Welcome to the Patholopgy Department at Calderdale &Huddersfield NHS Trust.

pathology directorate provides comprehensive laboratory services in support of diagnosis, monitoring and treatment for patients within the Trust’s hospitals and clinics, and for family practitioners within the local primary care trusts (PCTs). The directorate is made up of four major disciplines:• Cellular pathology (histopathology/cytology) • Microbiology/infection control • Blood sciences (haematology/clinical biochemistry/immunology) • Blood transfusion

All of these provide analytical, diagnostic and advisory services to their users. Analysis of specimens takes place within laboratories at Calderdale Royal Hospital and at Huddersfield Royal Infirmary, which are managed as a single unit. Cellular pathology is located at Calderdale Royal Hospital and microbiology at Huddersfield Royal Infirmary. Blood sciences and blood transfusion are on both sites. The directorate also provides an in-patient and out-patient phlebotomy (blood sample-taking) service at both of the Trust’s hospitals, and at certain clinics and surgeries within the community.

Anticoagulation Service The Haematology department supports the provision of an out patient oral anticoagulation monitoring service across the Trust. The service does NOT prescribe, dose or issue low molecular weight Heparin, this is the responsibility of the ward or patients GP. The department, based at HRI, uses the nationally accredited computerised dosage support system DAWN AC, which is networked between the services. Huddersfield area patientsPatients discharged from the wards who have been started on Warfarin therapy, or patients who have been referred for induction of Warfarin therapy by a consultant or GP are seen at the out patient clinic. This is held in the Out patient department at HRI and is situated on corridor 4 on the ground floor. The clinic is held from 08.45-11.30 on Monday and Thursday mornings. New patients will be seen by a pharmacist who will advise on Warfarin and its interactions. Blood tests are done by finger prick and dosing done by a Biomedical Scientist, although a Haematology registrar attends on Thursday mornings to deal with new/problem patients.

PatientsPatients can register with the practice by requesting a registration application form from reception.We would like to clarify a few points regarding the registration procedure:1. When registering with a new GP

your former GP does not have to be informed. This makes the procedure much simpler.

2. You do not need to move home to be able to register with a new doctor.

3. The whole household does not need to register if they do not wish to - one or more members can be registered elsewhere.

4. Confidentiality regarding all aspects of registration is absolute. Neither your former doctor nor other members of your household will be informed of the name or address of your new doctor.

All new patients will be offered a new patient health check.

PrescriptionsThe surgery will dispense your medication to you from ourown dispensary if you live more than one mile from achemist. If you live within a one mile radius of a chemistyou will be issued with a prescription which can be collectedat a chemist of your choice. This includes your repeatprescription.

Repeat Prescriptionstel 01422 372057 at least 24 hours before theprescription is required.

Prescriptions for drugs which have been authorised as arepeat prescription by the doctor may be ordered:• In person by handing in a written

request at any time during reception hours.

• By telephone after 8am Monday to Friday during opening hours.

• By post (please enclose a SAE if you require the prescription posting back to you).

Repeat prescriptions will be processed and ready forcollection wherever possible within 24 hours of receipt.Home delivery of medication is available to eligiblepatients (those with serious difficulty in collectingmedication eg. elderly, disabled, housebound) pleaseenquire at reception.

• THIS have circa, 250 staff distributed unevenly across 8 sites

• Our accommodation cost us £700k last year – Do we get value for money?

• The cost of our travel last year was £290k – Was it all justified?

• Under the current reforms there is a constant need to increase productivity whilst at the same time reduce costs

• In order for THIS to attract and retain talented individuals we need to provide them with an environment in which they can flourish – We need to embrace the values of Generation Y

• Our host – CHFT are about to embark on a Flexible Working Pilot across Estates & Facilities and Finance. We must follow their lead

It has been agreed by THIS’s Senior Team that we will shortly be initiating a Flexible Working Pilot with the Information Governance Team and the Project and Programme Support Office. The attached communique provides staff with an overview of the pilot.

• A corporate Webex account will be created allowing individuals to hold virtual meetings

• Each member of the pilot will be provided with a SMART phone

• Where possible each member of the pilot will be given access to a sites Wi-Fi (BLH, SWYMHT, CHFT, THIS)

• AnyConnect will be deployed to each member of the pilot to allow them to access everything they access at work anywhere via an Internet connection

Calderdale and Huddersfield NHS Foundation Trust are about to embark on an ambitious journey to offer our patients the opportunity to attend outpatient appointments remotely by using one of our Telehealth options.

