SECTION II – INTRODUCTION AND SUMMARY OF WORK

01 INTRODUCTION

The Metropolitan Washington Airports Authority (referred to herein after as "Airports Authority") is responsible for the operations, maintenance and repair for the Dulles Toll Road (referred to herein after as DTR). The Airports Authority's headquarters is located at 1 Aviation Circle, Washington, D.C. (referred to herein as "COB"). This Statement of Work (SOW) addresses services, which are to be performed for the DTR.

02 SUMMARY OF WORK

The contract that will be awarded by this solicitation shall require the Contractor to provide for all labor, supervision, transportation, equipment, tools, materials, supplies, traffic control and incidentals necessary to complete assigned projects for Lighting maintenance, repair, and replacement on the Dulles Toll Road Main Line, Ramps and Toll Plazas.

The term of this requirement is intended to consist of a two year base period with three one-year option periods.

03 PERIOD OF PERFORMANCE

The Period of Performance for this Contract will be a Two-Year Base Period with Three One-Year Option periods.

SECTION III - DEFINITIONS

AIRPORTS AUTHORITY - Shall refer to Metropolitan Washington Airports Authority.

BUSINESS OFFICE HOURS - The COTR’s work hours are 7 AM – 3:30 PM EST/DST, Monday through Friday, excluding weekends and federal holidays.

CALL BACK - A response to any service requests during business office hours.

CALL ORDER - Document which contains a detailed description of the services, cost estimate, and schedule required for any work performed under Emergency Callback or Supplemental Services.

CONTROL ROOM – A 24/7 unit of the DTR. Where the Contractor shall check-in and checkout upon arriving and departing the job site. This unit is responsible for receiving, dispatching, and tracking service requests; 703-572-4902.

CORRECTION - The elimination of a violation or deficiency.

COTR - Contracting Officer’s Technical Representative.

DEFICIENCY - Any non-compliance with the contract.

DTR - Dulles Toll Road

Emergency Callback – A response to any service outside of business office hours.

MA-240 - Dulles Toll Road Division.

METROPOLITAN WASHINGTON AIRPORTS AUTHORITY (the Airports Authority) - The public body responsible for the operation and management of Washington Dulles International Airport, Ronald Reagan Washington National Airport, and the Dulles Toll Road.

VWAPM – Virginia Work Area Protection Manual.

MUTCD – Manual of Uniform Traffic Control Devices.

OEM - Original Equipment Manufacturer

OSHA - U. S. Occupational Safety and Health Administration.

QUALITY CONTROL PROGRAM - A method used by the Contractor to assure that a consistent level of quality services are maintained thorough out the terms of the contract, ensuring the characteristics of their work satisfy the proper standards of the contract requirements, and that the Contractor is being attributable to the performance necessary to maintain the desired results.

QUALITY ASSURANCE - A means by which the Airports Authority is able to confirm that the quantity and quality of services received conform to contract requirements. These methods/procedures are not intended to aid the Contractor in the performance of the contract requirements and shall not be a substitute for contract quality control.

SECTION IV - BASE SERVICES

01 DESCRIPTION OF SERVICES

The Contractor shall provide all labor, supervision, transportation, equipment, traffic control, tools, materials, supplies and incidentals necessary to complete assigned projects for Lighting maintenance, repair, and replacement on the Dulles Toll Road main line, ramps and toll plazas.

The Contractor shall provide experienced and qualified staff to perform all requested Lighting maintenance and repair. Employees shall have valid electrical licensing certification as required (i.e. Master Electrician License and/or Journeyman’s Card/Certification) to perform the duties outlined in this scope of work. A copy of licenses shall be furnished with the bid package.

These services are a statement of the minimum level of work and services that the Contractor shall provide. They are not intended to be, nor shall they be considered as, either minimum performance levels or limitations of the effort of the Contractor. At a minimum, the Contractor shall take all steps and measures that a prudent Contractor would take to maximize the life expectancy of the property.

The Contractor shall provide sufficient and proper equipment necessary for the satisfactory execution of work. A list of equipment shall be provided using the DTR Proposed Equipment and Vehicle Plan (Appendix D) attached to this SOW.

