IRICENstandsforIndianRailwayInstituteOfCivilEngineering.ItisacentralisedtraininginstituteforIRSEprobationers.ItislocatedinPunecityintheposhlocalityofKoregaonPark.Itwasstartedin1959asPermanentWayTrainingSchoolforCivilEngineers.AfterclearingIES,civilengineersthosewhogetrailwaysstarttheirjourneyfromhereonly.Thisinstituteisourheadquarterfornextthreeyearsofprobation.



IRICENBuilding

OurbatchofIRSEprobationerswereaskedtoreporttheIRICENon15December2014.Accordingly,wemadeourreservationsforPune.AsImentionedpreviouslythatweweregivenfirstclasspasstotraveltoPuneforjoiningsowedidntfaceanymajordifficulty.OncewereachedPunerailwaystationtherewasreceptiondeskjustoutsidethestationforwelcomingus.StafffromIRICENhadarrangedvehiclestotransportustoIRICENhostelwhichisjustadjoiningtomainIRICENbuilding.Onceinside,wewereallottedourhostelrooms.Whenistartedmovingtomyroom,oneboytookmyluggageandleadmetowardstheroombuttheniinsistedthatiwillcarrytheluggageonmyown.ActuallyhesymbolizedthefailureofIndianStateintermsofemploymentandeducation.Heisinnowayinferiororlesserbeingthaniambutoursocietyislikesuchwherethereislesstoenvyandmoretopity.

IRICENhostelisthebestamongallcentralizedtraininginstituteshostelsofIndianRailway.WhenIopenedmyroomitwasequippedwithallthefacilities.Everythingwasinitsplacejustwaitingforus.Internetspeedisjustamazing,ithasmadeeveryonecrazyhere.Railwayprovidesserviceswhicharegenerallynotavailableelsewhere.



Thereafterwardsweproceededforthedinner.Letmetellyouthatmessarrangementsarealsonoordinaryhere.Ourdiningareaisbighallwherearound150peoplecaneatattime.Everythingwasonitsplace.Wejustsatthereandenjoyedourmeal.Therearecateringstaffwhichtakecareofyouralltheneeds.Foodisofverygoodquality.Youwillfallinlovewithdiningfacilitieshere.Itiscompletelydifferentfromwhatyouwillfindinyourcollegemess.Soifyouwanttobeherethanstartstudyingnow.

IthasjustbeenoneweekatIRICEN.HalfofthebatchhasbeensenttoNAIRVadodraandrestofthemhavebeensenttofieldtraining.Iwasoverwhelmedbytheexperienceherewhichhasbeenfulfillingtillnow,wehavetoreturnagainherefornextphaseoftraining.ThisistheplacewherewewillbeincubatedasofficerofIndianRailways.

REGULATIONSRELATINGTOTHEPHYSICALEXAMINATIONOFCANDIDATES

1.Theseregulationsarepublishedfortheconvenienceofcandidatesandinordertoenablethemtoascertaintheprobabilityoftheircominguptotherequiredphysicalstandard,theregulationsarealsointendedtoprovideguidelinestothemedicalexaminersandacandidatewhodoesnotsatisfytheminimumrequirementsprescribedintheregulationscannotbedeclaredFitbythemedicalexaminers.However,whileholdingthatacandidateisnotfitaccordingtothenormslaiddownintheseregulations,itwouldbepermissibleforaMedicalBoardtorecommendtotheGovernmentofIndiaforreasonsspecificallyrecordedinwritingthathemaybeadmittedtoservicewithoutdisadvantagetoGovernment.

Note:TheMedicalBoardwhileconductingmedicalexaminationofthecandidateswhohaveappliedagainstthepostsreservedforphysicallyhandicappedcategorywillkeepinviewtherelevantprovisionsofthePersonswithDisabilities(EqualOpportunity,ProtectionofRightandFullParticipation)Act,1995whereintheextentofpermissibledisabilityhasbeendefined.

3.(a)Inthematterofthecorrelationofage,heightandchestgirthofcandidatesofIndian(includingAngloIndian)race,itislefttotheMedicalBoardtousewhethercorrelationfiguresareconsideredmostsuitableasaguideintheexaminationofthecandidates,iftherebeanydisproportionwithregardtoheight,weightandchestgirth,thecandidatesshouldbeHospitalisedforinvestigationandXrayofthechesttakenbeforethecandidateisdeclaredfitornotbytheBoard.

(b)However,forcertainServicestheMinimumstandardsforheightandchestgirth,withoutwhichcandidatecannotbeacceptedareasfollows:

TheminimumheightprescribedisrelaxableincaseofcandidatebelongingtoScheduledTribesandtoracessuchasGorkhas,Garhwalis,Assamese,NagalandTribes,etc.whoseaverageheightisdistinctlylower.

