VISIONAn Ireland where fewer lives are lost through suicide, and where communities and individuals are empowered to improve their mental health and wellbeing.

MISSIONAn Ireland where individuals, communities and organisations are provided with the awareness, knowledge and capacity to identify those at risk, respond appropriately and contribute to a reduction in the rates of suicide and self-harm.

OVERALL GOALTo enable NOSP to oversee the coordination, quality assurance, monitoring and evaluation of all education and training actions identified in Connecting for Life. This work will build the capacity of government departments, NGOs, community organisations, groups and individuals to identify and respond appropriately to people at risk of suicide and self-harm.

OUTCOMESWithin the lifetime of Connecting for Life, education and training in suicide prevention, intervention and postvention contributes to:

• Reduced suicide rates in the whole population and amongst priority groups.

• Reduced rates of presentations of self-harm in the whole population and amongst priority groups.

OBJECTIVES

1. Provide a suite of standardised training programmes for the general public, community care givers, professionals and volunteers.

2. Provide suicide prevention and mitigation education and training programmes focused on supporting the practice of frontline health and social care professionals.

3. Ensure a consistent and standardised approach to the provision of education and training through the development of a National Quality Assurance Framework.

4. Monitor and evaluate the effectiveness and cost-effectiveness of suicide prevention education and training programmes in line with Connecting for Life.

5. Will provide oversight for the coordination and implementation of the Education and Training Plan.

Guiding principles This National Education and Training Plan 2019-2020 is underpinned by the following guiding principles:

Collaborative – Suicide prevention is the responsibility of all. This plan ensures that all suicide prevention education and training is planned and delivered in collaboration with key strategic partners across all relevant sectors and government departments.

Accountable – Implementation structures to support this plan will be built on accountability, competence and openness to ensure quality and delivery of education and training in the most efficient and cost-effective way.

Responsive – Based on the premise that suicide can be prevented, all suicide prevention education and training will be responsive to the level of need, accessible and appropriate. Prioritised among those most likely to come in to contact with individuals at risk of suicide, and prioritised in settings which can have the greatest impact on suicide and self-harm rates.

Evidence-informed and outcome focused – All education and training will be evidence-informed, quality assured, evaluated and outcome focused, contributing to the strategic goals of Connecting for Life.

Adaptive to change – Suicide prevention education and training will be continually reviewed. Providers must adapt to emerging evidence and changing circumstances and be open to implementing new and successful evidence-based programmes, which can best contribute to the outcomes of Connecting for Life.

Section 1 Developing the National Education and Training Plan1.1 Introduction

Connecting for Life (CfL) Ireland’s National Strategy to Reduce Suicide 2015-2020, sets out a vision of “an Ireland where fewer lives are lost through suicide and where communities and individuals are empowered to improve their mental health and wellbeing”.

This vision is to be realised through seven goals;

1. Better understanding of suicidal behaviour

2. Supporting communities to prevent and respond to suicidal behaviour

3. Targeted approaches for those vulnerable to suicide

4. Improved access, consistency and integration of services

5. Safe and high-quality services

6. Reduced access to means

7. Better data and research.

The defined outcomes of the strategy are;

• Reduced suicide rate in the whole population and amongst specified priority groups

• Reduced rate of presentations of self-harm in the whole population and amongst priority groups.

Connecting for Life is a whole of society strategy to co-ordinate and focus our national effort to reduce the loss of life by suicide. Addressing suicidal behaviour means supporting people in many different ways and requires a coordinated effort across many different sectors and levels of society. Usually, no single risk factor is sufficient to explain a suicidal act.

The need for an evidence-informed, coordinated and quality assured approach to suicide prevention education and training was identified and recommended from a number of strands in the process to develop Connecting for Life. These included:

• Policy Paper on Suicide Prevention – A review of national and international policy approaches to suicide prevention, commissioned by the NOSP.

• A review of the evidence base for interventions for suicide prevention by the Health Research Board (HRB): Suicide Prevention: An evidence review, commissioned by NOSP.

