The pathway model within i-focus consists of a simple series of stages. Each stage has attributes such as due date, PROMs to be filled out, and Patient Information resources such as PDFs and videos.For example, this shows the PROMs and information associated with the “Initial Assessment” stage within a simple “Orthopaedic Knee pathway”

The patient – Huw - receives an invite email and/or SMS to join the “My Pathways” service. They can choose to log in via a web interface, or download the “My Pathways” app.

Once logged in, Huw receives an interactive timeline graphic representing the stages of the pathway(s) he is on – in this case the Knee pathway.

He can see appointments, and the information (PROMS) he is being requested to give at different different stages, and what is due now.

He can also see that there are associated videos, leaflets, etc that it is suggested he looks at.

The patient can access either as a web application, or via a mobile app. The data is securely stored in their PHR and is tied to their NHS number.

On joining the persistent pain pathway, the patient is requested to fill out relevant PROM and HNA questionnaires. These can be saved and sent on to the clinician portal and if required on to the GP system.

A body chart is provided to allow the patient to indicate areas of pain.

A PROM measure similar to EQ5D called the “Dolo Test” enables the patient to report the effect that their condition is having on their life, including on sleep, work, social activity, etc. An engaging “rotating wheel” graphical representation is provided to facilitate entry.

Results are sent through to the secure PHR system. The patient can view previous scores and progress over time, to help build understanding and ability to manage more successfully.

Once all the PROMS and appointments within a pathway stage are complete, Huw will be moved automatically to the next stage.

Updates on appointment status can be parsed from incoming HL7 v2 messages.

The administrator can also move Huw manually between stages if desired.

• Annualised saving in total cost of pathway of c£280,000 representing a 9% reduction in total pathway spend of £2.9million

• Switch from 58% of 1st OPs being seen in acute trusts, to 29%, representing substantial transfer of care to community sector. This trend is expected to continue

• Reduction in hospital in patient and day case procedures of 6% with an associated cost reduction of 15%, indicating fewer and lower complexity medical pain interventions

The clinician may wish to request Huw fills out additional PROMs .. For example an additional Oxford Knee Score PROM. This can be requested with a simple click

The clinician may also want to suggest Huw views some particular leaflet or video .. These can be pre-programmed into the pathway or selected manually just for Huw.

For example, this short video on the knee might be helpful to Huw in advance of an operation.