(c)FortheMilitaryEngineeringServiceandIndianOrdnanceFactoriesServiceGroupA,minimumexpansionof5centimetreswillberequiredinthematterofmeasurementofthechest.

4.Thecandidatesheightwillbemeasuredasfollows:Hewillremovehisshoesandbeplacedagainstthestandardwithhisfeettogetherandtheweightthrownontheheelsandnotonthetoesorothersidesofthefeet,hewillstanderectwithoutrigidlyandwiththeheels,caves,buttocksandshoulderstouchingthestandard,thechinwillberepressedtobringthevertexoftheheadlevelunderthehorizontalbarandtheheightwillberecordedincentimetresandpartsofacentimetrestohalves.

5.Thecandidateschestwillbemeasuredasfollows:Hewillbemadetostanderectwithhisfeettogetherandtoraisehisarmsoverhishead.Thetapewillbesoadjustedroundthechestthatitsupperedgetouchestheinterioranglesoftheshouldersbladesbehindandliesinthesamehorizontalplanewhenthetapeistakenroundthechest.Thearmswillthenbeloweredtohanglooslybythesideandcarewillbetakenthattheshouldersarenotthrownupwardsorbackwardssoastodisplacethetape.Thecandidatewillthenbedirectedtotakeadeepinspirationseveraltimesandthemaximumexpansionofthechestwillbecarefullynotedandtheminimumandmaximumwillthenberecordedincentimetres,8389,8693.5etc.Inrecordingthemeasurementsfractionoflessthanhalfacentimetreshouldnotbenoted.

N.B.Theheightandchestofthecandidateshouldbemeasuredtwicebeforecomingtoafinaldecision.



6.ThecandidatewillalsobeweightedandhisweightrecordedinkilogramsfractionofaKilogramshouldnotbenoted.

7.Thecandidateseyesightwillbetestedinaccordancewiththefollowingrules.Theresultofeachtestwillberecorded:

(i)General.Thecandidateseyeswillbesubjectedtoageneralexaminationdirectedtothedetectionofanydiseaseorabnormality.Thecandidatewillberejectedifhesuffersfromanymorbidconditionsofeye,eyelidsorcontiguousstructureofsuchasortastorenderorarelikelyatfuturedatetorenderhimunfitforservice.(ii)VisualAcuity.Theexaminationfordeterminingtheacutenessofvisionincludestwotestsonefordistanttheotherfornearvision.Eacheyewillbeexaminedseparately.Thereshallbenolimitformaximumnakedeyevisionbutthenakedeyevisionofthecandidatesshallhowever,berecordedbytheMedicalBoardorothermedicalauthorityineverycaseasitwillfurnishthebasicinformationinregardtotheconditionsoftheeye.

Thestandardsfordistantandnearvisionwithorwithoutglassesshallbeasfollows:

Satisfactorycolourvisionconstitutes,recognitionwithcaseandwithouthesitationofRed,GreenandYellowcolours.TheuseofIshiharasplates,showningoodlightandasuitablelanternlikeEdrigeGreensshallbeconsideredquitedependableforrestingcolourvision.BoththeIshiharasplatesandEdrigeGreenslanternshallbeusedfortestingcolourvisionofcandidatesforappointmenttothetechnicalservices.

NOTE(3):FieldofvisionThefieldofvisionshallbetestedinrespectofallservicesbytheconfrontationmethod.Wheresuchtestgivesunsatisfactoryordoubtfulresultsthefieldofvisionshouldbedeterminedontheperimeter.

NOTE(4):ForNightBlindnessNightblindnessneednotbetestedasaroutinebutonlyinspecialcases.Nostandardtestforthetestingofnightblindnessordarkadaptionisprescribed.TheMedicalBoardshouldbegiventhediscretiontoimprovisesuchroughteste.g.recordingofvisualacuitywithreducedilluminationorbymakingthecandidaterecognisevariousobjectsinadarkenedroomafterhehasbeentherefor20to30minutes.



Candidatesownstatementsshouldnotalwaysberelieduponbuttheyshouldbegivendueconsideration.

NOTE(6):Ocularconditions,otherthanvisualacuity:(a)Anyorganicdiseaseoraprogressiverefractiveerror,whichislikelytoresultinloweringthevisualacuity,shouldbeconsideredasadisqualification.(b)Squint:Fortechnicalservicespresenceofbinocularvisionisessential.ThepresenceofSquint,evenifthevisionacuityineacheyeisoftheprescribedstandardshouldbeconsideredadisqualificationintheabsenceofbinocularity.FornontechnicalservicethepresenceofSquintshouldnotbeconsideredasadisqualificationifthevisualacuityisoftheprescribedstandards.

