NBSD QUARTERLY OSH POLICY/TRAFFICRODS SAFETY COUNCIL MEETING

The NBSD Quarterly OSH Policy/Traffic/RODS Safety Council meeting was conducted on 06 December 2011, in Building 72, Nimitz Conference Room, at 1000.

The purpose of the council is to identify, define and assess issues, problems and needs at NBSD and make recommendations to improve the effectiveness of the NAVOSH, Traffic and RODS programs.

A Safety Rep gave opening remarks and reviewed minutes from the last meeting. After introductions the NBSD XO, spoke briefly about motorcycle traffic, vehicles transiting the base, and drivers going around speed bumps. Base Construction / Areas of Caution / GatesPlease disseminate the following information to your Sailors and employees so they can plan which gate to enter:

- Gates Hours & Manning- Gate 2: 0530-1700 (2 sentries during rush hour)- Gate 6 (Main Gate): 24hrs (3 lanes/3 sentries during 0500-0800)- Gate 6A: commuter traffic 0530-0800; then truck deliveries 0800-1430 - Gate 9: 0530-2000 (4 sentries during rush hour)- Gate 13: 0500-0800 / 1500-1700 (2 sentries)- There are no changes in hours of operation on the dry-side

Outside the Blue line:Upon NBSD’s request, the city of San Diego has begun looking into the following traffic improvements:

1. Adjusting the Gate 6 traffic light for outbound traffic to permit left turns when trolley is present.2. Installing a left turn signal off the I-15 into the Norman Scott entrance. Traffic coming off the I-15 would have a green light to turn into the dry side or proceed onto 32nd St. Doing so could enable opening of Norman Scott Gate (start with rush hour first). 3. Installing a left turn signal at Main St and Vesta St so northbound traffic can make a protected left into the dry side of NBSD.4. Laying a right turn lane at Main St. and Vesta St so southbound traffic can make a right into the dry side of NBSD. 5. Laying a left turn lane at Main St and Yamma St so that northbound traffic can make a left into the dry side of NBSD.

Base Projects Update:- Vesta St & Cummings Rd in front of B-94 area has been slurry sealed, restriped,

wheel stops around perimeter of roadway and area opened back up at the end of NOV 2011.

- Gate 7 closed on 28 NOV 2011 for renovation of the gate guard house by the Seabees. This gate will be secured for at least 6 months. During this time Gate 6-A will handle all the commercial vehicle traffic from 0800 to 1430. Keep in mind that Gate 6-A is open to regular morning traffic from 0530 to 0800. See attachment for other gate hours.

- Parking Lot (PL) 3-K has been asphalt overlayed, restriped, and reopened on December 2nd. The area south of B-3486 (old Q-Mart) will be one of the laydown areas for PL 405 (BEQ homeport ashore & parking structure) which will take in PL 3-H and a portion of PL 3-I starting on 19 DEC 2011 and continue until completion.

- Area west of B-3300 where there used to be MWR personal storage will be turned into a new asphalt parking area, PL 3-R starting 19 DEC 2011.

- B-118, B-119, B-121 and B-291 compound area will be asphalt overlayed and slurry sealed starting from 05 - 27 DEC 2011. This will be done in two phases, first the concrete section will be striped on 09 DEC, opened up, then the asphalt area will be slurry sealed from 12 - 26 DEC and restriped on 27 DEC 2011.

- Pier 12 will be demoed and a new pier built in its place. This is set to occur sometime during the FY 2012 timeframe.

- Solar powered flashing crosswalk will be installed during the week of 27 - 30 DEC 2011 at the Cummings Rd & Surface Navy Blvd (SNB) intersection. There will also be some non-intrusive work in that area with directed traffic control during the week of 19 DEC 2011.

During construction periods there will be directed traffic control. Request all personnel utilize alternate routes if possible. Personnel are requested to obey all traffic directing personnel & related signage during construction.

Please wear reflective material when you jog and watch for vehicles. Stop for pedestrians and do not smoke in GOVs or while walking in uniform. Put your trash in the right place and keep our base clean.

