Dear Mom,

In our first edition of Mom's Notes for 2013 we focus on beginning things the right way. Although the school term is already in full swing, it's never too late to kick-start healthy routines at home. Cora Breytenbach offers us some helpful hints on how to do just that on page 6.

The first school term is packed with sports activities. To make sure that little active bodies get all they need, our Mom's Notes dietician has summarised some important tips on what to give your child before and after a sports event (see page 8). With so much fun in the sun, the Lowveld CANSA Care Centre reminds us about the importance of sunscreen on page 12!

February is Pregnancy Awareness Month - in the spirit, Kids Emporium collects the latest in maternity and stylish child must-haves. To give your child an extra boost, Little Hoot has hand picked some great learning toys to choose from.

NuLight shares their secret on more productive and focused spaces at home and work on page 19. Spend more time with the family in the pool - rather than maintaing it this year (page 16).

Find a FREE MEAL VOUCHER as part of Arkansas and Mountain Spring Spurs' colour-in competition with a total of 4 bicycles up for grabs!

REady StEady, ROUtinE!

After a long holiday and after the excitement of a new school year has waned - it is time to establish the routines that will not only make Mom’s life easier, but will also benefit children in more ways than one.

Routine establishes so many aspects of healthy living, good habits and good behaviour. Children need and even desire routine – even if they don’t know it. there are plenty of ways to get them going by building a schedule that works for the whole family. Children don’t have a lot of control in their lives and it can give them a sense of organization, stability and comfort. that, in turn, should help develop better behaviour and some sense of personal control – always a challenge, especially with young children.

how to sEt up A routiNE:• Start to build the routine around the times when

family members wake, eat and sleep.

• Be realistic and allow enough time for your children to wash and dress, especially if this is new for them.

• add in extras to the routine which you know your family needs; chore time, one-to-one time if you have more than one child, homework time and any out of school clubs or classes.

• Pay close attention to night time: if bags are packed, sports stuff is ready, school clothes are laidoutandwaterbottlesfilledthere’llbelesstodo in the morning. School age children can help with all of these things.

• if you can, remember to rotate duties with your partner, especially the bedtime routine as this will increase your child’s trust in both parents.

• Get as much input as you can from your family and talk it through with other carers. a routine which works for everyone will be much easier to stick to.

• if there’s one activity, like toothbrushing, which is a nightly sticking point, set up a Reward Chart to deal with it.

doing things like bathing and brushing teeth as part of morning or night time routines, can help establish good personal hygiene and health habits. Having built in chore time somewhere in the week or day, having them help pick up at the end of dinner, tidy up the house at the end of the day or playing, establishes responsibility and work ethic.

MorNiNg rituAlsGetting up and going can be hard whether young or old, kid or parent, in school or not. no matter how getting started in the morning; if you have established a solid routine, you will have an easier flowtotheday.

Sticking to a similar wake-up call could mean all the difference between having a good day and having a bad one. think about it, even if it is obvious, if the kids are used to waking up at 6.30 and for some reason rise an hour earlier or later you now needtofigureouthowtoaddanhourtothedayor squeeze in a day’s worth of routine in 60 less minutes. the implications are worse if the kids are in school or there is something taking place that morning. Having a solid morning ritual, maintains a good stress level for everyone. MEAltiMEsMealtime are a perfect way to establish good eating habits. always eat breakfast, and a good one. lunch anddinneraremoreofagiven, butspecifictimesset a structure for kids so there are no surprises. Set standards for when snacking takes place and what kinds of foods are eaten. don’t indulge in food whenever a child wants something. the results should equal good eating and eating habits.

Mealtime is always a good time for family interaction!Alwaystrytofindtimetosit down together, preferably at a table, to talk and eat. additionally, have the children help get dinner started, set the table and clean up afterwards to teach the them responsibility.

BEdtiMEgetting the children to sleep, whether for a nap or for the night, is one of the greatest challenges for parents. a consistent bedtime routine is important for the establishment of a regular sleep-awake cycle. a regular schedule allows the child to be

6

Page 7: Mom's Notes Jan/Feb 2013

physio-logically ready for sleep at the same hour each evening. • Be sure to establish a regular hour for both going

to bed and for waking up. these times should be changed only for special occasions. the hour just before bed should be a quiet, relaxing time for the child. thus it is not a time for playing rough with the child or watching scary tv programs.