The Contractor shall furnish a three person work crew consisting of a Journeyman, an Electrician, and a Helper or Laborer.

The Contractor shall provide each three person work crew with all the necessary tools and equipment to perform the assigned work including but not limited to a bucket truck capable of reaching heights of 66 feet, trencher, jet lines, air compressor, inspection truck, power tools, and cell phones for daily work assignments of electrical maintenance and for repairing, or replacing luminaries, lamps, ballasts, photocells, navigational lights, conduits, conductors, electrical cables, junction boxes, pole foundations, pole anchor bolts, resetting poles, pole arms, and high mast cables, locating and repairing electrical faults and modifying service panels.

The Contractor shall provide the Contracting Officer’s Technical Representative (COTR) or his representative a signed service report within 24 hours that includes as a minimum the names of the work crew members, a detailed statement of defect or problem(s) identified, repaired or actions taken, materials or parts furnished or used (Airports Authority or Contractor supplied), and number of hours expended to complete repairs or assignments.

When necessary, the Contractor shall perform work from a bucket truck or lift. The use of sign structure walkways, “cat walks” and/or scaffolding is strictly prohibited.

The Contractor shall be responsible for traffic management for work operations performed under this contract, and shall provide all necessary traffic control devices and safety equipment to make the conditions of work areas as safe as possible for all employees and the public. Traffic control equipment to be provided shall include but not be limited to; Portable Changeable Message Signs, Electronic Arrows, Truck Mounted Attenuators, Cones, Barrels, Sign Stands, Signs, etc.

The Contractor shall establish work zones and manage traffic in accordance with the latest version of the Virginia Work Area Protection Manual (VWAPM) and the Manual on Uniform Traffic Control Devices (MUTCD)inclusive of any subsequent revisions and mandated requirements thereof. The Contractor shall set up, move, and remove work zones, signs, cones, and other traffic control devices as required.

All shoulder/lane closures shall be submitted to the COTR and the DTR Management 48 hours in advance for approval. Once approved the Contractor shall be required to coordinate all closure notifications with the DTR Control Room. The DTR reserves the right to remove the Contractor from the roadway at any time due to high volume traffic conditions and situations that are deemed to be in the best interest of the traveling public. The Contractor shall not perform any lane closure activity until the COTR approves the closure.

The Contractor shall document the location (mile marker or the nearest pole number) of the junction boxes or manholes, and the number of splices made for all electrical fault repairs for invoicing and payment.

The Contractor shall not perform any work unless approved or assigned by the COTR or his representative.

All labor, traffic control, debris removal, including legal disposal as required to perform trouble shooting, inspection, replacement, cleaning, fastening, adjusting and aligning shall be considered incidental to this contract and shall be included in the base bid price for “HOURLY”.

02 BASE SERVICES TASKS

On a weekly basis the Contractor shall perform troubleshooting and repair of the roadway lighting on the DTR to identify and repair electrical shorts or faults at the manholes, service panels, and junction boxes in accordance with the National Electrical Codes (NEC) and the U. S. Occupational Safety and Health Administration (OSHA) Standards using appropriate safety equipment and procedures. Once a month at night, the Contractor shall perform a visual inspection of all roadway lighting owned by the DTR to document and prepare a list of any outages or issues in need of repair.

A. Sign and Under Bridge Lights – Lamps and Ballasts: The DTR has HPS 150 watt sign and 100 watt under bridge lamps and ballast. The Contractor shall perform the following:

1. Establish appropriate lane closures, traffic control, and work zones. 2. Furnish and install new lamps and ballasts. 3. Inspect all fasteners for electrical/lighting systems, fixtures and accessories, and

securely tighten or replace as needed. Replacement fasteners shall be incidental to the contract.

4. Clean and polish lighting fixture inner surface reflectors. 5. Clean all gaskets of foreign material, oxidation, and align as needed. 6. Inspect all lamp sockets and replace where necessary. Replacement sockets shall be

incidental to the contract. 7. Wash and spot-free dry all glassware. Replace missing, cracked, or broken glassware

in kind. Replacement glassware shall be incidental to the contract. 8. Ballast for sign lighting system lamps shall be multi-volt ballast rated for 120, 208, 240,

277, and 480 volts due to various power voltages at various locations. 9. Ballast physical dimensions shall fit fixtures without modifications for proper installation;

otherwise single tap ballasts may be used in accordance with the manufacturer’s specifications.

