Mukwonago Area School District Online Registration Step- by-Step Directions for Existing Families

Parent/Guardians of Returning Students (Current 4K-12):

The Mukwonago Area School District’s 2018-19 Online Registration for returning students begins June 18th, 2018. ALL families of returning students are required to enroll/register online. Access to online registration is through the Infinite Campus Parent Portal. Please contact Sue Brezovar [email protected] if you have any questions regarding the process of Online Registration.

3. The following screen will appear for all students currently enrolled in a Mukwonago Area School NOTE: If you have a new incoming student that is not currently listed or a child not

previously in our schools you will add this student later in the process.

4. The next screen will show the currently enrolled student(s) and their grade level for the 18-19 School Year

6. The “Authentication” page will appear. Please type your full name in to verify you are the authorized person for the user account and sign on the line provided.

7. The Welcome to the Mukwonago School District Online Registration page will appear. Please review all documents that may be needed. Most information will auto populate from the data base system. Press Begin

8. Information will be uploaded into the online registration application based on the information listed in our student information system for your household and student(s). THIS MAY TAKE A MINUTE OR TWO depending on the size of your household. Check the spelling, capitalization and punctuation because the data is imported into our system as entered.

9. Please read the directions in each topic area. Review the uploaded information and make any necessary changes. Please make sure to

update your student’s emergency contact(s) in case of an emergency and we can’t get in touch with the parents.

Once you have logged in you will navigate through each index tab to enter primary

household, parent/guardian, emergency contacts, other household members and student information.

When entering information into the application • Please DO NOT use all capital letters • Read the directions listed in each area • Look for the required information – a RED asterick * indicates required

information that may be missing.

The student’s PRIMARY household information should be entered into this index area. In the case where there is a split household, the household considered as the SECONDARY household will be entered into the Parent/Guardian area.

Student(s) Primary Household Phone

1. Review/Enter 10-digit primary home phone number. If you don’t have a land line, enter the primary household cell phone number to be used ~~Voice contact preferences are automatically selected, You will need to enter your text(SMS) preferences if these are not selected, you will not be notified by text. These must be set by the parent, the district doesn’t preset texting options

Primary Household Home Address

1. Your address as listed in the portal will appear first.

2. If you check the box The home address is no longer current: • Please enter: Date the address became inactive • Please enter: New address

3. Select Next to proceed to Declaration of Residency (not commonly used) if applicable

If you do not own or lease or lease the residence, please complete the Declaration of Residency and upload or bring to the district office. Press Next to move to the Free Reduced Application and Waiver pleat.

Free Reduced Application and Waiver (New application available after 7/1) 1. The district is required to waive certain fees for students considered “indigent” under state

law. If your family qualifies for free or reduced price meals, you may give the district permission to use that information for a fee waiver.

2. Click Yes or No and also whether or not the District has permission to share the information provided in the fee waiver section for the purpose of waiving or reducing other student/family fees.

4. Press Save/Continue

Parent/Guardian Demographics Parents, guardians, step-parents and foster-parents of the student should be entered in this area. If there is a split household, the “Primary Household” parent should enter the information of the other parent including their phone numbers and address. (This was determined in the Student(s) Primary Household Tab)

All parent/guardians in the household will appear on this screen

Please review information for all parent/guardians listed to make sure that we have the most up-to-date information, ie. Cell phone numbers, email, etc.

The parent/guardian is highlighted in yellow, it is required to review all information in order to proceed. Once the information is review, the parent will no longer be highlighted and there will be a green check mark in the completed field.

Review the following information for each parent, guardian, step-parent or foster-parent:

First Name, Middle Name, Last Name, Birth Date, and Gender.

1. Review if the person still lives at the same address as the student, check the box.

2. If the parent lives at a different address please enter the new information or uncheck the

box and enter the correct address for this parent or if you do not know the parent/guardians address, please check the box “I will not provide an address for this parent.

3. Click Next to move to PRIMARY Parent/Guardian Contact Information pleat.

Parent/Guardian Contact Information 1. Review/Enter the following information for each parent, guardian, step-parent or foster-parent:

Cell Phone, Work Phone and Email (If parent has no email, mark the “Has no Email” checkbox.) Indicate your Message Contact Preferences.

Please note that the Text messaging may not be checked – we can’t check this automatically because some plans charge for texting – this has to be determined at this point if you want to receive text messages from the district wide communication system. (Infinite Campus)

2. Click on the Save/Continue to move to the Military Connections pleat (now required by DPI)

2. IF you answer ‘YES’ to any of the 3 questions, you will be prompted with additional questions.

3. Please complete and then press Save/Continue to proceed to the Emergency Contact pleat

Emergency Contact Demographics Review all persons listed in the emergency contact area. (YOU MUST HAVE AT LEAST 1 EMERGENCY CONTACT THAT IS NOT A PARENT/GUARDIAN) Emergency contacts are listed in this area. Former students may appear here as well Enter individuals who are not parents and can act as an emergency contact when a

parent cannot be reached. The emergency contacts entered in this area are authorized by you for the Mukwonago Area School District to release your student to following an emergency requiring the dismissal of school or in the event of illness. All Emergency Contacts will be asked to show a valid form of picture identification and provide a signature before the Mukwonago Area School District will release your child (student) to them.

