MHA 100 YEAR ANNIVERSARY SPONSORSHIP DETAILS AND CONDITIONS

� Exposure and recognition is based on date of commitment and receipt of Intent to Sponsor form. An EPS (vector) format logo must be included at the same time, or there is no guarantee your logo will be printed on materials.

� To be included in all of the MHA 100 Year Anniversary promotions and to be recognized in the MHA 100 Year Anniversary video, the statewide news release, and on the MHA 100 Year Anniversary banner, sponsorship must be confirmed by submitting an Intent to Sponsor form by Dec. 7, 2018.

� To be recognized on the MHA 100 Year Anniversary traveling display, sponsorship must be confirmed by submitting an Intent to Sponsor form by Jan. 11, 2019.

� To be recognized in the Breakthrough meeting guide, sponsorship must be confirmed by submitting an Intent to Sponsor form by Jan. 11, 2019.

�To be recognized in the Annual Meeting brochure, meeting guide, digital announcement and during the centennial celebration, sponsorship must be confirmed by submitting an Intent to Sponsor form by May 10, 2019.

� Upon receipt of your Intent to Sponsor form, you will be sent an invoice for sponsorship. The MHA has the right to release and resell any sponsorship items not paid in full 30 days after the invoice date. Payment by check sent with the Intent to Sponsor form is also accepted.

� Verbal commitments will not hold sponsorship for your organization.

� Sponsors will be notified of their sponsorship acceptance and receive information with additional details.

The MHA and the MHA Health Foundation reserve exclusive rights to determine appropriate items for distribution and use of sponsor logos or sponsor name recognition on any and all MHA and MHA Health Foundation meeting materials, clothing, gifts, signage, displays and all other items, events, venues and materials associated with sponsorship.