Virtual Desktop Infrastructure

Virtual Desktop Infrastructure (VDI) is the practice of hosting a desktop operating system within a virtual machine (VM) running on a centralised server.

The NHS is a complex environment with a multitude of different systems that are used across many disciplines. Our technicians are currently working with our customers to research where VDI would be appropriate and where it would add value or create efficiencies. This type of infrastructure requires a significant capital investment so understanding the long term benefits is essential.

The areas where VDI shine are: • Data Security: Keeping data within the confines

of the data centre improves security. • Reduced Hardware Expenditure: Consolidation

and reduced upgrade cycles minimise hardware costs for server and customer side equipment.

• Easier Management: Centralised patching and application installation or streaming without loading the network.

• Mobile Workforce: User desktops are portable, users can reconnect from any location with a variety of devices.

VDI technology will become much more common in the future along with virtualised server platforms and ‘Cloud’ services. THIS will continue to share the benefits of these new technologies to improve the end user experience.

During November staff at Oak House celebrated the month of Movember with a number of Mo Bro’s who had grown a moustache to raise funds to help change the future of men’s health and raise awareness of prostate and testicular cancer.

The day was celebrated with a quiz and a buffet lunch. Richard Hill (organiser) helped to raise £270 which included website sponsors and donations made on the day.

Neil Talbot-Paterson, the Oak House winner, was presented with a sash and goodies by Mandy Griffin, Chief Operating Officer who herself sported a moustache for the occasion.

Of course the real message is about our ability to walk the talk in that we have a greater responsibility for our own health and well-being as NHS employees when compared with other members of the public.

The Health Informatics Service has agreed a proposal with Dan Evans Consulting to understand how we maximise our reach and influence using social media. The objective is to understand how the integration of social media into our business model will provide the opportunity to engage with clients, prospective clients, thought leaders, project teams, the media and industry bodies in a more strategic way.

The purpose of this work will be to develop and implement a strategy that harnesses social media platforms such as LinkedIn, Twitter and Blog. A team of 6 early adopters will contribute to a programme that will optimise and integrate into our business the following social media platforms:• LinkedIn company page including Products and

Services• LinkedIn individual profiles for the contributors• Twitter Company Profiles• Twitter individual profiles for the contributors• Company Blog

The team will be responsible for agreeing the social media strategy for THIS and how we maximise the benefits across the business.

Evidence from around the world suggests that when we replace handwritten forms and notes with electronic media, both hospitals and patients benefit enormously.

Calderdale and Huddersfield NHS Foundation Trust has for some time recognised the need to move away from paper and a reliance on manual processes to

an electronic record with seamless integration of systems both within and external to the hospital setting. This has been partly achieved using PASweb, ICE Order Entry and others. However, evidence would suggest that continuing to use existing systems in this way is unlikely to achieve an integrated record.

As such the Trust is proposing to embark on an ambitious digital programme that will ultimately deliver a digital care environment and fundamentally change the way it delivers its services to patients and the public.

Examples could include digitising case notes and using ehealth applications to provide remote care to patients in different settings.

In order to progress this piece of work, Dave Lang – IM&T Modernisation Programme Director has been working with CHFT’s IM&T Strategy Group to formulate a plan in order to achieve these benefits.

The plan was presented to CHFT’s Executive Board on Monday 14th January 2013 which resulted in all the recommendations being approved.

Walk the TalkVirtual Private Network

It all starts with the VPN (Virtual Private Network) Concentrator

Cisco has been a leader in remote access VPNs since 1999; and its latest release, the AnyConnect Secure Mobility Solution (part of Cisco’s Borderless Network initiative), has been rolled out across our customer base.

AnyConnect allows customers to remotely access their organisation’s network from home, giving them access to personal and shared drives, clinical systems and the Microsoft Office suite.

Using up-to-date software has allowed us to provide a remote access solution for mobile devices that is far above industry standards.

The project is almost complete and we currently have over 1200 end user devices registered on this system. We have been able to accomplish this with the help of our technical staff, server team and service account managers.