If the person should be removed as an Emergency Contact, please check the box and press Confirm to remove this Emergency Contact

2. Verify/Enter the information for the emergency contact: Legal First Name, Legal Last Name, Gender

3. Press Next to go to the Contact Information pleat for each Emergency Contact You must enter at least one (1) phone # for each Emergency Contact you list. If the emergency contact lives in the same household check “Please check this box if this person lives at the address below” if not, please enter the Complete Address (if known) or the Street and City the emergency contact lives in. • We will use this information to see if the person you are listing is currently in our database

to avoid duplications. Repeat this for all Emergency Contacts you would like to add. • Press Next to go to the Verification pleat and enter required information if necessary

4. When done, click Save/Continue to move on to the Other Household Members pleat.

Other Household Members Other household members should include children who are living in the same household as the student you are verifying the information for and should not be or are too young to currently be enrolled or are attending a private school or a school in another district (OE). Do not enter any students that you will be enrolling for the upcoming school year. These children will be added to the student tab. If you are not adding any Other Household members, you may click on the Save/Continue button and then click on the Student index tab.

1. Click the Add New Household Member button to begin.

2. Enter the following information for children living in the household who are not already enrolled: First Name, Middle Name, Last Name, Birth Date and Gender. Please include the birth date as this information is used census purposes for the Mukwonago Area School District

3. Click Add New Household Member or Save/Continue to move to the Student index tab after you have entered all of the information.

Review existing student information for EACH student. Any fields with a red *asterick or flag are required fields and must be completed before proceeding to the next pleat.

1. Is the student Hispanic or Latino?* YES/NO

2. Mark all of the following that apply to the student’s race ethnicity. If you marked Yes to Hispanic/Latino, you MUST also choose one of the following:

Student Housing This is information about the student(s)/family home situation.

1. Mark either Yes/ No for the student’s homeless status. If you selected yes, check the options that best represent the student’s current housing situation.

2. Click Next to move to Home Language

Student Home Language Survey This information is gathered for English Language Learners (ELL) and/or Bilingual/English as a Second Language (ESL) services for pupils and families that communicate in a language other than English. Use the dropdown list to select the language for the following:

1. Student Language?* 2. Parent/Guardian Language? 3. What was the first language spoken by the student?* 4. What is the language most spoken at home? 5. What is the language most spoken by the student with friends? 6. Has your child ever received and/or been tested for English Language Learners (ELL)

Intra District Transfer (Grades 5K-6 Only) This is for parents that wish to move their child from their current school to another school in the Mukwonago Area School district boundary area. Please read the information carefully.

1. Please select the school that you would like to transfer your student to from the dropdown of

school choices.

2. Complete the Intra District Transfer Form and upload or bring to the Mukwonago Area Schools district office.

Student Relationships – Parent and/or Guardian 1. Select the Relationship* between the parent/guardian and the student from the dropdown

2. Select the Contact Preferences for each of the parent(s)/guardian(s) listed. A description of each type is provided.

3. Select a Contact Sequence* number. (1&2-Father/Mother, 3&4-Step/Foster Parents)

2. Select a Contact Sequence number. (Usually 3 or above). This is the order that the schools will call if they are not able to contact the parents

3. Press Next to proceed to the Elementary Transportation pleat (Grades 5K-6 only) otherwise proceed to the Relationships – Other Household pleat

Please verify/enter the correct relationship for each student to the listed individuals, typically this list includes other siblings/children in the household. Press Next to proceed to the Elementary Transportation – 5K through 6th Grade Only Tab or Health Services – Medical and/or Health Conditions & Immunizations for students in 7th-12th

This pleat is to help with any special busing situations that your student may have. Please complete all information as accurately as possible at this time. We understand that sometimes situations exist and changes may need to be made. Once the application is processed, please call with any changes to your child(s) transportation needs.

3. Repeat for all conditions If a condition is no longer current–, please remove the condition

5. School year for Update (This replaces the Annual Health Update Forms) Please list any health condition/concerns noted above (if any)

Student Health Services – Medications

1. Check the box if this student has no medications.

2. Click the Add Medication button if the student is currently taking a medication.

3. Enter: Name of Medication, Where is it Taken, Medication Frequency (Daily, Emergency, As needed), Comments and Instructions for the medication. If the medication is no longer taken or has changed, please click on “Remove Medication.” For each medication that will be taken/administered at school. Complete the

Authorization to Administer Prescription Medication form for each prescription and this form must be signed by the Parent and Doctor

MHS Students Only: Parking Pass Information (For those students purchasing a Parking Pass) Only Grades 10-12 This pleat is only for those students wishing to purchase a parking pass for the upcoming school year. There is a mandatory video and quiz that must be taken before pass will be issued)

Please fill in all vehicle information, download the form and bring to the Associate Principal’s office with proof of payment and/or payment to receive parking pass.

When all students have been reviewed, press Add New Student if you need to add another student or press Save/Continue to proceed

FREQUENTLY ASKED QUESTIONS

Q: I forgot my Infinite Campus username/password. What do I do? A: You can recover your username/password on the login screen.

Q: I have multiple children at different schools. Will I have to log-in separately for each child? A: No. You should be able to see all of your children in one location. If you don’t see all of your children, please contact Sue Brezovar @ [email protected].

Q: I did not get the link. If my child has parents that live at two different addresses, who will be able to register the child? A: The link to register your child will be available to all parents who have custody rights to the child. Contact Sue Brezovar @ [email protected] with further questions.

Q: Will I be able to register my child for classes in high school and middle school? A: No. Class registration is separate from online form completion. Online form completion will update your address, emergency contact information, etc.

Q: If I start the application and then get interrupted, will I be able to stop and continue later? A: Yes. You will be able to save your incomplete registration and return to it at a later date using the link in the Infinite Campus web portal.

Q: If I don’t have computer access at home, how can I register my child? A: You can use any computer with Internet access. The district office will have chromebooks available to assist parents who need Internet access. Public computers are also available for parents to use at the Mukwonago Public Library.

Q: Has form and fee day been eliminated? A: No, although online form completion is in place, each school will still host a day for school photos, fee payments, online form completion assistance, etc.