• An internal review of Reach Out - the Government’s previous suicide prevention strategy.

• The outputs of the Practice Improvement Working Group, whose remit was to make recommendations to the Strategy Planning Oversight Group on suicide prevention. The recommendations from this group included a number of areas related to training and education.

In 2014, as part of the development of Connecting for Life the NOSP commissioned a review of the provision of training. This review involved three key components:

1. To ascertain, through a national mapping exercise, the level and type of suicide prevention and mental health promotion training being delivered by key stakeholders from the voluntary and statutory sectors (under Reach Out).

2. To determine what knowledge, skills and competencies in relation to suicide prevention were needed by the stakeholders at different levels and what gaps existed in training to meet these needs.

3. To identify the structures needed to integrate training into the new Connecting for Life Strategy (2015-2020).

1.2 Mapping training provision

A national mapping exercise was undertaken across the statutory and non-statutory sectors in order to identify training being delivered under Reach Out. This mapping exercise captured, for the first time, a comprehensive overview of the nature and type of mental health promotion and suicide prevention training taking place in Ireland. A specific mapping document was produced for NOSP (June 2014), which presented an outline of all related training provided or coordinated by the HSE and by relevant non-statutory services and organisations across the country. Information on all relevant courses and training was presented under the following headings;

• Existing training courses/programmes currently provided

• Nature of the training (mental health promotion or suicide prevention)

• Target audiences for the training

• Duration of the training course/programme

• Lead agency involved

• Training coordinator

• Was the training course evaluated and is information available on same

• Outputs (numbers of courses provided / numbers of participants trained in the previous year)

• Training outcomes for participants.

1.3 Analysis of training needs

A national Training Needs Analysis (TNA) was undertaken to determine the training needs (knowledge, skills and competencies) required by key stakeholders at different levels who have a role in suicide prevention across statutory and non-statutory sectors. The TNA identified five different levels of stakeholder groups.

Training needs were identified by and for each of these stakeholder levels with gaps identified at each level as relevant. Recommendations were made by key stakeholder groups regarding the future provision of training. Appendix 1 (page 30) provides details of the themes arising from the TNA.

1.4 Role of education and training

Education and training has been identified as one of the key components of work to achieve the vision of Connecting for Life. Education and training is a frequently used intervention in suicide prevention. The Health Research Board, 2015 evidence review highlighted that suicide prevention interventions are rarely delivered in a vacuum; thus presenting challenges in attributing changes in the national suicide rate to a particular intervention.

Outcome monitoring tracks the results or effects of the work. Outcomes related to changes in attitudes, knowledge, capacities and motivations of individual suicide prevention training programmes, have reported a number of benefits;

• Improved knowledge of risks associated with suicide

• Positively shaping attitudes to suicidal behaviour

• Improved awareness of the signs of a person at risk of suicide

• Increased knowledge, enhanced positive attitudes towards and confidence in dealing with people who self-harm

• Confidence in addressing the issue of suicide and communicating about suicide.

• Increased confidence in dealing with people at risk of suicide

• Increased confidence and ability to connect a person at risk of suicide with services.

In 2017, the NOSP Monitoring and Evaluation team conducted pilot studies of HSE funded suicide prevention training programmes. The dashboards below provides information on the aims and outcomes of these evaluations.

To support the five different levels of stakeholders as identified in the Training Needs Analysis, Connecting for Life recommends the provision of a suite of standardised training programmes in suicide prevention, intervention and postvention. The suite of training available will allow progression from more generalised training to high-level, specialised and accredited training. Training and education is widely recognised as pivotal in supporting good practice among health and social care professionals.

The HSE National Mental Health Promotion Plan will provide a framework for upstream activity in the area of mental health promotion and wellbeing. The National Office for Suicide Prevention (NOSP) recognises that mental health exists along a spectrum, with mental health promotion providing the opportunity for upstream activity.

The table below provides an overview of the recommended programmes for key target groups identified in Connecting for Life. A detailed matrix of the most relevant training programmes recommended for the target groups requiring training as identified in Connecting for Life is presented in Appendix 1 (page 30).