Bradford Teaching Hospitals NHS Foundation TrustI was privileged to be invited by Brian Miller, Chief Executive of Bradford Teaching Hospitals NHS Foundation Trust, to review some of the IM&T services within the hospital that were a cause for concern to the Chief Executive and members of the Trust Board.

We have undertaken 4 or 5 reviews in recent years and in doing so we have always been able to learn from colleagues who at times encounter difficulty with the services that they provide. It is always a privilege and an honour to undertake work such as this and to recognise the need

The Pennine Acute Hospitals NHS Trust manages North Manchester General Hospital, The Royal Oldham Hospital, Rochdale Infirmary and Fairfield General Hospital in Bury. It is a large Trust with a total operating budget of £560m. THIS was approached to provide a range of services to the Trust and has been working with them since February 2012. The main projects have been to:

• Develop an output based specification for a new pharmacy system

• Produce a requirements document for a Consultant Job Planning system

Rob Birkett, Head of Enterprise Services has also spent time in the trust working with the IT Support teams looking at the Incident and Problem management processes, Middle Management Leadership and Customer Feedback.

This work has been well received and has resulted in an excellent relationship with the Trust. There is the potential for us to work collaboratively in a number of areas in the future.

Monday 5th November 2012 saw the inaugural NPEx User Group take place in Bredbury Hall, Manchester. The event was co-hosted by THIS and X-Lab Limited and saw over 80 NPEx users from up and down the country unite to share experiences, best practice and to consider future developments of the solution.

The day was chaired by Jeff Seneviratne - Chair of the Greater Manchester Pathology Network with the keynote address delivered by Ian Barnes - National Clinical Director for Pathology at the Department of Health whose session focused upon Reconfiguration of Pathology Services.

In addition the NPEx developers from XLab carried out a

number of facilitated workshops exploring a variety of technical and business orientated themes whilst considering and exploring a number of potential system developments.

The inaugural event was extremely successful and as a result THIS & XLab Limited NPEx Management Board has agreed that this event will be held annually.

An Interview with Our Employee of the Year

Describe your role in The Health Informatics ServiceI am the manager of the Web Development Team, which consists of 6 Web Developers/Designers and I am also heavily involved with the Software Development Team that develops and maintains Calderdale and Huddersfield NHS Foundation Trust’s clinical systems.

The Web Development Team supports both internal and external requests for website/intranet builds and also requests for web based applications. The daily duties range from offering support to users of our websites, fixing any bugs in our sites and applications, organising and prioritising our workload, scouting for and securing new business and keeping everyone on their toes! I manage a team of great individuals who have a varied skill set to pull from when picking the right approach for a customer request.

I support the Software Development Team in the development of systems. Past developments have included the first version of the PASWeb portal, the Trust’s main Patient Administration System which is an integral part of the Trust’s day to day operations. We have also developed the electronic discharge and GP copy letter modules.

What do you most enjoy about your role?The diversity of my role is what makes it really interesting and a massive challenge. One minute I can be meeting potential customers and going through what web services we can offer them. Then in the next hour I could be dealing with clinical developments, like the patient self-check-in kiosks or the capturing of stroke data from the bedside.

What is your background in the NHS?I started with THIS in 2006 on the Service Desk - when the Service Desk consisted of only 4 members of staff! After 10 months on the Service Desk I moved to the Web Development Team as an Assistant Developer. It was through this role that I was introduced to the Software Development Team and I eventually moved over to work with them on building a web based clinical portal which is the current PASWeb. After a few years in software land the opportunity arose to come back and manage the Web Development Team. I have been in my current management role since March 2012.

THIS Staff Awards 2012

The 2012 Staff Awards was held at The John Smith’s Stadium, Huddersfield on 20th December 2012. The awards are the one occasion each year when the whole organisation gets together to celebrate the success and achievements of that year, and give special recognition to teams and individuals that have gone above and beyond their call of duty.

Over 200 nominations were received from service users and members of The Health Informatics Service. Nominations were welcomed for the following 5 categories selected by the Executive Team: Excellence in Customer Service Innovation and Service Improvement Most Effective Team Excellence in Leadership Employee of the year

A panel of judges, representing all areas and levels of the service, met in December to select a winner for each category and award special commendations to those nominees who had not won but were strong contenders within their nominated category.

