VI. TEACHING (OHSU Educator’s Portfolio):

Overview of your Role as an Educator:

My academic focus is surgical education. My role as an educator is my top priority as an academician. I

have and continue to pursue the skill set that I need to become an effective clinician-teacher-researcher. I

have completed the OHSU Education Leadership Initiative (ELI) program in 2008. This year long course

supported a cohort of OHSU faculty from differing fields and provided the elemental principles behind

educational theory. I have also completed a Surgical Education Research Fellowship sponsored through

the Association of Surgical Education (ASE). This program paired novice researchers with mentors

experienced in surgical education research. The course was designed to provide the fundamentals of

research methodology used in surgical education research. Through this program I evaluated the

educational value of morbidity and mortality (M&M) conference. I hypothesized that introduction of a

standardized format of presentation would improve the quality of M&M presentations and improve

learning outcomes for both junior and senior M&M attendees. This work was presented at the American

College of Surgeons, 2010 Clinical Congress meeting and has been submitted to the ASE meeting.

I am currently enrolled in a Master’s of Surgical Education program at Imperial College in London,

England. This master’s program provides sound theoretical background to the principles of education,

curriculum design, and assessment. My research and dissertation will focus on the assessment of

operative competence in Vascular Surgery. The design of the study utilizes both qualitative and

quantitative research design; it is my hope that I can develop a tool for assessing intraoperative operative

competence using both qualitative and quantitative research methods.

My main teaching environment is the surgical ward, clinic and operating room, where I attend as an

Assistant Professor. I care for patients while teaching medical students, surgical residents, as well as

vascular surgical fellows. The majority of my clinical teaching of medical students and surgical residents

is through one-on-one precepting, although I do teach in various other settings, such as small and large

groups for didactic sessions and discussions. I teach surgical operative skills and techniques primarily in

the operating room through a one-on-one basis but I also teach basic open and endovascular operative

skills to medical students and surgical residents in a weekly vascular surgical skills laboratory.

Curriculum Development:

Title: Vascular Surgical Skills Lab: Low Fidelity Simulation Curriculum for Surgical Residents Role: Designer- responsible for content and development of the modules.

Purpose/Need: Teach surgical skills outside of the operating room, in a safe learner centered

environment. The vascular surgical skills taught in this lab are considered essential for beginning vascular

surgical residents on a vascular surgical service. These skills include: performance of an ABI and a

vascular specific physical examination, interpretation of non-invasive vascular laboratory studies,

ultrasound guided percutaneous vascular access, ultrasound assessment of venous conduits for bypass

grafting and dialysis access, techniques of performing basic vascular anastomoses, interpretation of

imaging studies pertinent to vascular surgery (angiography, CTA , MRA), radiation safety, fluoroscopy,

and basic catheter skills.

Intended audience: General Surgical Interns, PGY 2-4’s

Duration: 2 hours/module, 4 weeks/module, 40 weeks year. 2007- 2009.

Methods/Design: Modules include didactic portion, video demonstration, and deliberate practice with

formative feedback.

Significant Results & Outcomes: Resident performance is evaluated using the objective structured

assessment of technical skills (OSATS) checklists and global rating scales, final product ratings,

completion time and offline videotape assessments.

Dissemination:

Title: Increase in complement with reorganization of the OHSU Vascular Surgical Fellowship

Program

Role: Program Director for Vascular Surgery at OHSU

Purpose/Need: The vascular program has been restructured to accommodate a 3rd

vascular resident. The

curriculum has been expanded to include a 3rd

rotation. Rotations include 2 clinical rotations at OHSU

and Emmanuel Legacy Hospital. A 2nd

OHSU rotation has been created to allow the resident to gain more

experience in endovascular techniques (Dotter Institute), endovenous laser techniques (Center for Health

and Healing Vein Clinic) and outpatient clinical experience (OHSU Vascular Clinic). A vascular lab

reading and interpretation course has also been designed and integrated into this rotation. A fellow’s

lecture series has been implemented into the program to cover topics essential to a vascular surgical

practice.

Intended audience: Vascular fellows

Duration: 2010-present

Methods/Design: The curriculum format has been restructured according to the ACGME competencies.

Significant Results & Outcomes: Inclusion of M&M evaluation form, conference presentation

assessment tool and operative assessment tool.

Title: Vascular Surgery Cadaver Dissection Course

Role: Associate Director for surgical simulation, VirtuOHSU. Designer- responsible for content and

development of the course.

Purpose/Need: This course was designed to help 3rd, 4th and 5th year surgical residents understand

difficult anatomical relationships with regard to vascular surgical exposures. This Course was designed to

complement the ACS/APDS phase 2 national surgical skills lab curriculum.

Intended audience: General surgical senior residents and vascular surgical fellows.

“Strength in What Remains” by Tracy Kidder was discussed in 2010 with the group. The discussion

emphasized the role of perseverance and resilience on personal achievement particularly within medicine.

The book “Cutting for Stone” by Abraham Verghese was discussed in 2011 with the group. This

simulator. Non-invasive vascular lab interpretation and

vascular imaging modules are also included in this

comprehensive vascular skills lab.

seminar are all junior faculty from multiple specialties in the clinical-educator track with

an interest in education. The course includes the following topics: instructional design,

learning styles and managing learner behavior, cooperative or group/team based learning,

selecting the appropriate instructional method, clinical/workplace teaching and assessing

curriculum. Approximate time: 8 hours/week.

2008 E. J Wylie Travelling Fellowship Award.

Lifeline Programs of the American Vascular Association (AVA).

The E. J. Wylie Memorial Traveling Fellowship awards $12,000 for expenses of travel,

research, and clerical help.

Aims: The E. J. Wylie Memorial Traveling Fellowship provides the recipient with the

opportunity to visit a number of vascular surgery centers in the United States and abroad.

Though brief, these visits stimulate academic inspiration, promote international

exchange, and foster development of fraternal fellowship in vascular surgery. The

achievement of these objectives will help to enhance the development of the fellow's

career in vascular surgery.

2009 Surgical Education Research Fellowship (SERF) Program.

Association for Surgical Education.

This one-year home-site fellowship sponsored by the Association for Surgical Education

is designed to equip investigators with the skills and knowledge needed to plan,

implement and report research studies in the field of surgical education. The program

provides a faculty advisor to serve as the fellows mentor and consultant on research

conducted through the program. The objectives of the SERF program are as follows:

methodically and thoroughly assess and review the education literature, formulate

original and significant research questions, design and educational research project using

appropriate methodologies, manage a surgical education project, and organize

presentation of completed educational research projects for peer-reviewed academic

publications and/or conferences.