B. Roadway Lighting – Lamps and Ballasts: The DTR has HPS 400 watt, and HPS 250 watt roadway lamps and ballast. The Contractor shall perform the following:

1. Establish appropriate lane closures, traffic control, and work zones. 2. Furnish and install new lamps and ballasts. 3. Inspect and securely fasten all anchor bolts and nuts as needed. 4. Visually inspect foundation and report to the COTR any observed defects, undermining,

erosions, or cracks.

5. Visually inspect pole bases or breakaway support couplings and report to the COTR any observed structural or safety defects.

6. Inspect and securely fasten arm connections and fixtures as needed. 7. Securely fasten fixtures and lens covers when replacing lamps and or ballasts. In the

event a lens cover should become unfastened (open) within 30 days of lamp and/or ballast replacement, the Contractor shall secure (refasten in place) such cover(s) at no additional cost to the Authority.

8. Securely fasten all covers and plates after inspection and/or working on the poles. The Contractor shall document on the signed service report all missing covers and/or plates and notify the COTR or his representative of resulting unsafe conditions immediately.

C. Canopy Lighting- Lamps and Ballasts: The DTR has metal halogen lights with 250 watt bulbs and ballast. The Contractor shall perform the following:

1. Establish appropriate lane closures, traffic control, and work zones. 2. Furnish and install new lamps and ballasts. 3. Inspect all fasteners for electrical/lighting systems, fixtures and accessories, and

securely tighten or replace as needed. Replacement fasteners shall be incidental to the contract.

4. Clean and polish lighting fixture inner surface reflectors. 5. Clean all gaskets of foreign material, oxidation, and align as needed. 6. Inspect all lamp sockets and replace where necessary. Replacement sockets shall be

incidental to the contract. 7. Wash and spot-free dry all glassware. Replace missing, cracked, or broken glassware

in kind. Replacement glassware shall be incidental to the contract. 8. Ballast physical dimensions shall fit fixtures without modifications for proper installation;

otherwise single tap ballasts may be used in accordance with the manufacturer’s specifications.

D. Photocells: The Contractor shall furnish all materials and labor required for replacing photocells (pertaining to photocells located on poles and other locations if applicable) as follows:

1. Furnish and install new photocells. 2. Test and verify proper function. 3. Remove and legally dispose of debris.

E. Light Pole Foundations: The Contractor shall furnish all materials, labor, and testing required for replacement of light pole foundations as follows:

1. Remove and legally dispose of existing foundation. 2. Excavate appropriate cavity size and depth using proper equipment such as an Auger

truck. 3. Install necessary forms to secure reinforcement steel, conduits, and anchor bolts. 4. Furnish and install new reinforcement steel, conduits, ground rods, anchor bolts,

conductors, and concrete. 5. Remove debris and restore work area to include grading and reseeding. 6. Restore conductors terminate cables and grounding readying replacement foundation for

new light pole. 7. The Airports Authority’s Engineering Department, MA-30 will furnish the Contractor with

the standard foundation details that includes soil compaction, materials, dimensions, sizes, and types for the existing foundation being replaced.

F. Anchor Bolts: The Contractor shall furnish all materials and labor required to replace anchor bolts as follows:

1. Remove defunct anchor bolts using proper equipment. 2. Bore appropriate cavity size and depth using proper equipment. 3. Furnish and install new anchor bolts and grout epoxy. 4. Remove and legally dispose of debris. 5. Restore conductors terminate cables and grounding readying replacement foundation for

the new light pole.

G. Broken Studs: The Contractor shall furnish all materials and labor required to replace studs as follows:

1. Remove all broken, bent damaged and structurally deficient studs. 2. Furnish and install new studs. All studs replaced shall have the structural integrity as

required for firmly securing the pole or other fixture. 3. Remove and legally dispose of debris. 4. Furnish nuts and washers required for resetting light pole. 5. Restore conductors terminate cables and grounding readying replacement foundation

for new light pole.

