Cardiovascular Emergency

Level One Heart Attack ProgramBefore this program, patients living a significant distance from a major cardiovascular center would receive less optimal care. The Level One Heart Attack Program at Minneapolis Heart Institute® was the first in the U.S. to demonstrate that patients could receive outstanding heart attack care, even if they live 200 miles away. This reduced heart attack mortality rates in 36 outreach communities by 50 percent. Our emergency network has been emulated by programs across the country and has been expanded to include a full spectrum of cardiovascular emergencies. As of 2017, Minneapolis Heart Institute® has treated more than 5,000 patients through this system of care.

Center for Aortic Disease

The Center for Aortic Disease provides a multidisciplinary approach in caring for patients with complex aortic pathology. The program focuses on early detection, monitoring and current treatments of aortic disease. Vascular and cardiothoracic surgeons, cardiologists, intensivists and emergency department physicians collaborate to significantly decrease the time to surgery and mortality.

Critical Limb Ischemia

We know that prompt diagnosis and treatment are mandatory in preventing limb loss. We provide treatment algorithms to help referring clinicians diagnose critical limb ischemia and facilitate transfer. Emergent endovascular and surgical therapies are provided by a multidisciplinary vascular team.

Limb Salvage Center

This year we founded the Limb Salvage Center. This program offers the latest minimally-invasive interventions to treat arterial disease, hyperbaric treatment (for wounds in which the healing process has stalled) and access to clinical trials for cutting-edge medical and interventional therapies.

Cool-It Program

In 2006, the Minneapolis Heart Institute® led groundbreaking research in using therapeutic hypothermia in out-of-hospital cardiac arrest. Combined with the Level One Heart Attack Program, we have offered this care to more than 650 cardiac arrest patients since inception.

Shock and ECMO ProgramMinneapolis Heart Institute® has one of the leading ECMO programs in the nation. Patients with any type of shock can be rapidly transferred from rural and metro facilities to Abbott Northwestern Hospital using our level one network. Patients are treated for various types of shock and pulmonary embolism, as well as advanced heart failure care for those requiring ECMO.

Innovative Vascular CareMinneapolis Heart Institute® combines high-level cardiovascular imaging technology with subspecialty expertise, leading to unique treatment approaches for vascular disease. Our accredited, noninvasive vascular laboratory is one of the largest in the country and serves multiple locations in our outreach network.

Our physicians are pioneers in: • Treating patients with complex aortic aneurysms with

fenestrated and branched endografts; our group has published on the subject.

• Carotid artery stenting using a novel technique called TCAR (trans carotid artery revascularization), the only group doing so in our region. We have the site principal investigator for the roadster clinical trial (TCAR procedure).

The Cool-It care team

Midwest Adult Congenital Cardiology CenterMinneapolis Heart Institute,® Children’s Hospitals and Clinics of Minnesota and Children’s Heart Clinic have collaborated on a center to care for this complex and growing patient population. Optimal care is achieved through collaboration of pediatric and adult cardiologists, med-peds specialists, electrophysiologists, advanced imaging, advanced heart failure, interventional and surgical teams.

Genetic Arrhythmia CenterMinneapolis Heart Institute® created a first-of-its-kind center and care network that utilizes a multidisciplinary approach to care for patients and families affected by inheritable cardiac conditions. SUDDEN CARDIAC ARREST NETWORK

Cardiovascular Outreach Network The Minneapolis Heart Institute® outreach network has been developed over 36 years and is one of the largest in the country. Our network of outreach locations allows providers to care for patients locally in more than 36 communities across Minnesota and western Wisconsin. In 2017, we expanded our network with TeleHeart capabilities in Fargo, North Dakota and Naples, Florida.

This network allows the Institute to implement innovative care delivery systems. For example, the Level One Heart Attack Program has been successful largely because of the mature community relationships our outreach network has fostered through the years.

Our network efforts are guided by a strategic outreach committee led by Minneapolis Heart Institute® physician vice presidents and their administrative partners. This group meets monthly to analyze regions, sites and opportunities for expansion.

One of our most useful outreach tools is the outreach dashboard, an automated tool that assesses our commitment to the triple aim in real-time. This tool uses traditional metrics (patient volume, patient satisfaction, number of new patients, direct visit revenue) to evaluate the impact of visits from a total cost of care perspective.

The strategic outreach committee and the dashboard have helped the Institute create a unique model of data-driven decision-making, bringing tremendous value to patients and the organization.

In addition to extensive regional outreach, we offer global care and training to improve international health outcomes. Efforts include leadership of organizations that provide surgery and specialized training for local clinicians around the world.

MHI Outreach Dashboard

Minneapolis Heart Institute® TeleHeart is a telemedicine program developed through our outreach network. This service provides greater access to sub-specialty cardiovascular care in remote communities, allowing patients the convenience of care close to home. TeleHeart has developed into the largest and most sophisticated cardiology telemedicine program in the country.