Navy PMV Fatalities Update Personal Motor Vehicle (PMV) mishaps result in the largest number of off-duty Navy deaths each year and are always a subject of concern. There have been 6 traffic-related fatalities so far this fiscal year. From NSC, these are the vehicle fatality stats as of 0800 06 December 2011, both two wheel and 4 wheel.

Stay involved with your subordinates, mentoring and even parenting them on occasion, to keep them from adding to this list. While we have not had any pedestrian or jogger fatalities, the possibility is there. As always, be an active participant in your travels and play the “what if” game.

Military Sport Bike Rider Courses – conducted on NBSD, in the former “bottle yard”, just south of Bldg 280.

To sign up for traffic classes as well as dirt bike, ATV classes, and more log on to ESAMS and click on “Classroom Training”, or visit www.navymotorcyclerider.com which will link to the same class registration page available to all ESAMS customers. All riders should be encouraged to continue their education through reading technique guides and attending advanced riding classes.

Operational Risk Management/TrafficSeatbelt Survey

In a Seatbelt Survey conducted on 30JUN11 at Gate 6, 74 inbound cars were observed. One driver was not wearing a seatbelt, and two passengers were not wearing seatbelts for a total of 4% non-compliant inbound drivers and passengers. At the same time, 36 cars were observed in the Outbound Lanes. Three drivers and one passenger were not wearing their seatbelts. This gave a total of 11% non-compliant outbound drivers and passengers. Combining the inbound and outbound surveys gave a total compliance percentage of 94%.

In a more recent seatbelt survey conducted on 16 November 2011, at the intersection of Cummings Rd / Southall Rd (JNO Gate 9), 170 vehicles were observed. Four drivers and one passenger were not wearing seatbelts, for a total compliance of 97.1%.

Starting next year, the Safety Office staff will be conducting random safety surveys for proper usage of safety equipment on the installation roadways, to include seatbelt usage in low speed vehicles such as golf carts and forklifts.

OSH Inspection Findings Upon review of the most commonly found hazards during the Occupational Safety and Health Inspections many of the specialists reported the following:

- Electrical breaker panel blocked- Daisy chained re- locatable power tap- Fire extinguisher not visually inspected and documented on a monthly basis- Broken faceplate electrical receptacle- Emergency exit light not working properly- Personnel delinquent in training (Motorcycle, Ergonomics, NAVOSH Orientation)- Chemical bottles not labeled with the corresponding number of the MSDS sheet- Area in need of housekeeping - Employee unable to locate MSDS - Hearing exam overdue- Breaker box has an unused opening- Eyewash sprinkler head comes off when eyewash is activated- Shelving units not secured to prevent tipping- Light bulbs are not guarded- Evacuation plan is missing from building- Exit door blocked- Fire extinguisher missing

Safety is everyone’s responsibility. Please ensure that your coworker, supervisor, subordinates, and ancillary personnel are performing their work safely. Working in a safe environment can cut down on injuries, time, and money.

Motorcycle UpdatesPersonnel operating motorcycles and other authorized two-wheel, non-self powered vehicles on board NBSD will comply with training requirements in paragraph 13 of OPNAVINST 5100.12H, and the licensing, registration and insurance requirements of NAVBASESANDIEGOINST 5560.5G.

a. No motorcycle or motorized bicycle, including mopeds, shall be operated on board NBSD unless operators and passengers are wearing protective clothing as listed below:

(1) A protective helmet properly fastened under the chin, which meets U.S. Department of Transportation (DOT) standards.

(2) Eye protective devices, designed for motorcycle operators (impact/shatter resistant), i.e., safety glasses, goggles, or face shield attached to the helmet. A windscreen or fairing is not considered proper eye protection.

(3) Protective sturdy footwear such as over-the-ankle shoes or boots that will protect the rider’s foot and ankle from abrasion during a mishap.

(4) Full length trousers. Recommend trousers be constructed of sturdy denim or other material of equivalent abrasion resistance.

(5) Full finger gloves designed for use on a motorcycle.(6) Long-sleeve shirts or jackets that protect the arms. Sleeves shall cover the

rider’s entire arm, so that no skin is visible. If a jacket is worn with a uniform, it must be plain with no markings except safety markings. Motorcycle specific jackets are recommended.

