Accessing Your Educator Evalua�on Forms

1. On the le� side, choose

My Evalua�ons

2. Choose Evalua�on

3. Choose an Educator

Evalua$on component by

clicking on the + sign to the

le� of the component $tle

4. The component will open and

status informa$on will appear

5. Click on the “Document”

icon and the selected

form will open

You can minimize/maximize each sec�on

by clicking on the arrow in the upper right

corner

3. A�er an evaluator scores your ar$facts/

evidence, the box will be colored and re-

flect your score.

4. Evaluators may add a ra$onale, jus$fying

the score

5. You can upload ar$facts here for each

individual PF component (rather than the

Ar$fact Files)

6. If you upload items into Ar�fact File and

note that they align with Prof Founda$ons,

the ar$facts will show up here

8. Once you have submi<ed your ar$facts

and have received scores, you are asked to

Acknowledge that you received the scores

— this does not mean you agree or disa-

gree, simply that you received the form

back with scores from your evaluator.

7. When you have everything uploaded,

you should click SAVE or SAVE and NOTIFY

to let your evaluator know you are ready

for them to score. Then click SUBMIT to

send the completed form to the evaluator.
