Statement of Qualifications

Schneider Laboratories Global, Inc. specializes in the analyses of metals, organic compounds, asbestos and mold in various matrices including air, paint, soil, wipes, building materials, wastewater, drinking water and hazardous wastes. SLGi is an independent, AIHA (ISO-17025)/ NVLAP /VELAP accredited commercial analytical testing laboratory located in Richmond, VA. The laboratory is dedicated to serving the testing needs of occupational/environmental health and safety professionals worldwide. The hallmark of SLGi's success has been our ability to create partnerships with customers to provide accurate laboratory analysis at competitive prices. SLGi provides the security, instrumentation and storage to efficiently and reliably perform all analyses. SLGi utilizes a 20,000 square feet facility which is continually evaluated and renovated as needed to accommodate required analytical/safety requirements. Overall Scope of Services

Since its inception in 1987, SLGI has experienced tremendous growth in all analytical areas including Industrial Hygiene, Environmental, Lead, Asbestos and Mold. Below is a list of the wide array of analytical testing services offered at SLGI. Asbestos Testing–

Bulk Asbestos analysis performed by Polarized Light Microscopy (PLM) using EPA Methods 600/R-93/116 and 600/M4-82-020 and New York ELAP 198.1 and/or 198.6. Airborne Asbestos analysis is performed by Phase Contrast Microscopy (PCM) using NIOSH Method 7400.

Environmental Lead Testing–

Lead in Paint, Dust and Soil by EPA 6000 and 7000 series analysis methods, Lead in surface coatings analysis by CPSC-CH-E1003-9.1 and Lead in Air by NIOSH 7082, NIOSH 7300, and NIOSH 7105 and 40 CFR 50 Appendix G methods.

Multi- Metal Testing–

Metals in Solid, Bulk and Soil Samples by EPA 3000 Series digestion Methods and by 6000 and 7000 series analysis methods; Metals in Water by both EPA 6000 Series and EPA 200 Series Methods and Metals in Air by NIOSH 7082, NIOSH 7300, and other NIOSH methods as well as OSHA ID 121. Metals testing using TCLP preparation by EPA 1311, SPLP preparation by EPA 1312 and California Waste Extraction Test (WET) Method followed by the appropriate analysis methods.

Gravimetric Testing–

Total Suspended Particulate by 40 CFR 50 Appendix B method, Total Nuisance Dust by NIOSH 0500, Total Respirable Dust by NIOSH 0600, and Carbon Black by NIOSH 5000.

Silica Testing–

Samples are analyzed by FTIR using NIOSH 7602 Method.

Mold Testing –

Air, Bulk, Tape and Swab samples are analyzed using AIHA approved In-house methods. Testing includes the identification and quantification of mold spores.

Microbiology Testing –

Drinking water samples are analyzed using NELAC approved methods. Testing includes the identification and quantification of Total Coliform and E.coli Bacteria.

Industrial Hygiene Testing-

Air Samples are analyzed by using NIOSH, OSHA and EPA Methods. Air samples can be tested for Acids, Benzene, Toluene, Xylenes, Formaldehyde, Isocyanates, Hydrogen Sulfide, Ammonia, Sulfur Dioxide, Total Petroleum Hydrocarbons as Hexane, Organic Solvent Profile and many other analytes.

Environmental Testing-

Samples are analyzed by using EPA 300 Series, EPA 600 Series, EPA 3000 Series, EPA 8000 Series Methods and Standard Methods depending on the matrix. Samples can be tested for PCBs, Volatile and Semi-Volatile Organic Compounds, Herbicides, Pesticides, Chlordane, Toxaphene, BTEX and other analytes.

Testing Policies

Turnaround Time (TAT): Same day, 1-day, 2-day, 3-day and 5-day turn-around times are available for most analyses. Standard turn around time on most analyses is 5 business days with weekend analysis available upon advanced notification at NO additional charge. A 2-hr TAT is available for some analyses. Examples of rate increases for faster TATs are as follows:

Priority 1-Day : 100% additional charge

Priority 2-Day : 75% additional charge

Reports are available online or transmitted via fax, email, electronic deliverable (EDD)

The Full TCLP test and certain metals profiles by Graphite Furnace Atomic Absorption have a standard 10 day TAT but may be completed sooner. Any samples received after 3 PM ET are entered into the data entry system as if they were received on the next day business day. All results are reported by the end of the business day which is 6:00 pm.