• a warm bath can help to prepare a child for sleep.

• Children will go to bed more readily when they receive advance notice from their parents that bedtime is approaching. Give an advance warning 5 to 10 minutes ahead of bedtime so the child can get ready for it.

• try to associate going to sleep with as much pleasure, affection and relaxation as possible. For example, you might read or tell a child a bedtime story, say an evening prayer, sing a goodnight song, or have a close intimate talk with the child after he/she is in bed. these bedtime rituals should be calm and relaxing.

• Make it clear to the child when you give the goodnight kiss that this is the end of your inter-action for the night. leave the room without hesitation or uncertainty and, if necessary, state that you are not coming back. if the child whines or cries after you leave, try to ignore it. the key is to expect your child to go to bed at the same timeeachnightandtofirmlyandconfidentlyenforce the time for bed.

• a blanket or cuddly stuffed animal in bed can give the child much security as he/she makes the transition from wakefulness to sleep.

one of the best ways to prevent sleep disorders in the very young, is to be sure the child is getting plenty of affection from both parents. do not give constant or intense criticism which will make the child feel uncertain of your love. do not send you child to bed early as a punishment. Children tend to associate going to bed with punishment. A good sleep routine, can keep your child hAppy!

For a longer game or training session, you may need to add some protein to sustain energy Choose fruit or low-fat protein options such as milk, chicken, cottage cheese or yogurt.

avoid fatty foods (these slow digestion too much) and foods with high sugar content such asfizzydrinks, sweetsandsportsdrinksbeforeexercise. these cause a spike in blood sugar. if sugar levels then drop quickly during athletic events, your child could become sluggish or even dizzy.

Meal ideas 1-2 hours before the event:• whole-grain bread, crackers, tortillas, or pretzels • wholegrain cereals• Brown Rice with chicken and veggies• Pasta with grated cheese and cooked ham• low-fat cheese with crackers, yoghurt, or milk• Snack ideas pre – event to boost energy levels:• Hydration is important- so keep the water

flowing• Bananas (cut in half for younger kids so they can

peel and eat more quickly) • Orange slices • Clementines • Grapes • Small slices or chunks of melon • apple wedges (sprinkle with orange juice to

prevent browning) • 125 – 150 ml of Clear fruit juices such as apple

School age is the perfect time for children to learn about healthy food, bodies and activity. Make this the year of moving more! Physical activity is a natural and lifelong part of healthy living. It can be as simple as riding a bike, playing tag, jumping rope or doing chores. Inactivity plays a major role in the development of obesity, whether it results from television and computer use, limited opportunities for physical activity, or safety concerns that prevent children from enjoying free play outdoors.

the amount of physical activity they have in a day will be an important part of how much they need to eat. Children need a wide variety of foods for a well-balanced diet and when children are busy and active, snacking is important to keep energy levels high.

it is important to keep young athletes strong andfitwithhealthysnacks,andwithmealpre- and post training and events that provide the right nutrients at the right time. it’s also important to limit ingredients that will impede performance.

HEaltHy SnaCKS BEFORE tRaininG

Help your child to train at his/ her best. Fuel muscles with carbohydrates one to two hours before an athletic event or practice. Grains, such as pasta or whole wheat crackers such as Provitas are your best healthy snacks for exercise less than 60 minutes.

if they’ve really been sweating and/or it is extremely hot outside, athletes also need sodium and potassium, which is why sports drinks contain these electrolytes.

Finally, carbohydrates and proteins help kids refuel muscles and re-energize. while a little sugar is OK, don’t go overboard; it’s not wise to reinforce the idea that sweets are a good way to reward yourself.

DID YOU KNOW?• Childhood cancer is not

transferable.

• 77% of childhood cancers can be treated successfully if detected early.

• Most cases of childhood cancer have genetic causes.

• other causes include man-made chemicals such as BPA in plastics.

• Children have the right to a smoke-free world. Tabacco and second- hand smoke is harmful to children.

• Use of Hubbly Bubblies (aka hookahs) are more harmful than smoking of regular cigarettes.

• Play it safe in the sun - only two blistering sunburns can result in skin cancer later in life.