H. Resetting Light Poles: The Contractor shall furnish all materials and labor required to reset light poles as follows:

1. The Contractor shall supply replacement light poles. 2. Raise light pole using proper lifting equipment. 3. Securely fasten new light pole leveled and plumb to foundation using proper nuts and

washers.4. Install arm, fixture, luminary, and/or offset lights. 5. Reconnect conductors to new light pole laminar and restore power. 6. Test and verify proper illumination. 7. Remove and legally dispose of debris including knocked down pole.

I. Night Inspections: The Contractor shall perform night inspections once a month as scheduled by the COTR as follows:

1. Provide a supervisor or technical level employee to perform the visual inspection with a DTR representative after dark.

2. Note locations and type of roadway and sign illumination defects observed in the field. 3. Submit a typed or neatly written detailed report of the type and location of all defects

and observations of the roadway lighting and electrical system by email or fax to the COTR within two business days promptly without exception.

4. The DTR will use the report to determine work priorities for the upcoming month.

J. Upon completion of any troubleshooting, maintenance, repair, and/or replacement work, the Contractor shall provide a copy of the work ticket to the COTR at the completion of all preventive maintenance, repairs, or replacement. Work tickets shall identify all work performed and items found deficient and shall note any potential issues. Work tickets shall be provided no later than the next business day after completion of the required work performed.

K. The Contractor shall be capable of fulfilling all requirements of this SOW.

03 PARTS AND MATERIALS

The Contractor shall provide all materials and parts necessary for maintenance/repair of the system(s) under this contract. All replacement materials, parts, and units shall be the same as the existing or an approved equal (approved by the COTR). Approved replacement of lighting or parts shall be billed to the Airports Authority at the Contractor's actual invoice cost plus 10%. There will be no mark-up for shipping and handling. Parts and materials shall be itemized on the Contractor's invoice with back-up documentation of the Contractor’s invoice.

04 MATERIAL AND WORKMANSHIP

A. All parts, supplies and/or materials furnished under this contract shall be new and genuine manufacturer's recommended or authorized replacement parts. Use of manufacturer rebuilt parts will be considered on a case by case basis and must be approved by the COTR, provided these parts carry the same warranty as the new part or component. Use of used parts is strictly prohibited.

B. All manufacturers' installation, operation warranty if applicable, and parts manuals shall accompany invoices submitted to COTR for installation of equipment provided by Contractor.

C. All repairs and replacement parts shall comply with the most current editions and meet the applicable minimum requirements of standards of the following organizations and approved by the COTR.

1. National Association of Highway and Transportation Officials (ASHTO). 2. National Electrical Manufacturer’s Association (NEMA). 3. National Electric Code (NEC). 4. American Society for Testing Materials (ASTM).

D. The Contractor shall use lamps with a minimum 24,000 hour-rated life and electrical components that conform to Road and Bridge Specifications, Section 238 current addition of the MUTCD.

E. The Contractor shall maintain sufficient quantity of repair parts and tools on hand or have ready access to these parts and tools in order to prevent unnecessary downtime.

F. The Contractor shall guarantee all parts and workmanship for a period of six months or the manufacturer’s warranty, whichever is longer, beginning on the date of repair or replacement, on all electrical, mechanical equipment, and related components. The Contractor, at no additional cost to the Airports Authority, shall make repairs for the same reason or problem within this time period.

G. The Contractor shall notify the DTR of any repairs which may be covered under the equipment manufacturer’s warranty or may be considered resulting from defects in the equipment prior to proceeding with any work.

H. The DTR reserves the right to provide necessary project materials and/or equipment when determined to be in the best interest of the Dulles Toll Road.

I. All leftover materials, whether provided by DTR or by the Contractor and charged to DTR, shall be turned over to DTR at the completion of the project. All salvaged material shall be disposed of by the Contractor or turned over to the DTR as directed by the COTR.

J. The DTR will have the authority to suspend work, fully or in part, due to failure of the Contractor to correct conditions unsafe for the workers or the public, due to failure to carry out orders or for any other circumstances deemed to be in the public’s interest.

05 CONTRACTORS GENERAL PROCEDURES AND RESPONSIBILITIES

A. CONTACT PERSON: The Contractor shall assign a person as the contact who will have the authority to execute the contracted work. This person's name and contact numbers shall be provided on the form titled Normal and Emergency Telephone Numbers.

