Dear Sheridan Families,
On behalf of the staff at Sheridan School, I am happy
to welcome you to the 2015-2016 school year! We are
looking forward to a productive partnership with you to
ensure our children can achieve their highest potential. We
recognize that in order to be successful in school, our children
need support from both the home and school. We know a strong
partnership with you will make a great difference in your child’s
education. As partners, we share the responsibility for our
children’s success and want you to know that we will do our very

Welcome to May! We have had an amazing year! We are
wrapping up our Dibels Assessment this first week of May
as well as celebrating our teachers and staff!
As we continue through the month of May, we continue to
focus on respect and kindness. We are encouraging our
students and support them with behavior choices. Please
continue to review our expectations: be ready, be respect-
ful, be responsible and be safe. Thank you for all of your
help and support!

My School Bucks Customers: At the end of the 2016-2017 school year the following will be done as maintenance to student accounts:
1. Any account balance valuing $0.00-$9.99 will be left in account or refunded upon request only.
2. Any account balance $10.00 and above will be automatically refunded to the parents/caregivers of senior students unless a
request is made to transfer to a sibling’s account.
3. Any student who is not a senior that has a remaining balance of $10.00 or more can be refunded upon request, transferred to
a sibling’s account, or remain in the student’s account for use the following school year.
As the end of the school year approaches, you are encouraged to be aware
of your student’s balance.

Dear Parents or Guardians:
It will not be long now, and your 5th grader will be moving on to Junior High School. In order to prevent
a delay in your registration, we strongly suggest you start planning for these requirements immediately.
Waiting until late in the summer may result in your child being excluded from school if their physical
and immunizations are not up to date. Please see the following requirements:
6th Grade physical, dated August 18, 2016 or later .
Parents should complete and sign the Medical History portion of physical.
Completed immunization record including 2nd Varicella vaccine.
T dap and Meningococcal immunizations required with 6th grade physical.
Dental exam (is not due until May 1, 2018).
If your student requires medication at school please have the
Medication Authorization Form signed by your doctor .
If your student has food allergies, please have your doctor sign the
Diet Modification form.
Forms are available in our school office or online
Immunizations are available by appointment at the McLean County Health Department, 200 W.
Front St., Room 104, Bloomington, IL, (309) 888-5455. Please make your appointment as soon as
you can.
Box Tops, Coke Rewards and Campbell’s Soup Labels
We collect these all year round so when you do have some, please
bring them into the office!
Money earned from the collection goes into buying recess items