Weekends: Schneider Laboratories Global offers same day turn-around on Saturday and Sunday testing (except TEMS and send-out/referral tests). Advance notice of sample arrival and are required by Friday at 3 PM ET in order to receive Saturday and Sunday results. Rush turn-around-time requests remain the same for weekends and weekdays. Turnaround time on send-out/referral tests is based on the customer needs. Call your Project Manager to get available turnaround times.

Chain of Custody (COC): Chain of custody forms are provided at no additional charge and are also available online at http://www.slabinc.com/content/resources/. Chain of custody forms should be legible and complete. Include the following information when submitting samples to the laboratory. SLGI will pre-print your COC upon request at no charge.

1. Customer name and address (if not using pre-printed form) 2. Telephone number and fax number 3. Project name/number 4. Date, time and state of sample collection 5. Certification Requirements 6. Number and types of samples 7. Type of analysis requested on each sample 8. Sample identification, including time, flow rates, volumes and areas 9. Purchase order information 10. Signature of person submitting samples 11. Sample identification numbers must be clearly marked on each sample submitted

and are necessary for cross-referencing and sample tracking throughout the laboratory. The number chosen to identify your samples should also be written on the chain of custody form submitted with the samples.

Duplicates: Enough sample quantity must be sent to the laboratory so that the laboratory can perform a duplicate sample analysis for solid and water matrices. If enough sample is received the laboratory will also perform a sample spike analysis. Blanks: Blanks act as a control for analysis by accounting for contamination that can occur during handling, storage, or shipment of samples. A field blank is opened and handled in the same manner as a sample, except in the case of industrial hygiene samples no air should be drawn. Since blanks are analyzed as any other sample in the laboratory, they are priced the same as the analysis requested. Sample Collection Media: A nominal fee will be charged for collection media. Sampling media are routinely shipped by ground delivery FedEx. If air express shipping is requested, the customer will be billed for this service. Billing: Invoicing is by sample group and payment is net 30 days. International customers are prepaid by credit card only. A line of credit can be established upon review of a customer’s credit application. Credit Cards: For your convenience, VISA, MasterCard, Discover and American Express are accepted. Warranty: All sample results are strictly confidential and will not be released to anyone other than the sample submitter without the permission of the customer. Schneider Laboratories Global cannot be responsible for loss of samples, and reserves the right to refuse samples that are not collected on the appropriate sampling medium. Schneider Laboratories Global will employ qualified personnel to analyze samples and, where possible, will use established and recognized analytical testing procedures. Schneider Laboratories Global makes no other warranties expressed or implied. QA/QC Data Packages: For your convenience the laboratory is able to supply the necessary QC package as listed below. The customer has the responsibility to notify the laboratory about the desired QC package prior to or at the time of sample receipt. Level 2 Data Package – additional 25% of the invoice total Basic QC Data:

LCS, LCSD, MS, MSD, DUP and MB

Level 3 Data Package – additional 50% of the invoice total

Calibration Data plus Basic QC data: LCS, LCSD, MS, MSD, MB, RB, ICV, CCV, CCB, Internal Standard and Retention Time Summary

Level 4 Data Package – additional 100% of the invoice total

Analytical Raw Data and Summary plus Calibration and Basic QC data:

LCS, LCSD, MS, MSD, MB, ICV, CCV, CCB, Internal Standard Summary, Retention Time Summary, Pesticide Breakdown Check and all chromatograms, daily instrument checks, sample prep and standard prep logs plus a written summary of analytical results

This is to inform you about Schneider Laboratories Global sample disposal policy. The bottom right hand

side of our COC has a box in which the sender can choose how they want the laboratory to dispose of their

samples. If this box is not checked then the laboratory will dispose of the samples within the appropriate

time frame.

The Sample Disposal box shown above also has a statement which says that a fee of $50 will be charged

for samples with an excessive weight. This is due to the high cost of disposing hazardous waste.

This process bases an excessive weight on the actual amount of sample needed for the analysis. This

disposal fee affects the analysis of soils, paints and bulk samples.

Schneider’s liability is hereby expressly limited to the direct cost of correcting any error or omission in the performance of its services (which shall be strictly limited to the direct out-of-pocket cost of correctly performing the specific analyses or tests referenced herein, or the direct out-of-pocket cost of correctly reporting any result of Schneider’s work).

Acceptance Policy

BASIC REQUIREMENTS

• Samples must be submitted with a SLGi Submittal form or equivalent, with full instructions including tests requested, turnaround time desired, certification needs, submitter and customer information.

• It is the customer’s responsibility to communicate specific methods or detection limits required.

• Samples must be appropriately sealed to prevent leakage or cross-contamination.

