has published on trauma training in counselor education programs to increase trauma-sensitive treatment for underserved populations, and reduce risk of Compassion Fatigue in counselors. In collaboration with Global Trauma Research, she is performing trauma training and outcome research in Haiti. She is a Fellow of the American Academy of Experts in Traumatic Stress, Clinical Fellow of the American Association of Marriage and Family Therapists, and Diplomate in the National Center for Crisis Management.

of 2017 to announce the 2017 RPA executive board, share appreciation for outgoing board members and leaders, and give an idea of what is to come in the year ahead for our organization. In getting into appreciation for outgoing board members and leaders, first, I want to share a big thank you to Greg Thigpen, Ph.D., the 2016 Past President , for chairing the election committee and working very hard to lead us in a successful election process. Alongside that I want to express gratitude to the members who ran for board positions to serve the RPA community. As the CEO of CPA Jo Linder Crow shared, having several members run for several positions in the past election is a great sign for the engagement of the organization.

Next I want to express deep appreciation for outgoing board members Meg Tippy, Psy.D. (2016 Secretary) and Michael Pinkst on Ph.D. (2016 Treasurer). Last but not least, gratefulness to Carol Weser , Ph.D. for her multiple years of service as CE Coordinator. I know each of these leaders and members of RPA will continue to play varying roles of service to the organization, but I wanted to make sure and take this opportunity to honor the work they have done in the past year. I truly believe if most members knew how much hard work and good intention has been shared with the organization from the members above, they would give each of them a big hug or a handshake in thanks next time they see them. I hope you take my word for it and thank Greg, Meg, Michael and Carol wholeheartedly if you get the chance.

President 's Colum n con'tThe current 2017 board has met two times this year

and I have found the group passionate, hardworking, ambitious and motivated. As we sat for our first meeting and shared why we wanted to serve on the RPA board, a wonderful shared vision of fostering a progressive, connected and engaged organization emerged.

I want to highlight several projects the current board has been working on. First, the organizational bylaws are due for revisions. A committee will be formed to tackle this task and members interested in playing a role in this can contact the committee chair Christine Naber.

Also there will be some changes and updates to the website and listserve to make both even easier and cost efficient for members. Stay tuned to organizational and listserve emails for updates.

Next, to learn how to best serve the organization as a board, we would like to hear from our members about what they want and need in RPA. First up will be a survey coming out some time in the Spring to all members asking for their interests in future Continuing Education events.

Another important item to note is that board procedures will be moving to align closer with CPA and other chapter organizations by starting to publish board meeting times and minutes either through organization wide emails and or postings on the website.

Finally, as the board moves forward this year we will be reaching out to invite members to serve and share their expertise and passions in various roles. If you are interested in serving in some way and or playing a leadership role in the organization, whether it be in a committee, at an event or a submission to the newsletter, please contact me or one of your board members to share your interest.

Birding enthusiasts and RPA members once again joined Jeffrey Martin as he led all on the Annual RPA Birding Adventure. The weather cooperated as members and their families explored the remarkable bounty of nature in our own backyard at Shollenberger Marsh

Members also enjoyed the post-birding brunch and took the opportunity to get to know each other better.

Dr. Jeffrey Martin presented an engaging evening on using psychotherapy homework to cultivate a corrective emotional experience in our patients. Participants were introduced to collaborative processes that tapped into their own skills and creative approaches to identify readiness and opportunities for patient change and working with psychodynamic resistance.

It?s hard to believe that Medicare hasn?t been around forever. But it is actually a ?new? program having been signed into law by President Lyndon B. Johnson in July, 1965. Since that time, Americans 65 and older, and individuals with disabilit ies have benefited by ever-expanding coverage to insure the quality of health as they age.

Providers, on the other hand, have had to adapt to an increasing maze of rules and regulations that govern this insurance program. The focus of this article is on helping psychologists understand where we fit into the overall scheme of payments, and hopefully to shed some light on the challenges of outpatient treatment under Part B.

Medicare Part A covered hospitalization and was funded through payroll deductions. Back in the 1960?s, hospitals typically provided services to people for emergencies, surgery, chronic care, and birth. Average length of stay for older adults in 1968 was 7.8 days, according to a JAMA article (Utilization of Short-Stay Hospitals.

