1. Request Form 49

To use mobile application from mobile handset, following is the perquisite

· Android based mobile handset (Android versions should be 4 and above) with internet connectivity

You need to download the MPVAT mobile app from Google play store. Once it is downloaded and installed, please follow the below process to download form 49.

1.1. Process to request form 49

1.1.1. To login to application from mobile handset ,

use your MPCTD web portal login id and password

1.1.2. Enter User Name as your TIN

1.1.3. Enter Password

1.1.4. Click Login.

1.1.5. Click Form 49

1.1.6. Choose Consignor/Consignee of other State

1.1.7. Enter Consignee TIN

1.1.8. Enter Name of Consignee

1.1.9. Enter Address of consignee

1.1.10. Select Consignee State

1.1.11. Enter Destination from which goods

dispatched

1.1.12. Click Proceed

1.1.13. To enter invoice details, click Add Invoice Details

1.1.14. Enter Invoice No

1.1.15. Enter Invoice Date

1.1.16. Enter Destination Address

1.1.17. Select Goods Category

1.1.18. Enter Goods Description

1.1.19. Select Units

1.1.20. Enter Total quantity/weight

1.1.21. Enter Total Value

1.1.22. Click Add

1.1.23. To add new invoice detail click Add Invoice Details

1.1.24. To delete invoice detail Click Delete

1.1.25. To modify invoice detail click Modify

1.1.26. To submit details Click Proceed

1.1.27. Enter Transport Company Name

1.1.28. Enter Transport Company Address

1.1.29. Enter Bilty/LR No

1.1.30. Enter Bilty Date

1.1.31. Enter Vehicle No

1.1.32. Click Submit

1.1.33. Message will be displayed as “Request for form 49 submitted successfully. Please check your registered email id for form 49 pdf”

2. Request for form 50

2.1. Form 50

2.1.1. To request form 50 , click Form 50 in the step 1.1.5

2.1.2. Choose Consignor/Consignee of other state

2.1.3. Enter Name of Consignor

2.1.4. Enter Address of Consignor

2.1.5. Select Consignor State

2.1.6. Select District

2.1.7. Click Proceed

2.1.8. Enter Name of Consignee

2.1.9. Enter Address of Consignee

2.1.10. Select Consignee State

2.1.11. Enter Place from which goods dispatched

2.1.12. Enter Destination (including

district)

2.1.13. Click Proceed

2.1.14. Enter Transporter Company Name

2.1.15. Enter Transporter Company Address

2.1.16. Enter Bilty/LR No

2.1.17. Enter Bilty Date

2.1.18. Enter Vehicle No

2.1.19. Click Proceed

2.1.20. Enter Mobile Number

2.1.21. Enter Email id

2.1.22. Click Add Invoice Details

2.1.23. Enter Invoice No

2.1.24. Enter Invoice Date

2.1.25. Select Goods Category

2.1.26. Enter Total quantity/Weight

2.1.27. Enter Total Value

2.1.28. Click Add (in case multiple invoice detail)

2.1.29. Click Submit

2.1.30. Request submitted successfully.

3. Request for form 60

3.1. Form 60

3.1.1. Enter Consignor TIN

3.1.2. Enter Name

3.1.3. Enter Address

3.1.4. Enter Railway Receipt No

3.1.5. Enter Railway receipt date

3.1.6. Enter Place from which goods

dispatched

3.1.7. Click Proceed

3.1.8. Enter Invoice No

3.1.9. Enter Invoice date

3.1.10. Select Brief Description of Goods

3.1.11. Enter Total Quantity

3.1.12. Enter Total weight/volume

3.1.13. Enter Total Value

3.1.14. Click Modify ( in case you need to modify the

invoice details)

3.1.15. Click Submit

3.1.16. Form 60 requested submitted successfully.

4. Process to Verify Statutory Forms

4.1. Following is the process to verify the statutory Forms (Form 49, C, F, H, E-1 and E-2 Forms) from the app.

4.1.1. After login, Click Verify Statutory Forms in step 1.1.5

4.1.2. Click Add Form No

4.1.3. Select the Form Type

4.1.4. Click Add Form No

4.1.5. Click Get Details

4.1.6. Status of the forms will be display

4.1.7. Click Form No to check the form details

4.1.8. Form Details displayed

4.1.9. Click Invoice No to view the invoice details

4.1.10. Invoice details display

5. Process to View Return Status

5.1. Following is the process to view the return status.

5.1.1. After login, Click View Return Status

5.1.2. Enter TIN

5.1.3. Click Search

5.1.4. Select Act

5.1.5. Select Year

5.1.6. Select Period

5.1.7. Click Get Return Status

5.1.8. Return Status of TIN displayed.

5.1.9. Select the return form no to check the details

5.1.10. Return Details displayed on the screen

6. Track Application Status You can track the status of your application with Track Status functionality To track the status selects Track Application Status in Step 1.1.5

6.1. Select the Type of application

6.2. Enter Acknowledgement No

6.3. Click Search

6.4. Status of the application displayed on the screen

7. Track dealer

7.1. Track dealer status

You can track the details of a dealer with Track Dealer functionality To track the dealer selects Track Dealer in Step 1.1.5

7.1.1. Enter dealer TIN

7.1.2. Click Verify Dealer

7.1.3. Select the service using which you want to find information of dealer.

Note: For SMS standard charges apply according to

operator

7.1.4. Dealer information will be displayed on the screen if Internet is selected in previous step. If SMS is selected in such case dealer information sent through SMS.

7.1.5. Click Ok to return to main screen