B. EMPLOYEE LIST: A list of the Contractor's employees with skill level and years of experience performing this type of service shall be provided prior to award of contract. Only employees listed may report to work on this contract. The list may be updated as needed with approval of the COTR.

C. COMMUNICATION: The Contractor shall have on the work sites at all times a competent and authorized individual (Lead Person on site) who is able to efficiently and effectively understand directions in English from authorized DTR representatives and provide these directions, be they oral or written, to the Contractor's work force in a manner which facilitates timely and proper execution of the work in a satisfactory manner as per the contractual obligations.

D. DAMAGES: The Contractor will be fully responsible for damage to DTR property caused by negligence or abuse by Contractor employees or equipment, as determined by an authorized DTR representative. Should the Contractor, as a result of its work cause a cut in any communication media, it is the Contractor’s responsibility to repair that cut immediately. If any utility service is interrupted as a result of accidental breakage or of being exposed or unsupported, the Contractor shall promptly notify the proper authority and shall cooperate with that authority in the restoration of service. If utility service is interrupted, repair work shall be continuous until service is restored. The Contractor shall be responsible for any damage to the utilities that are attributable to its neglect or methods of performing operations.

E. CONTRACTOR VEHICLES: The Contractor shall not park its equipment on DTR property overnight or for extended periods (i.e. over weekends) without prior approval from the COTR. All vehicles shall have company/owner name displayed on both the driver and passenger doors. Also, all vehicles shall be equipped with high intensity amber strobe lights.

F. No work shall be undertaken around fire hydrants until the local fire authority has approved provisions for continued service. When the Contractor’s work operations require the disconnection of “in service” fire hydrants, the Contractor shall notify the locality’s fire department or communications center at least 24 hours prior to disconnection. In addition, the Contractor shall notify the locality’s fire department or communications center no later than 24 hours after the reconnection of such hydrants.

G. The Contractor is responsible for the conduct of all personnel while on the job.

H. All electronic and measuring equipment used in the performance of this contract shall be calibrated as per the manufacturer’s specification requirements.

I. The Contractor shall call Miss Utility for utility markings, locating, and clearance notification prior to starting any work. It shall be the Contractor’s responsibility to comply with all Miss Utility Rules and Regulations.

J. It is the Contractor’s responsibility to be familiar with and understand the terms, conditions, specifications, and requirements of this contract, including needed equipment to perform the work.

K. The Contractor shall cooperate with owners of utility lines so that removal/adjustment operations may progress in a reasonable manner, whereas, duplication of adjustment work may be reduced to a minimum, and services rendered by those parties will not necessarily be interrupted.

06 AUTHORIZED HOURS OF WORK

Normal work hours are 9:30 AM to 3 PM, Monday through Friday, at most locations with the exception of federal holidays. On occasion, weekend and holiday work may be required. Federal holidays include, but are not limited to: New Year's Day, Martin Luther King, Jr. Day, President's Day, Memorial Day, July 4th (Independence Day), Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. With prior approval, the contractor may be allowed to work on these holidays.

Man hours paid under this contract shall only be for productive hours at the job site. Time spent for transportation of workers, material acquisition, handling and delivery, or for movement of Contractor-owned or rental equipment is not chargeable directly but is overhead and the cost shall be included in the hourly rate bid for labor.

Routine Response: The Contractor will be notified when service is needed. The Contractor shall provide required services within three days of notification.

Emergency Response: The Contractor shall have sufficient qualified personnel to respond promptly within two hours of notification to trouble calls on a 24/7 (twenty-four hours a day/seven days a week) basis upon receiving notification from the COTR or his representative. The qualified personnel shall be prepared to perform temporary repairs or maintenance work to restore power and or stabilize unsafe conditions. The Contractor shall provide and maintain current telephone numbers and names of those personnel that will respond to emergency calls. Emergency Response will be invoiced under Supplemental Services. See Section V – Supplemental Services.

07 QUALITY CONTROL AND FINAL ACCEPTANCE

At the conclusion of the work, the Contractor shall demonstrate to the COTR that the work is in compliance with contract specifications and all applicable codes. Any deficiencies shall be promptly and permanently corrected by the Contractor at the Contractor's sole expense prior to final acceptance of the work.