• Samples must be clearly labeled.

• Samples should be collected on the appropriate media for the test and/or submitted in the appropriate vessel type. (See Sampling Guide for information per test.)

• Samples should be shipped in a manner to preserve the sample’s safety, quality, and integrity.

• It is the customer’s responsibility to ship the samples to the lab at the appropriate temperature for sample preservation.

• Sample temperature will be monitored upon receipt for temperature-sensitive samples. The receipt temperature for organics environmental samples is displayed on the final report. Customers may be notified of the receipt temperature upon request.

LABORATORY RESPONSE TO UNSUITABLE SAMPLES

• When testing requests, sample labeling, or other associated information is unclear, the laboratory will contact the customer for clarification.

• The laboratory retains the right to reject, or request re-collection and re-submission of any sample believed to be unsuitable for testing.

• The laboratory will provide report comments regarding any samples with questionable suitability.

LABORATORY RESPONSIBILTY & SAMPLER’S / CUSTOMER’S RESPONSIBILTY

• The laboratory does not provide sampling services for its customers.

• All samples are tested as received, and all laboratory analysis results relate only to the portion tested.

• It is the sampler’s responsibility to provide a sample that is representative of the material being investigated.

Schneider Laboratories Global Pump Loan Program

a.) SLGI will provide customer with sampling pumps and accessories at no cost. In return, the

customer must submit $150 worth of analysis back to the laboratory. Customer is responsible for all shipping charges pertaining to this program from and to SLGI.

b.) Items are to be returned by the expected return date, in the condition they were received, unless the customer has written permission to extend the term of use. A customer is allowed one free extension if a week’s notice is given to SLGI. The second extension will cost $20 and the third will be a $50 charge. SLGI recommends insured two-day courier service (Federal Express, UPS, etc.) or sooner. SLGI will ship sampling pumps, chargers, etc. to Customer by ground unless otherwise requested by Customer.

c.) SLGI guarantees that all pumps shipped are fully charged and ready for service. Customer understands and agrees to recharge all pumps before use if sampling is not done within two (2) days from the date shipped by Schneider Laboratories Global, Inc.

d.) Customer can hold pumps for a maximum of 2 weeks. The return date is to be noted on the Supply Request Form under “Expected Return Date” and will be noted on the shipped checklist. Customer will confirm receipt of all requested items and will contact Schneider Laboratories Global, Inc. immediately upon receipt if any items are missing or damaged. This packing list should be signed and returned, otherwise the customer will be held liable for any missing pump parts, upon SLGi receiving the pump.

e.) Customer is responsible for any damage to any sampling pumps, chargers and accessories Customer will be notified within 3 days of receipt if any damages were incurred. Below is a table listing the Pump Loan Program items and their individual replacement cost:

f.) If items are not returned within fifteen (15) days from the Expected Return Date (including extensions), Customer agrees to purchase items and authorizes SLGI to charge the credit card on file (see above table for charges).

g.) In situations where sampling pumps were not able to perform as provided, customer would be reimbursed for any shipping paid proportionately (i.e. if one pump failed to perform out of 5 pumps shipped, Customer would be entitled to reimbursement for 20% of shipping cost). The customer may order replacement of the non-performing equipment at no charge including shipping.

TERMS of Agreement

a) This Agreement is governed by the laws of the State of Virginia.

b) Customer acknowledges and agrees to reimburse SLGi for any SLGi attorney and court costs relating to enforcing this Agreement if it is established that SLGi is the prevailing party.

c) This Agreement is the Sole Agreement between the parties and supersedes any previous agreement signed between the parties.

d) The Customer Representative, indicated below, has appropriate authority to sign this Agreement on behalf of the Customer.

e) The Customer has set forth his/her initials in the box next to the Pump Loan Program Customer understands and agrees to be bound by the terms and conditions of this program that Customer has initialed herein.

f) Schneider Laboratories Global, Inc. may terminate Customer’s participation in any or all of the Schneider Laboratories Global, Inc. programs set forth herein, at Schneider Laboratories Global, Inc.’s sole option, by providing written notice to that effect to Customer. Customer may terminate this agreement and its participation in any of the programs set forth herein at any time by providing written notice to Schneider Laboratories Global, Inc., provided that all equipment, sampling pumps, chargers, accessories, sampling badges and all other Schneider Laboratories Global, Inc. property have been returned to Schneider Laboratories Global, Inc. and provided that Customer has paid Schneider Laboratories Global, Inc. in full for all services rendered by Schneider Laboratories Global, Inc.