Hospitalization has always been the most expensive covered service, and hence, it is the one that most regulations are written to manage.

Medicare Part B was initially an optional program, where individuals would pay a small additional fee out of pocket to cover limited outpatient treatment. Both Part A and B were known as ?fee-for-service? programs, and the Federal government set reimbursement rates for these services using the ?usual, customary, and reasonable standard?, which was then adopted by the Centers for Medicare and Medicaid (CMS). This held true until the Omnibus Reconciliation Act was signed in 1989. Congress adopted a new method to calculate payment known as ?relative value units? (RVUs) that consisted of three factors: the physician?s work, practice expense, and malpractice costs. Once identified, this RVU is then assigned to a specific CPT code. (Physicians Practice

According the Physician?s Practice, ?CMS sets RVUs based upon the recommendations of the Specialty Society Relative Value Scale Update Committee (RUC). The RUC is made up of 29 physicians, 23 of whom are nominated by professional societies. Almost all are specialists. CMS is not bound to accept either the professional society nominees or the RUC's recommendations, but it has historically approved more than 90 percent of RUC recommendations.? Various groups have argued for more transparency, including private insurers, employer-sponsored plans and other interested groups. Psychologists and other non-physician providers do not have a seat at the table.

Two other items go into the fee calculation: Geographic Practice Cost Indices (GPCI) and a Conversion Factor (CF). A GPCI is calculated, by CMS, for each of the RVU components. The GPCIs are reviewed every three years and attempt to take into account the different costs associated with different areas of the country. Sonoma County has suffered under this as we are still considered a ?rural county?, but unlike our surrounding neighbors in the North Bay, CMS has determined that we are should receive a very low GPCI and RVU. We cannot blame CMS completely, as they are limited by Federal statute in how much they may increase their annual budget (no more than $20 million.) If shifts in the RVUs would increase CMS' budget by more than $20 million, the CF is used to achieve, essentially, budget neutrality. Other terms that are necessary to understand in finding the correct reimbursement rate include ?Non-Facility? Payment Amount (e.g., a physician's office). Inpatient facilit ies, hospital outpatient clinic

settings, and off-site hospital-owned locations are considered "facilit ies." The payment for each CPT code in a non-facility is calculated as follows:

Payment = [(Physician Work RVU X Work GPCI) + (Non-Facility Practice Expense RVU X Practice Expense GPCI) + (Malpractice RVU X Malpractice GPCI)] X (Conversion Factor, adjusted for budget neutrality) Just so you don?t get too upset, back in 2014, the largest fee increases went to psychiatry, clinical psychologists, and clinical social workers, as well as other providers of mental health services. This, however, was offset by the Sequestration orchestrated by the Republicans when they shut down the government in opposition to the Affordable Care Act.

So, is everyone clear? A simple explanation is that as psychologists, we are at the mercy of a secretive group within which we have no representation, who recommend to CMS changes in rates that may be overridden by regulatory restrictions that continue to determine the value and worth of our services.

The Affordable Care Act began a process of shifting from this fee-for-service model by replacing the Sustainable Growth Rate (SRG) with a value for outcomes model with the passage of the Medicare Access and CHIP Reauthorization Act (MACRA). This is a quality based system designed to give

providers financial incentives to keep

people healthy and out of the hospital. Because of the change in administration and the current majority party platform to Repeal and Replace the ACA, it is not clear how MACRA and the Quality Payment Program (QPP) will unfold.

Michael Newm ark , Psy.D. I began my private therapy practice in Santa Rosa seven months ago, and I?m grateful to be doing this work. I hold a B.A. in Psychology from Vassar College and an M.A./Psy.D. from the Wright Institute in Berkeley. I enjoy treating a variety of clinical concerns (addiction, relationships, eating disorders and trauma, most notably), and I utilize a flexible and client-centered approach, developed over years of finding my own therapeutic voice. I believe in the power of personal connection to enlighten, soothe and heal, especially in today?s difficult and disjointed world. And I look forward to cultivating further connections with the North Bay?s psychological community.