(c)Ifapersonhasoneeyeorifhehasoneeyewhichhasnormalvisionandtheothereyeisambylyopicorhassubnormalvision,theusualeffectisthatthepersonlacksstereoscopicvisionforperceptionofdepth.Suchvisionisnotnecessaryformanynontechnicalposts.Themedicalboardmayrecommendasfitfornontechnicalservice.Suchpersonsprovidedthenormaleyehas(i)6/6distantvisionandJ/Inearvisionwithorwithoutglasses,providedtheerrorinanymeridianisnotmorethan4dioptresfordistantvision.(ii)hasfullfieldofvision.(iii)normalcolourvisionwhereverrequired:Providedtheboardissatisfiedthatthecandidatecanperformallthefunctionsfortheparticularjobinquestion.TheaboverelaxedstandardofvisualacuitywillnotapplytocandidatesforPosts/ServicesclassifiedasTECHNICAL.NOTE(7):ContactlensesDuringthemedicalexaminationofacandidate,theuseofcontactlensesisnottobeallowed.NOTE(8):Itisnecessarythatwhenconsideringeyetesttheilluminationofthetypelettersfordistantvisionshouldhaveanilluminationof15footcandles.NOTE(9):ItshallbeopentoGovernmenttorelaxanyoneoftheconditioninfavourofanycandidateforspecialreasons.

8.BloodPressure

TheBoardwilluseitsdiscretionregardingBloodPressure.Aroughmethodofcalculatingnormal,maximum,systolicpressureisasfollows:(i)Withyoungsubjects1525yearsofagetheaverageisabout100plusage.(ii)Withsubjectover25yearsofagegeneralruleof110plushalftheageseemsquitesatisfactory.



N.B.1.Asageneralruleanysystolicprescription140mmanddiastolicover90mmshouldberegardedassuspiciousandthecandidateshouldbehospitalisedbytheBoardbeforegivingtheirfinalopinionregardingthecandidatesfitnessorotherwise.Thehospitalisationreportshouldindicatewhethertheriseinbloodpressureisofatransientnatureduetoexcitementetc.orwhetheritisduetoanyorganicdisease.InallsuchcasesXrayandelectrocardiographicexaminationsofheartandbloodureaclearancetestshouldalsobedoneasroutine.Thefinaldecisionastothefitnessorotherwiseofacandidatewill,however,restwiththeMedicalBoardonly.

2.MethodoftakingBloodPressure(II)

Themercurymanometertypeofinstrumentshouldbeusedasarule.Themeasurementshouldnotbetakenwithinfifteenminutesofanyexerciseofexcitement.Providedthepatientandparticularlyhisarmisrelaxed,hemaybeeitherlyingorsitting.Thearmissupportedcomfortablyatthepatientssideinamoreorlesshorizontalposition.Thearmshouldbefreefromclothestotheshoulder.Thecuffcompletelydeflated,shouldbeappliedwiththemiddleoftherubberovertheinnersideofthearmanditsloweredgeaninchortwoabovethebendoftheelbow.Thefollowingturnsofclothbandageshouldspreadevenlyoverthebagtoavoidbulgingduringinflation.Thebrachialarteryislocatedbypalpitationatthebendoftheelbowandthestethoscopeisthenappliedlightlyandcentrallyoveritbelow,butnotincontactwiththecuff.Thecuffisinflatedtoabove200mm.Hg.andthenslowlydeflated.ThelevelatwhichthecolumnstandwhensoftsuccessivesoundsareheardrepresentstheSystolicPressure.Whenmoreairisallowedtoescapethesoundswillbeheardtoincreaseinintensity.Thelevelatwhichthecolumnwellheardclearsoundschangetosoftmuffledfadingsoundsrepresentsthediastolicpressure.Themeasurementsshouldbetakeninafairlybriefperiodoftimeasprolongedpressureofthecuffisirritatingtothepatientandwillvitiatethereadings.Recheckingifnecessary,shouldbedoneonlyafewminutesaftercompletedeflationorthecuff.(Sometimesasthecuffisdeflatedsoundsareheardatacertainleveltheymaydisappearaspressure,fallsandreappearatastilllowerlevel.ThisSilentGapmaycauseerrorinreading.)