SECTION V – SUPPLEMENTAL SERVICES

01 DESCRIPTION OF SERVICES

The Airports Authority may, during the term of this contract, request that the Contractor perform supplemental services which are outside the requirements of the base services portion of the contract, i.e., labor for repairs, replacement of parts and/or replacement of units, which are outside of the normal monthly preventive maintenance requirements and within normal business hours, will be measured and paid for by the hour under Supplemental Services.

The Contractor shall provide all materials and parts necessary for maintenance/repair of the system(s) under this contract. All replacement materials, parts, and units shall be the same as the existing or an approved equal approved by the COTR. Approved replacement of the roadway lighting components or parts shall be billed to the Airports Authority at the Contractor's actual invoice cost plus 10%. There will be no mark-up for shipping and handling. Parts and materials shall be itemized on the Contractor's invoice with backup documentation of the Contractor’s invoice.

02 ADDITIONAL THREE PERSON CREW

The Contractor shall furnish an additional three person crew when necessary to expedite completion of a large task and/or to assist in resolving emergency calls. Activation of an additional crew will be at the sole discretion of the COTR or his representative. The Contractor shall contact the COTR or his representative to get approval of the additional three person crew. The three person crew shall consist of one Journeyman, one Electrician, and one Laborer/Helper.

03 EMERGENCY RESPONSE

The Contractor shall have sufficient qualified personnel to respond promptly to trouble calls on a 24/7 basis for Emergency Response and be capable of responding within two hours of receiving notification from the COTR or his representative.

The Contractor shall provide and maintain current telephone numbers and names of those personnel that will respond to handle both the emergency and non-emergency notifications by the COTR or his representative. All equipment, labor, and materials used to handle the emergency response shall be included in the bid price.

The Contractor shall be responsible for traffic management for emergency work operations performed under this contract, and shall provide all necessary traffic control devices and safety equipment to make the conditions of work areas as safe as possible for all employees and the public. Traffic control equipment to be provided shall include but not be limited to; Portable Changeable Message Signs, Electronic Arrows, Truck Mounted Attenuators, Cones, Barrels, Sign Stands, Signs, etc.

04 CONTRACT SERVICES CALL ORDER

A. The COTR shall provide the Contractor with a “Contract Services Call Order” (Call Order) which includes a description of the work that is required. The standard Call Order form is attached as Appendix A. The Contractor shall provide the COTR a detailed cost and time estimate which includes an itemized breakdown for labor, parts, and materials as well as a schedule with critical milestones for completing the requested supplemental service for work not itemized in the Price Schedule. The COTR will authorize the Contractor to proceed by signing the Call Order. The Call Order becomes binding when the Contractor signs the Call Order.

B. Labor and unit rates included on the price schedule for the contract shall be used in preparing supplemental services call orders. The Contractor shall not proceed with any of the work described in such call orders until written authorization has been received from the COTR.

C. The Contractor will be reimbursed for the unit rates or labor and materials to complete such tasks in accordance with the provision of Section VII of this SOW entitled "Method of Payment".

D. The Airports Authority shall incur no obligation for out of scope work that is not authorized in advance and in writing with the exception of Type I inspections on post accident/injury inspections that may be directed verbally to initiate the emergency service by Airports Authority staff. Call orders for Type I inspections will be issued the following business day.

SECTION VI – GENERAL REQUIREMENTS

01 CODE COMPLIANCE

All work shall be accomplished in compliance with all applicable Federal, State and Local laws, ordinances, rules, regulations, manufacturer's recommended procedures, industry standards and codes including Virginia OSHA standards, the NEC, and the International Building Code (IBC).

All work performed under this contract shall be done in a manner that will not adversely affect the integrity of the building's structural, mechanical, electrical, plumbing, fire protection and life safety systems or any other building features that will overload or render useless any portion of the facility.

02 PERSONNEL

A. Contractor employees shall be experienced and qualified in all aspects of their discipline.

B. Contractor employees shall wear a shirt, jacket or another similar article that identifies the company and the employee name when performing services under this contract.

C. Employees shall carry on their person at all times either Contractor Identification or a form of legal governmental identification (i.e.: driver's license).

