View Current and Future Benefits

“View Current and Future Benefits” to determine if changes are needed for existing benefits and if dependents need to be added or removed.

• Click “Done” when finished to return to the “Benefits homepage”.

Click “Change Benefit Elections” to begin the enrollment process

Enrollment Process: Add or Change Benefits

IMPORTANT –If you have not set up your contacts (dependents and/or beneficiaries) you MUST “Create Contact” before proceeding to “Enrollment Options”. Otherwise you may click “Continue”.

Adding Contacts

Authorization Page

IMPORTANT – To continue with your enrollment, you MUST review Annual Notices and Authorization then click “Accept”.

IMPORTANT- To validate your wellness discount / participation you must “CHECK ” the appropriate box: • Wellness Screening? (Check “Yes” or “No”) • Are you a Tobacco User? (Check “Yes” or No”) • If waiving medical, please check off the “Waive Coverage” box

Medical: Wellness Discount

IMPORTANT- To enroll or change your medical insurance coverage, select Medical and “CHECK” the appropriate boxes. • To enroll dependent(s), make your selections in the check box. • If waiving medical, click the “Waive Coverage” check box. • Click “Next” to continue.

Medical Election Page

Select Dental/Vision, to enroll or change your dental and vision insurance coverage and “CHECK” the appropriate boxes. • To enroll dependent(s), make your selections in the check box.

• If waiving dental, click the “Waive Coverage” check box.

• Click “Next” to continue.

Dental/Vision Coverage and Elections

Select Flexible Spending, to enroll in the Healthcare Flexible Spending Account, Health Savings Account, or Dependent Care Flexible Spending Account and enter coverage amount

• If waiving Flexible Spending, click the “Waive Coverage” check box.

• Click “Next” to continue.

Flexible Spending Elections

• Click “Basic Life Insurance and AD&D” option • Scroll to “Beneficiaries” listed at the bottom of enrollment screen • Select “Primary Beneficiaries %” • Select “Contingent Beneficiaries %” • Click “Next” to continue

Life Disability Coverage / Elections

IMPORTANT- Company-paid benefits do not require re-enrollment. A beneficiary designation is required. A total beneficiary % must equal 100%.

IMPORTANT- In this screen you have the option to choose Voluntary Life Insurance for yourself, spouse, and/or child: • Click on the plan(s) and amount(s) that best suits your needs. • For each plan selected, set up your “Primary Beneficiaries” by inputting a % to that selected contact. • For each plan selected, set up your “Contingent Beneficiaries” by inputting a % to that selected

contact. • If Waiving coverage, please check off “Waive Coverage” for each election. • Click “Next” to continue.

Voluntary Life Coverage / Elections

This screen will prompt you to review the following: – Plans – Dependent(s) – Beneficiaries – Authorization

• Click “Back” to make changes or corrections. • Click “Submit” to finalize your benefit elections.

Review Elections and Submit

Congratulations! You are now finished making your benefit elections.

You can:

• Print a “Confirmation Statement”

• Review any “Pending Actions”

• Click “Done” when completed.

Confirmation and Review Pending Actions

IMPORTANT- PENDING ACTION ITEMS REQUIRE SUPPORTING DOCUMENTATION FOR COVERED DEPENDENT(S). (SEE BELOW LIST) Email the following to [email protected] within 45 days from the enrollment effective date (i.e.- Jan. 1, 2017). If not received, dependent(s) will be removed from plan.

If Adding Spouse/Domestic Partner:

• Marriage Certificate

• State Domestic Partnership Certificate

• Affidavit

– Must also submit a bill or account where both parties are showing. (ie, utility bill, bank account, etc.)

If adding Child, Step Child, DP Children:

• Birth Certificate

• Adoption Documents

• Custody Court Documents