(7) Reflective safety vest. All personnel shall wear a yellow or orange visibility vest as an outer garment at all times while riding on NBSD.

b. If a backpack is worn while riding, it must be either covered by the vest, a separate vest secured around it, or reflective material sewn to it. Any method used must clearly meet the intent of maintaining rider visibility. The backpack shall have at a minimum, retro-reflective material strips at least two inches wide prominently visible while riding.

c. Military service members, civilians, contractors, and dependents attempting to enter or stopped onboard NBSD for violation of safety items 3(a) (b) of this instruction will result in confiscation of the vehicle or denial of base access as appropriate until evidence is provided that all proper PPE is being utilized. A citation will be delivered to the service member’s Command Master Chief (CMC), (or Director if no CMC) for corrective action. In the event the individual is cited three times for improper protective gear, motorcycle driving privileges will be revoked onboard NBSD and associated areas.

d. Motorcycle operators will not rev their engines while riding on base, and shall not accelerate quickly to make a display of speed. Motorcycle operators will obey posted speed limits and will not circumvent speed bumps by swerving around them.

Strict compliance with the provisions of this instruction by all personnel within the jurisdictional areas of the Commanding Officer is directed. Violators of noise and speed rules will be cited and may be barred from the base.

Current Mishap Trends Analysis will be reviewed in the next meeting.

Slips/ Trips/Fall We are 67 days into this fiscal year, and so far we have had a total of 4 incidents; 2 slips, 1 trip, and 1 fall. - By the end of fiscal year 2011, we had accumulated 4 slips, 6 trips, 3 falls, and 27

other injuries classified as contusions, pinches, twists, sprains, faints, and strains. Thank you for your diligence in documenting workplace and off-duty mishaps. If you have any question regarding the entry of a mishap into ESAMS, please call us for guidance.

*Remember to report all injuries to your supervisor and record them in ESAMS.

There were no council action items for this quarter. Zone inspections are still in progress every Tuesday with the CO, XO or CMC. Remind your people use designated sidewalks and crosswalks. Xeriscape around

the base is becoming worn from pedestrians not using sidewalks.

Open Forum: A member expressed the recent survey performed at Navy Medical Center San Diego

revealed that six (6) ATV drivers were not wearing seatbelts. Do ATVs have the same rules as GOVs?Answer: OPNAV regulations still apply and PPE is required when operating ATVs.

Another member suggested that the next seatbelt survey should be performed at the first stop sign in lieu of the gate entrance. “It would be a different perspective”. Answer: This will be looked into further.

A member had concerns about motorcycle passengers lack of PPE when entering the base. Should have the same PPE as the rider?Answer: Yes, per the base instruction, riders must wear bright orange or yellow vests for visibility. Passengers must also follow the instruction.

Another member asked if assessments in workplace are performed by safety office for PPE justification. Answer: No, this is an internal issue. The Industrial Hygiene survey will have the required information needed for PPE use/placement.

A member had concerns about pedestrian safety on 28th Street outside the gate by Navy Lodge. The area is not well lit in the morning and there is no defined crosswalk. Can NAVFAC install a light in the area?Answer: This area is outside the blue line but safety can ask NAVFAC and report back next meeting any definitive answers or suggestions that NAVFAC may have.

A member asked is it legal to ride a bicycle on the sidewalk? Answer: Yes, however the bicyclist has to stay on the sidewalk and cannot alternate from sidewalk to street. The bicyclist must also go in the same direction as the flow of traffic.

A member wanted to reiterate a concern for joggers during peak traffic hours wearing hearing devices. Answer: Yes, jogging during peak traffic hours should be discouraged and the use of off road/jogging paths are encouraged.

A member asked if pedestrians are allowed to use a cell phone while walking. Answer: Yes, however the pedestrian should be using a handheld phone. Bluetooth devices are not allowed while walking. Pedestrians are not allowed to use a cell phone in a crosswalk.

Adjournment: The meeting was adjourned at 1121. It is requested that you disseminate these minutes to personnel not having electronic access, and brief them on contained pertinent information. The next meeting is tentatively scheduled for 1000 07 MAR 2012. All commands are encouraged to have a representative present.
