the apex beat (lat. ictus cordis), also called the point of maximum impulse (PMI), is the furthermost point outwards (laterally) and downwards (inferiorly) from the sternum at which the cardiac impulse can be felt. the cardiac impulse is the result of the heart rotating, moving forward and striking against the chest wall during systole. it is also known as the "apical impulse.


lateral and/or inferior displacement of the apex beat usually indicates enlargement of the heart, called cardiomegaly.

sometimes, the apex beat may not be palpable, either due to a thick chest wall, or conditions where the stroke volume is reduced; such as during ventricular tachycardia or shock.

the character of the apex beat may provide vital diagnostic clues. a forceful impulse indicates pressure overload in the heart (as might occur in hypertension).


an uncoordinated (dyskinetic) apex beat involving a larger area than normal indicates ventricular dysfunction; such as an aneurysm following myocardial infarction.

Matthew Prouty December 5 at 12:57pm Ed, I have a question regarding the concept of Qi. From lectures on space time theory, I understand qi as the signature of space and time created by the dynamic breath of yin and yang - the breath of the universe. In this weeks lectures, you referenced passages describing each Jing Mai in terms of amounts of qi and blood contained within each system. When you read a passage such as this, what comes to your mind in terms of defining Qi in this context? My mind has moved away from thinking of channels carrying "qi" as we studied in TCM school, but clearly this aspect of each Jing Mai is important. Thanks.

Lower yangming branches 43:26 on the video, branch 4 ends on the inner aspect of the second toe and the 5th branch ends on the outer aspect of the second toe. But at 51:15 the arrows show this on the 3rd toe, suggesting the stomach mai covers the 3rd, 2nd toes and lateral aspect of big toe? thx


Lower yangming stomach mai river - 6 branches

First branch - bridge of nose, upper teeth, great welcoming (daying), jaw axle (jiache) , guests host (kezhuren), forehead

Second branch - great welcoming (daying), human welcoming (renying), open basin (quepen), stomach, pancreas

Third branch - open basin (quepen), breast, umbilicus, qi ravine (qijie)