• Balanced lifestyle - regular exercise and a diet consisting of as many fruit and vegetables as possible can reduce the risk of cancer.

Knowing the milestones of your baby's vision development (and what you can do to help it along) can ensure your child is seeing properly and enjoying his world to the fullest. We will continue this developmental journey to that of primary school level with Grant and Stacy Freeman. This Month we are going to look at the vision of a child from birth to 6 Months of age.

Early detection and treatment are essential to preventing eye conditions that have the potential to cause permanent loss of vision. Screening by the pediatrician or other primary care physician is important at birth and during the first 6 months of life when the visual system is highly susceptible to interference. However, screening this population has been problematic, leading to under detection of significant refractive error and strabismus (squint)

Let’s start with a few definitions:Visual acuity: the clarity or clearness of visionAccommodation: the increase in thickness and shape of the eye’s lens in order to focus an image on the retinaStereopsis: when both eyes work together to enable you to see 3 dimensional or depth perception.

There are different theories about the visual acuity of infants. Some literature says that a baby has adult like vision at 6 months and others, at the ages of 3-5 years. If I watch my little one I personally think that their vision is adult like at 6 months of age.

How do you tell if your little one has poor vision? This is a difficult one as your baby can’t tell you that they are unable to see.

The sure signs that your infant might not have good vision are:• Abnormal eye movements are often the first symptom of significant visual impairment• Intense eye rubbing• A baby that is unresponsive to the parents

NOW ALSO: Lowveld HospitalOpposite Nel-Med, Nelspruit | Tel: +27(0)13 753 2020

Accommodation:For the first month of your little ones life they are unable to focus on an object further or closer than 30cm. It is thought that the distance from your face to where you hold your baby when you breast feed is the distance that they are able to focus on in the first month. So when that little one is lying in your arms staring at your face they can see you clearly. They have adult like accommodation at 3-4 months of age which means that at this age their eyes can focus an object no matter how far or close it is from them.

Stereopsis:Sensory fusion is present at birth so from the get go your little one will not have double vision but will see singularly.

Colour Vision:The Fovea (part of the eye responsible for colour vision) is immature in infants. Newborns are unable to make colour discriminations. At about 7 weeks babies distinguish colours and at 2 months of age they have normal colour vision.

Below is a table showing at what age a baby will more than likely be able to fixate on an object and do certain eye movements etc.

SKill AGe Fixation to: Lights 3 monthsFaces BirthVisual objects 3 monthsAuditory objects 3 months

Eye Movements: Horizontal BirthVertical up gaze 4-6 weeksVertical down gaze 3 months

Stimulus (that babies react to) Faces BirthPenlight 3 monthsPursuits (ability to follow an object) 6-8 weeksCoordinated head-eye movements 3 months

And lastly, some babies appear to have a squint. Most of the time, it is because their visual system hasn't fully developed yet. In some cases however it is due to a fold of skin between the eye and the nose that covers the nasal corner of the eye making it look as if the eye is turned inward. This apparent deviation is most noticeable when baby looks sideways.

A lot of Optometrists and Parents feel that children should not wear contact lenses. However, this is incorrect. Contact lenses do have their place with children.

the main reason why contact lenses are prescribed for babies and children younger than 5 is to allow normal development of visual acuity and visual motor and perceptual ability and to overcome or decrease amblyopia (a disorder that is characterized by a vision deficiencyinaneyethatisotherwisephysicallynormal,or out of proportion to associated structural abnormalities of the eye.) Contact lenses are indicated in the following instances:1. high Myopia• Contact lenses improve the peripheral vision.• the image size is more normal.• visual perceptual and visual motor abilities im prove.• Some literature says that the contact lenses play a therapeutic role in retarding the progression of myopia. 2. Moderate to high hyperopia• Possible development of accommodative esotropia may be prevented. this is a form of strabismus, or “squint”, in which one or both eyes turns inward. the condition can be constantly present, or occur intermittently, and can give the affected individual a “cross-eyed” appearance.• For some children with high hyperopia, contact lenses are better because the strabismus that is responsible is better controlled.

3. Aphakia: which is the absence of the lens of the eye, due to surgical removal, a perforating wound or ulcer, or congenital anomaly.• Contact lenses would allow for quick changes in prescription if needed to respond to ocular growth• Theywouldalsocauselessmagnificationandamore natural visual perceptual environment.• they would also have a better cosmetic apprearance.