TeleHeart patient satisfaction scores are similarly excellent to those at traditional Minneapolis Heart Institute® sites. We currently operate 12 TeleHeart sites across Minnesota and in Fargo,

North Dakota and Naples, Florida. In 2017, the Institute was the first in the world to publish a study on a formalized telehealth program focused on cardiology. (Circulation: Cardiovascular Quality and Outcomes, October 2017)

TELEHEART

Anticoagulation and Thrombophilia ClinicThis clinic is dedicated to the diagnosis and treatment of thrombophilia and clotting disorders. In conjunction with our pharmacy and other experts, we have created guidelines for bridging and managing novel anticoagulants. This information is available online and through our mobile app, making guides and protocols immediately accessible and ensuring accuracy.

Advanced Practice Provider ProgramsAdvanced practice providers (APP) at the Minneapolis Heart Institute® have developed innovated care programs that improve quality and patient satisfaction.

Cardioversion and Implantable Loop Recorder Programs Our advanced practice providers run protocol-driven programs for elective cardioversion of atrial arrhythmias and implantation of loop recorders. This has improved overall quality by reducing unnecessary clinical variation and improving operational efficiencies.

Low-risk Chest Pain Unit at Abbott Northwestern Hospital In 2016, our advanced practice providers and leadership worked across practice lines with the emergency department, hospitalists and inpatient telemetry nursing to develop patient-focused process improvement strategies. We improved patient care and achieved optimal cost-effective care on short-term hospital stays.

Bridge ClinicAdvanced practice providers direct our bridge clinic efforts, ensuring patients who are hospitalized with acute decompensated heart failure have early follow-up. This has resulted in a decrease in 30-day readmission rate from 22.8 percent to 13.5 percent.

Cardiology Curbside Minneapolis Heart Institute® offers Cardiology Curbside, a unique and popular program that allows any physician from any institution to reach one of our expert cardiologists by phone for questions, advice or transfer assistance. The Institute has a full-time cardiologist available to answer questions Monday – Friday, 7:30 a.m. – 5 p.m.

This program continues to support our work on the Triple Aim of health care: Lower costs, higher quality of care and improved patient experience. We have reduced total cost of care by $3.2 million annually.

2016 – 2017 PATIENT CARE CENTER HIGHLIGHTS INCLUDE:

• Established the Minneapolis Heart Institute Foundation® Valve Science Center, which includes physician mentorship and patient seminars to increase awareness of valvular heart disease.

• First implant worldwide of the TriClip device treatment of native tricuspid regurgitation (August 10, 2017).

• Only center worldwide to perform therapy with a dedicated mitral prosthesis in patients with severe mitral annular calcification (4 patients successfully treated via FDA compassionate-use protocol).

• World leader in experience with transcatheter mitral valve replacement for native mitral regurgitation (total of 29 cases).

• Second in the nation for experience with transcatheter MV repair with MitraClip. When compared to all sites within the Society for Thoracic Surgery quality metrics, we are in the 97.2 and 98.7 percentiles for aortic valve replacement and mitral valve repair survivability, respectively.

Accomplishments include:

• Nine peer-reviewed publications in top-tiered journals.

• Two live TVT case presentations.

• More than 50 national and international faculty presentations.

• International training center for complex arrhythmia ablation, hosting more than 50 electrophysiologists from U.S. and Asia.

• First in the U.S. to use advance mapping (ripple mapping) for ablation of atrial fibrillation.

• In collaboration with Minnesota Perinatal Physicians, developed a first-of-its-kind cardio-pregnancy program to care for heart disease patients during pregnancy.

• Pioneered establishment of LifeCourse care guides and integrated palliative care into the heart failure clinic. LifeCourse received prestigious Circle of Life Citation of Honor by the American Hospital Association in 2017.

• One of 61 centers nationally to offer dual-organ heart and kidney transplant. Almost all other institutions on the list are university-based.

• Developed multidisciplinary ECMO team, including an operations committee that meets monthly.

• Lead Allina Health system-wide extracorporeal CPR program.

• ECMO program outcomes for veno-arterial ECMO includes survival-to-discharge of 55 percent, compared to national average of 41 percent.

CREATED THE CENTER FOR COMPLEX CORONARY INTERVENTIONS.

• Performed first randomized, controlled clinical trial comparing various techniques for CTO crossing.

• Published 100 peer-reviewed manuscripts, 10 book chapters and more than 20 editorials.

Physician Leadership and ManagementIn 1995, physician-led Minneapolis Heart Institute® had the vision to recognize the benefits of integrating with Abbott Northwestern Hospital and Allina Health. This was nearly 20 years ahead of the current national trend of integration, and has led to a tremendously successful relationship between the organizations. The partnership created the idea of “service lines” and allowed us to leapfrog ahead in providing cardiovascular care.

