Monarch Pioneer ACO SNF 3-Day Stay Waiver Participants

Under current Medicare law, Medicare only covers care in a Skilled Nursing Facility (SNF) if a patient has a prior three-day inpatient hospital stay. This feature may allow you to get Medicare covered SNF services at a participating SNF without a mandatory three-day inpatient hospital stay. As of August 1, 2015, Monarch has partnered with the following nursing facilities throughout Orange County.

Note: Monarch ACO does not authorize or restrict the care of a patient who is aligned to the ACO. ACO beneficiaries retain the freedom to accept or decline services offered by Monarch HealthCare ACO, including accessing the SNF Waiver feature.