03 DEBRIS AND WASTE DISPOSAL

The Contractor is responsible for the disposal of all refuse, rubbish, scrap materials and debris caused by its operations. No such refuse, rubbish, scrap material or debris shall be left within the completed work area nor buried on site, but shall be removed from the site and properly disposed of in a licensed landfill or otherwise as required by law. No Airports Authority dumpsters are to be used for debris disposal without prior approval. The Airports Authority shall have the right to ask for old parts or equipment.

04 SAFETY REFERENCES

It is the Contractor’s responsibility to verify site conditions are safe prior to starting work.

All work performed and materials used, unless otherwise noted in the bid documents, shall conform to the Airports Authority’s Construction Safety Manual, the May 2005 Virginia Work Area Protection Manual, and the most recent edition of the Manual on Uniform Traffic Control Devices (MUTCD).

All personnel shall at all times wear approved protective clothing, safety vests, and any other equipment required to meet current OSHA and VOSH standards. They will obey all traffic and safety rules and regulations and shall not create any hazardous conditions within their operation.

All Contractor supplied equipment, tools, and personal safety equipment shall meet or exceed the safety standards specified for such items by the American National Standards Institute Manual (ANSI Z133.1-1994).

05 SAFETY DATA SHEETS (SDS)

Prior to award of this contract, the successful Bidder/Offeror shall provide to the COTR a copy of SDS sheets for each product used within ten calendar days of the verbal or written request.

06 SUPPLEMENTAL SERVICES

The Airports Authority may, during the term of this contract, request that the Contractor perform supplemental services which are outside the requirements of the base services portion of the contract, i.e., perform routine inspections, provide post accident/injury inspections and tests (Type I Inspections), and/or to provide technical on-site consulting/inspections to the Airports Authority on projects such as additions to equipment, rehabilitation/refinishing or repair work (Type II Inspections). Type II inspections also include providing conveyance unit analysis (on maintenance and/or unit trending), presentations to Airports Authority staff, and consultation as required when it’s necessary to bring equipment up to new standards with the provisions of current applicable laws and the ASME A-17 Safety Code.

07 CHECK IN/CHECK OUT

Prior to start of each work day the Contractor shall immediately check in with the COTR or a Dulles Toll Road representative and provide description and location of services to be performed. At the conclusion of each work day and prior to departure from job site, the Contractor shall check in with the COTR or a DTR representative.

08 TOLL FEES

The Contractor shall be responsible for the cost of tolls to travel on the Dulles Toll Road.

09 ACCIDENTS

The Contractor shall be responsible for promptly notifying the COTR and the Airports Authority Police of any accidents arising from the performance of this contract involving bodily injury to workers, building occupants, visitors, or other persons. The COTR will provide information necessary concerning whom to contact and the specific form of the follow-up written notice.

10 SMOKE FREE ENVIRONMENT

The Airports Authority’s facilities are smoke free. The Contractor and its employees shall adhere to the rules and regulations in regard to this facility’s maintenance of a smoke free environment.

SECTION VII – METHOD OF PAYMENT

The Contractor shall submit an invoice on a monthly basis for all services completed to the satisfaction of the COTR at the end of the month in which the services were performed. The Airports Authority shall incur no obligation for out of scope work that is not authorized in advance, in writing. These monthly invoices shall be itemized to provide a breakdown of cost for all contract Base Services work according to the following:

01 BASE SERVICES

A. The Contractor shall invoice the Airports Authority for Base Services at the end of each calendar month in which the work was performed and completed in accordance with the SOW, and shall be paid for actual services performed.

B. The Airports Authority reserves the right to withhold a portion of the monthly payment to the extent the Contractor has not fulfilled the requirements of the SOW for the month in which the services were performed.

02 SUPPLEMENTAL SERVICES

The Contractor will be reimbursed for the labor required to complete supplemental service tasks in accordance with the fully loaded labor rates specified in the cost schedule. The Contractor will be reimbursed for the materials used in the performance of Supplemental Services at invoice plus 10%. The Contractor shall not invoice the Airports Authority for any work unless a call order signed by the COTR has been received or for any work described in a call order until after all the work described in the call order has been completed to the satisfaction of the COTR.