The Executive Team had the privilege of selecting the winner for the Quality Champion, who is chosen from those individuals who have been awarded the monthly champion during the year and have shown continued commitment to THIS.

John Rayner presents his personal award to the individual he believes brings something special to the organisation. This award is in memory of our dear colleague, John Hodgson.

For the 8th year running THIS was able to share in its success and publically say “Thank you” to all its colleagues. Congratulations to all of our deserving winners:

Has anyone else spotted the contradiction in the current NHS reforms? For several years now various parts of the NHS have been encouraged to compete with each other, or in similar markets, for the same business. To some extent those working in Health Informatics services have equally found themselves in a similar position. Interestingly, last year’s white paper made several references to competition but not a single reference to collaboration. The contradiction being that we all know that if the NHS is to survive in the current economic climate it must learn very quickly to collaborate.

Working together must become the norm rather than something that appears to happen now on a case by case basis. Finally, true collaboration requires a different kind of leadership from the type that has kept organisations safe in the past. Leaders must become whole system leaders rather than organisational leaders. This is the biggest challenge of all.

National Pathology Exchange (NPEx) User Group

Leadership Development

I would like to think that we work in an organisation that values strong and effective leadership and that our commitment to the leadership cause has stood us in good stead over the years.

The Health Informatics Service has been instrumental in working with others across the region to develop and facilitate a number of high quality personal and organisational development programmes.

More latterly THIS has been the sole provider of the highly successful Emerging Leaders Programme (ELP) which is now running for the 5th consecutive

year. ELP is specifically aimed at those working in middle management positions as well as those managers who are new to the NHS.

As well as providing an opportunity to consider personal leadership, the management of teams, customer service and future trends, the ELP has created a superb network across Yorkshire and the Humber which continues long after delegates have attended the four modules.

I am delighted that NHS YaTH has recognised the value of the ELP and has agreed an investment portfolio that will see the programme continue for

the next 3 years.

NHS YaTH has always been committed to supporting leadership development and as such has also invested in a Senior Leadership Development Programme for those working at Assistant Director level and immediately below.

The next Emerging Leaders Programme will commence in February 2013 with the Senior Leadership Programme beginning late summer. THIS will continue to be instrumental in delivering both of these programmes.

CollaborationOur Service Delivery and Project Management teams are supporting a collaborative approach between Calderdale & Huddersfield NHS Foundation Trust and Airedale NHS Foundation Trust to provide eHealth services into Care and Residential Homes across Calderdale and Kirklees. We are also in discussion with a number of Paediatric Consultants who are interested in using eHealth services to reduce the number of follow up outpatient appointments and to treat clients in their own homes. It is likely that young people suffering from epilepsy and other long-term neurological conditions are likely to be beneficiaries of this type of technology.

Clinical Coding CollaborationIn October 2012 we were approached by a local NHS organisation for help in addressing a backlog of un-coded Finished Consultant Episodes, i.e. admitted patient care, outpatient attendances, accident and emergency attendances and some outpatient procedures. Un-coded episodes can result in a loss of income.

In response to this request, we agreed a support package that has seen four of our clinical coders working off site in order to provide first hand a professional and high quality service.

We understand that our colleagues at the Trust are delighted with the work our coders are doing and our own Clinical Coding Team have continued to meet CHFT’s requirement of 100% of episodes coded within one working day of the month end.

West Yorkshire Deputy HR/OD Director Network SharePoint Facility A SharePoint facility has been created by The Health Informatics Service (THIS) hosted by Calderdale and Huddersfield NHS Foundation Trust to assist and support the work of the group.

The facility, which can be accessed by every network member using a login and password that will be provided, enables e-access to announcements, meeting dates/information, notes and papers as well as affording the opportunity to speedily test out what colleagues in other organisations are doing no matter what the issue by posting 'ask a colleague' questions. A linked email facility notifies network members when any item on the SharePoint site is logged or updated.

SharePoint can be accessed using the NHS network. It cannot be used from a home internet server.

We are NHS Greater Huddersfield Clinical Commissioning Group (GHCCG): a partnership organisation of 40 GP practices, representing a population of approximately 238,000 patients in Huddersfield and the Valleys.
