Dear Applicant, Thank you for applying for the Michael Lynch Memorial Foundation scholarship. Please review the instructions below and the eligibility criteria for the scholarship on our website (http://www.mlynch.org/ml/grants.htm). Please read the following instructions carefully and in their entirety before you begin.

Scholarship Application Requirements

1.) Application Form To expedite your application we have constructed a simple but essential Excel form (please download separately from (http://www.mlynch.org/ml/application.htm) for you to provide the data necessary for the scholarship committee to evaluate your qualifications and needs. White coded cells require you to enter necessary information to complete the application. Light blue colored cells require you to select the relevant data from a drop down list of options. The SAT total box will calculate your overall score. Please make sure that you let us know your current grade level. This should be noted on your application form in the spaces provided. While most applicants are high school seniors, we also accept applications from college-level students. If you are currently a college student, in addition to submitting all the basic high school senior requirements such as SAT scores, high school transcript, we will also need your resume updated to reflect both your high school and college activities, etc. along with your college transcript information. The checklist also spells out the requirements if you are currently a college student.

2.) Essay: One Page, Single Spaced The essay should describe your college and career goals and how a grant from the MLMF will better enable you to achieve these goals.

3.) Transcripts & Test Scores Please request your transcripts and SAT and/or ACT scores from your student guidance office and counselor. See submission instructions below.

4.) Two (2) Letters of Recommendation Please scan a minimum of two letters of recommendation provided to you into PDF files and attach in an email with all other application materials. At least one letter must be from a teacher in your school.

5.) Tax 1040 IRS Documents Please scan the first two pages of your parents IRS 1040 tax form (most recent that is available) or a PDF file exported from programs like a turbo tax, tax cut etc. Please block out social security numbers on tax forms to protect confidentiality.

6.) Outline of Extracurricular Activities & Work History / Resume Please submit a resume outlining all significant extracurricular and work history, including school activities, athletics, community service, clubs or other organizations. Please include key contributions and/or leadership roles you have held in these activities. Also provide hours per

week and years participating for work during summers and/or during the school year. There are no specific resume format requirements.

7.) Application Checklist: Note: this section must be filled out for your application to be

considered complete!

The Selection Committee will only review complete applications. It is the applicant's

responsibility to ensure all materials are submitted by the due date published on our website. A

checklist is provided on the application form to help the applicant submit a complete

application.

In summary, a complete application will include the following:

1. Application Form 2. Required Essay 3. Letter Of Recommendation #1 4. Letter Of Recommendation #2 5. Additional Letters Of Recommendation (Note: additional letters are optional) 6. Resume / Outline of Extracurricular Activities & Work History (Note: College

extracurricular activities should also be included for college level applicants) 7. Tax Forms - 1040 IRS (First Two Pages) 8. Transcripts (Note: College level applicants must include both their high school and

college transcripts) 9. Sat / Act Scores

8.) Submitting Your Application – Instructions:

Applications must be submitted by February 1. Electronic submission of documents is strongly preferred.

Please save all documents as Adobe PDF or Microsoft Word .doc, or. docx file. Please save / submit all documents using the following naming convention:

Last Name_First Name_MI_Document Name For Example: Doe_John _E_Applicationform.pdf
