HOW THE SERVICE HAS DEVELOPED

• New Intermediate Care Centre

• Increase in intermediate care beds - now 21

• Increase in Therapy imput to both intermediate care and stroke beds

• Development of Reablement Service

• Increase capacity to all service areas.

• Full integration including clear formal management structure.

• Developments in intermediate care services for older people with mental health needs.

What Have We Achieved•New Intermediate Care facility

•Integration in action.

•Increase in Intermediate Care provision.

•Intermediate Care Strategy identifying development up to 2007.

•Establish as key service provision in relation to prevention of hospital admissions and the facilitation of hospital discharge.

•Establish joint management arrangements between Carter Bequest and Intermediate Care Services.

•Staff in the Service have completed the Foundation of Rehabilitation Training..

•Clinical Co-ordinator working in Intermediate Care and managing access to beds across Intermediate Care and Community Hospital.

•Working towards the development of joint rehabilitation services across Intermediate Care and community hospital.

•Established joint management (Service Leads Structure) across intermediate care and the community hospital.

• Improved access to service for Older People with Mental Health needs in line with HASCAS Project.