4. irregular astigmatism: is often caused by a corneal scar or scattering in the crystalline lens, and cannot be corrected by standard spectacle lenses, but can be corrected by contact lenses. we usually use hard contact lenses to correct this. 5. Aniseikonia: which is an ocular condition wherethereisasignificantdifferenceintheperceived size of images. a contact lens will reduce the effect of this. 6. Amblyopia: a disorder that is characterized byavisiondeficiency inaneyethat isotherwisephysically normal, or out of proportion to associated structural abnormalities of the eye. there are 2 ways to treat amblyopia with contact lenses. we either use a high plus lens placed in front of the good eye to blur the image of that eye or we use a black lens to block out the vision of the good eye. this forces the weaker eye to start working. 7. corneal masking: we use this when the eye is damaged or looks unattractive. we use a contact lens to hide the defects. 8. corneal bandage: if the eye has a sore on it, the contact lens acts like a bandage and protects it from outside germs. It is important to remember that younger children can often not communicate problems - therefore contact lenses become the responsibility of the parent or caregiver.

Children’s eyes change rapidly with growth, both in refractive error and corneal topography, therefore it is necessary for more frequent visits. But Contact lenses definitely do have their place with children.

THE TRUE STORY

Every day, children are diagnosed with life threatening illnesses. Help is often available at specialized government hospitals, but for many children and their families the treatment they so desperately need is out of reach because the cost of transport to these facilities is simply not affordable. Where the treatment required is not available locally, we arrange and sponsor flight or bus tickets to a suitable medical facility.

ABOUT WINGS & WISHESWings & Wishes was established in 2006 after a group of employees from Coca-Cola Sabco (Pty) Ltd identified the need in their community and started donating their frequent flyer miles as part of an internal Corporate Social Responsibility initiative. The miles were committed to facilitating the transport of critically ill children and an accompanying adult to the Red Cross Children’s Hospital in Cape Town, or an alternative appropriate medical facility, for treatment.

The program is now a registered project under The Coca-Cola Sabco Community Foundation (IT 60/2007) and is run primarily by employee volunteers. The program is funded entirely through donations and is supported by many individuals and organizations in South Africa. As word of the program spreads, requests for flights and bus tickets are a daily occurrence and we are now assisting at least one child a day.

20

Page 21: Mom's Notes Jan/Feb 2013

HOW CAN YOU HELP?MoneyYour donation of funds towards flight costs and essential provisions will make a signifi-cant difference in the lives of these children and their families. Funds raised are also used to provide alternative transport methods should a flight not be a suitable option.

MilesYour donation of frequent flyer air miles, in amounts of 20 000 or 30 000, will be used to purchase air tickets.

MercyWe provide basic essentials for the children whilst in hospital. These Mercy Packs contain pyjama’s, soft toys, soap, toothpaste and toothbrushes, colouring in books and pencils, and any other comfort items to make their stay in hospital more bearable. Donations to-wards the Mercy Packs are gratefully received.

Pledge a flightAs flights are requested, the bookings are made, and the cost of the flight is invoiced to the individual or company who pledged the flight. Average cost of a flight is approximately R1300.

Purpose, Vision and ValuesCORE PURPOSE:Dedicated to transporting critically ill children to receive lifesaving medical treatment.

ViSiON:Transport should never be a barrier for a child to reach lifesaving treatment, wherever they live.

CORE VALUES:Compassion, Minimal bureaucracy, integrityWe undertake to provide compassionate support immediately, with minimal bureaucracy, whilst maintaining the highest levels of integrity for the children and their families.

CRiTERiA:We assist children who do not have access to the required specialised medical treatment in their home towns and whose families would not otherwise be able to afford the transport to get them to a suitable medical facility. We also require written motivation from a medical practitioner explaining that the treatment required is not available at a local institution.

“Camp”ing up with the Jones’s

“Pappa is in beheer!”

“So James, are we braaing rump tonight? Look at my home made three-legged-braai-contraption with adjustable levels to cook the perfect piece of steak”.

Keeping up with the Jones’s op die kampeerterrein is harder werk as tent opslaan. Klop aan by Camp & Gas in nelspruit vir goeie raad!

Groete uit die nes,Robin Jansma