Today, the Institute provides management to more than 1,100 employees at Abbott Northwestern Hospital and our 36 sites across Minnesota and western Wisconsin. This work is accomplished through a mature leadership structure, beginning with the executive leadership team. This group includes the physician president, physician specialty-section leaders, advance practice provider lead,

chief operating officer, director of cardiovascular nursing and director of strategy and business development.

Physician leaders are paired with administrative leads to form dyad units throughout the organization. These dyad teams lead all sections and locations, including hospital cardiac and cardiac surgical ICUs, cardiac telemetry units, outpatient clinic and advanced testing units and all 36 Minneapolis Heart Institute® outreach locations.

In 2016-2017, we continued to evolve our leadership structure to address the needs of our patients and organization. Four physician vice presidents were appointed to lead business development, operations, quality and innovation and surgical services. These physician vice presidents were partnered with administrative counterparts to add

continued synergy of skillsets. In addition, we added the director of cardiovascular nursing and advanced practice provider lead to serve on the executive leadership team.

In 2017, we recognized the need to better align with the departments of cardiothoracic surgery and vascular surgery. These groups were historically managed and operated independently. Given the inherent benefits of alignment, we integrated the two surgical groups within the cardiology group, creating a new Minneapolis Heart Institute® with a singular body of cardiovascular physicians. The unified team is now led by our improved structure. This enables us to be even more successful in developing and executing strategic initiatives as health care reform continues to move at an unprecedented pace.

Healthcare Delivery Innovation CenterThe rising costs of cardiovascular care and our aging population make it imperative that Minneapolis Heart Institute® leads in reforming care. The Minneapolis Heart Institute® Healthcare Delivery Innovation (HDI) Center is a cardiovascular model that focuses on improving quality, reducing costs, decreasing care variation and improving care coordination across the continuum. The work requires three unique elements:

1. Visionary leadership and skillsets.

2. Development and use of data through the Allina Health Enterprise Data Warehouse.

3. Commitment of Minneapolis Heart Institute® leadership and physicians to apply these principles.

MINNEAPOLIS HEART INSTITUTE® HDI FOUNDATIONAL PILLARS

CLINICAL AND OPERATIONAL EFFECTIVENESS

In 2016, the HDI Center demonstrated clinical and operational improvements, as well as clinical variation reduction, that resulted in $7.2 million in savings to Allina Health. One of the most important aspects of this work is the positive impact it has had on our patients.

• 25 percent reduction in bleeding complications among high-risk patients undergoing PCI.

• 18 percent reduction in hospital length of stay for patients undergoing cardiovascular surgery who develop post-operative atrial fibrillation.

• More than 1,200 heart failure patients had their care coordinated by an RN CHF Care Coordinator during the 30 days following discharge.

• More than 150 ICU days avoided for acute ST-elevation myocardial infarction (STEMI), transcatheter aortic valve replacement (TAVR) and carotid endarterectomy patients.

PATIENT ACCESS

An important part of the HDI Center is development and implementation of novel digital strategies for care. These efforts improve patient access, reduce wait times, coordinate care and increase connectivity between patient and provider.

• Cardiovascular Innovations 2017 was hosted in Denver (course directors Drs. Sorajja and Brilakis, multiple Minneapolis Heart Institute® providers participated).

• Minneapolis Heart Institute® is hosting the fourth annual Advanced Cardiopulmonary Support for the Critically Ill Adult conference in April 2018.

• The Institute is also hosting the second annual Abbott Multidisciplinary Conference on Heart Failure Management in April 2018.

Cardiovascular Fellowships

• We direct fellowships in the following: general cardiology, interventional cardiology, structural heart disease, vascular and advanced heart failure and transplantation.

• In 2017, we received approval to begin an advanced heart failure and transplantation fellowship.

• We are the flagship clinical site for the University of Minnesota’s ACGME-approved vascular surgery fellowship.

Summer Research Internship Program – The Minneapolis Heart Institute Foundation® offers one of the only cardiovascular-focused summer research internships in the country. This program is designed to allow pre-medical undergraduate students an opportunity to learn clinical cardiovascular research in a structured and supervised setting. 175 interns have completed the program in the past 14 years. They have published 67 original scientific manuscripts in peer-reviewed journals, and an astonishing 75 percent have matriculated medical school or are now practicing physicians.

The app was launched in 2017 to help referring emergency physicians at community hospitals quickly find emergency cardiovascular protocols. It’s been a huge success, with more than 4,000 downloads in six different countries.

We have added a wide rage of resources, including information on anticoagulation and statin therapy. Providers can also easily transfer a patient and connect with Cardiology Curbside within the app.
