• Create increased patient commitment– New information and communication channels– Individualized patient information – Inviting new players in the market– Encouraging co-production– Encourage new services, notably health status and nutrition

• Increase the patient’s empowerment– Improved transparency in health care processes– Let patients administer, own and manage their own processes

• Increase the safety of health care– Reduces the risk of mistakes

• Focus on four health care processes, relating to:– Stroke – Stockholm County Council

– Lung cancer – Stockholm County Council

– Hip surgery – Skåne Regional Council

– Heart disease – Region Västra Götaland

• Develop methods and guidelines– Describe the development of other health care processes and ancillary

services

– Co-production with trade associations representing health care, industry and the patients. Make it possible to solve problems together

• Stimulation for the development of new products– Enablement of a new service sector, both inside and outside the county

council
