paid thru my employer. This plan was established to provide members with vacation savings benefits. This

plan covers all employees of an employer who contributes to the Trust under the terms of a collective bar-

gaining agreement, provided contributions are being made to the Trust on the employee’s behalf. You may

withdrawal monies from your account twice (2x) per year; June 1st and December 1st based on your account

balance as of April 20th for June payment and October 20th for December payment. YOU MUST FILL OUT A