9.Theurine(passedinthepresenceoftheexaminer)shouldbeexaminedandtheresultsrecorded.WhenaMedicalBoardfindssugarpresentinacandidatesurinebytheusualchemicalteststheBoardwillproceedwiththeexaminationwithallitsotheraspectsandwillalsospeciallynoteanysignsorsymptomssuggestiveofdiabetes.IfexceptfortheGlycosuriatheBoardfindsthecandidatesconformstothestandardsofmedicalfitnessrequiredtheymaypassthecandidatefitsubjecttotheGlycosuriabeingnondiabeticandtheBoardwillreferthecasetoaspecifiedspecialistinMedicinewhohashospitalandlaboratoryfacilitiesathisdisposal.TheMedicalspecialistwillcarryoutwhateverexaminations,clinicalandlaboratoryheconsidersnecessaryincludingastandardbloodsugartolerancetestandwillsubmithisopiniontotheMedicalBoarduponwhichtheMedicalBoardwilluseitsfinalopinionfitorunfit.ThecandidateswillnotberequiredtoappearinpersonbeforetheBoardonthesecondoccasion.Toexcludetheeffectsofmedicationitmaybenecessarytoretaincandidateforseveraldaysinhospitalunderstrictsupervision.

10.Awomencandidatewhohasaresultoftestisfoundtobepregnantof12weeksstandingor



10.Awomencandidatewhohasaresultoftestisfoundtobepregnantof12weeksstandingorovershouldbedeclaredtemporaryunfituntiltheconfinementisover.Sheshouldbereexaminedforafitnesscertificatesixweeksafterthedateofconfinementsubjecttotheproductionofamedicalcertificateoffitnessfromaregisteredmedicalpractitioner.

11.Thefollowingadditionalpointsshouldbeobserved:(a)thatthecandidateshearingineachearisgoodandthatthereisnosignofdiseaseoftheear.Incaseitisdefectivethecandidateshouldbegotexaminedbytheearspecialistprovidedthatifthedefectinhearingisremediablebyoperationorbyuseofahearingaidacandidatecannotbedeclaredunfitontheaccountprovidedhe/shehasnoprogressivediseaseintheear.ThisprovisionisnotapplicableinthecaseofRailwayServices,otherthanIndianRailwayStoresServices,theMilitaryEngineerServices,CentralEngineeringServiceGroupA,CentralEngineeringService(Roads),CentralElectricalEngineeringServiceGroupAandBorderRoadsEngineeringServiceGroupA.Thefollowingaretheguidelinesforthemedicalexaminingauthorityinthisregard:

12.Radiographicexaminationofthechestfordetectinganyabnormalityoftheheartandlungs,whichmaynotbeapparentbyordinaryphysicalexaminationwillberestrictedtoonlysuchcandidateswhoaredeclaredfinallysuccessfulattheconcernedEngineeringServicesExamination.ThedecisionortheChairmanoftheCentralStandingMedicalBoard(conductingthemedicalexaminationoftheconcernedcandidate)aboutthefitnessofthecandidateshallbefinal.

13.IncaseofdoubtregardinghealthofacandidatetheChairmanoftheMedicalBoardmayconsultasuitableHospitalSpecialisttodecidetheissueoffitnessorunfitnessofthecandidateforGovernmentServicee.g.ifacandidateissuspectedtobesufferingfromanymentaldefectoraberration;theChairmanoftheBoardmayconsultaHospitalPsychiatrist/Psychologistetc.WhenanydefectisfounditmustbenotedintheCertificateandthemedicalexaminershouldstatehisopinionwhetherornot,itislikelytointerferewiththeefficientperformanceofthedutieswhichwillberequiredofthecandidate.

14.TheCandidateswhodesiretofileanappealagainstthedecisionoftheMedicalBoardarerequiredtodepositanappealfeeofRs.100insuchamannerasmaybeprescribedbytheGovernmentofIndia,MinistryofRailways(RailwayBoard)inthisbehalf.ThisfeewillberefundableonlytothosecandidateswhoaredeclaredfitbytheAppellateMedicalBoardwhereasinthecaseofothersitwillbeforfeited.Alongwith,appealthecandidatesmust,submitamedicalcertificatebyaregistereddoctorspecificallymentioningthatheisawareofthecandidatehavingbeendeclaredunfitbyaMedicalBoard.ThemedicalfitnesscertificatesubmittedbyacandidateisonlyaprerequisiteforappealingagainstthefindingsoftheFirstMedicalBoard.Candidatesmusthaveacopyofthiscertificatewhen,theypresentthemselvesbeforetheMedicalBoard.Theappealsshouldbesubmittedwithin21daysofthedaysofcommunicationinwhichthedecisionofthefirstMedicalBoardisconveyedtothe