Section 3Action plan for the implementation of the National Education and Training Plan 3.1 Implementation

The National Education and Training Plan 2019-2020 will provide the National Office for Suicide Prevention (NOSP) with an overarching framework to support the coordination, quality assurance, monitoring and evaluation of education and training actions identified in Connecting for Life. This work will build the capacity of government departments, NGOs, community organisations, groups and individuals to identify and respond appropriately to people at risk of suicide and self-harm. The plan centres around five objectives derived from the themes that emerged from the strategy development process.

1. Provide a suite of standardised training programmes for the general public, community care givers, professionals and volunteers.

2. Provide suicide prevention and mitigation education and training programmes focused on supporting the practice of frontline health and social care professionals.

3. Ensure a consistent and standardised approach to the provision of education and training through the development of a National Quality Assurance Framework.

4. Monitor and evaluate the effectiveness and cost effectiveness of suicide prevention education and training, in line with Connecting for Life.

5. Support and coordinate the implementation of the National Education and Training Plan 2019-2020 ensuring appropriate systems, structures and roles are in place.

There is no definitive theory or no single framework commonly accepted on how health strategy should be implemented but there is consensus that implementation is a process and not a one-time event. Implementation occurs in distinct stages (Exploration, planning, implementation & business as usual). The process is not a linear one, one stage does not crisply end as another begins; you may move back and forth between implementation stages1.

To take cognisance of the above implementation phases the Education and Training Plan will be a live document, a review of the 2019 actions will commence in June 2019 to inform 2020 actions.

NOSP has overall responsibility for the implementation of the National Education and Training Plan.

3.2 National Quality Assurance Framework (QAF)

A National Quality Assurance Framework (QAF) will be developed within the NOSP to ensure quality assurance for education and training, improving quality assurance approaches across existing and new education programmes. The NOSP will work closely with Resource Officers for Suicide Prevention (ROSP), training providers, government departments, funded agencies and the HSE - to ensure consistency, standardisation and quality assurance across all relevant training programmes and processes. The QAF will link closely with key NOSP divisions, providing a structure against which the suite of education and training programmes in Suicide Prevention can be assessed.

This QAF should include key areas which can be usefully benchmarked against the Quality Assurance Guidelines and Criteria for Further Education and Training2 as follows;

• Quality and risk management [in line with the HSE National Policy on Incident and Risk Management

• Recruitment and development

• Programme development, delivery, review

• Access and progression

• Governance and resourcing

• Collaborative provision

• Evaluation and improvement of programmes.

3.3 Coordination of training at CHO level

There are currently 17 local Connecting for Life Action Plans at a CHO level. Funding is provided by the NOSP to each CHO to support the delivery of a suite of Education and Training programmes with co-ordination led at a local level. The role of the designated ROSP for Training and Education is to work closely with the NOSP Quality and Strategy function to identify suicide prevention training need and the optimum structures for the implementation of training at CHO level.

3.4 Model of training

Over the lifetime of the Education and Training Plan the NOSP will work with ROSPs to identify sustainable, efficient and cost effective training models which will support the delivery of training from the suite of Education and Training programmes, including the cascade and hybrid models.

In partnership with the HSE, government departments, agencies and professional bodies’, trainers will be identified from within their own structures to be trained in relevant suicide prevention programmes. This will facilitate a comprehensive training for trainers and cascade model of training within those departments and agencies most likely to come into contact with people who are vulnerable to/at risk of suicidal behaviour.

In line with good practice all training on suicide prevention within HSE services, government departments and agencies will be delivered in the context of guidelines and protocols on responding to suicidal behaviour as described in Connecting for Life, goal 3, objective 3.1, action 3.1.2.

3.5 Area Training Plans

Local Area Training Plans will be closely aligned to the Local Connecting for Life Action Plans, comprehensively addressing the coordination, delivery, monitoring and evaluation of all training delivered in response to local needs. It is recommended that, where applicable, Area Training Plans will identify local annual training targets.
