Some are to our benefit and others would have a detrimental effect on how we conduct our practice. Our legislative watchdog, Dr. Charlie Doring, and our lobbying team, Dan and Fran Doherty, are keeping abreast of the business in Annapolis. Below is a summary of bills impor- tant to dentistry. l SSB228/SB1082/HB1283/HB1468 – Ownership of Dental Practices. As written, with some amendments, the MSDA can support SB 228 and HB 1283 (which were identical when introduced), however, we expect the spon- sors to offer amendments that would be totally unacceptable to MSDA. MSDA strongly opposes SB 1082 as it would give total control of dental practices to nondentist equity investors. HB 1468 is the bill that MSDA strongly supports. The bill provides that only a dentist licensed in Maryland would be permitted to own and operate a dental practice. The State Board of Dental Examiners will be given expanded power to fight the unlicensed prac- tice of dentistry. HB1468 is the house bill we prefer. l SB717/HB1339 – Regulation of Radiation Machines. This bill would allow the Department of the Environment to set fees and fines and remove the grace period for corrective action to X-ray units. We fought vigilantly in 2010 to stop these unreasonable practices. This bill is a step backwards and would be very expensive and onerous to the owner of these machines. The MSDA strongly OPPOSES this bill. l SB538/HB830 – Dental Hygiene General Supervision Oral Access to Care. Under this legislation the dentist would lose his/her place as head of the dental team. Patient examination, diagnosis and treatment author- ity would be passed to the hygienist with no oversight by the dentist with only a post-operative review. As of Febru¬ary 16, 2016 this bill has been withdrawn, however, it is important to keep it in mind in the future. The MSDA strongly OPPOSES this bill. We will be sending email blasts to update you on the progress of these and other bills that affect our profession. This has been a very busy year for the introduction of bills in the legislature that affect every dentist in Maryland. Some are to our benefit and others would have a detrimental effect on how we conduct our practice. Our legislative watchdog, Dr. Charlie Doring, and our lobbying team, Dan and Fran Doherty, are keeping abreast of the business in Annapolis. Below is a summary of bills impor-tant to dentistry.

l SSB228/SB1082/HB1283/HB1468 – Ownership of Dental Practices. With Spring approaching – ie., in the dead of Winter – you and your patients will be hit with an avalanche of entreaties encouraging you to “diet.” Generally, a bad idea!

Of course any “Health Bar” is not good nutrition, nor is a chocolate type “energy-vitamin” supplement drink. In the Jan-Feb 2016 (Vol., 35) Journal of American College of Nutrition, 41-49; C.J. Rebeblo, W.D. Johnson, et al found that oatmeal (instant) eaten at

breakfast, suppressed appetite, increased satiety and reduced energy intake, compared to ready-to-eat breakfast cereal (RTEC). I think you can even get oatmeal at McDonalds. Put some blueberries (Flavonoids) and almonds or walnuts (Omega 6 Fats) on it and you’ve really got yourself a great breakfast.

Another great food is soy products. Isoflavones have been associated with lower breast cancer risks, as well as lower prostate cancer risks. Again, the whole forms are going to be better, and less risky, than a supplement form. The less processed, also, the better, the bean being the best, but a “Tofu” burger has benefits over the real thing, while providing lots of protein and no saturated fat.

In Vol. 48, #1, Jan 2016 Med Sci Sports Exerc, 132; P.W. Mumford, A.C. Tribby, et al, found a benefit of caffeine on golf performance and fatigue during a competitive tournament. Caffeine consumed before and during a round of golf improved performance and reduced fatigue in skilled golfers. I guess that lets me out – the skilled part – but caffeine mobilizes C-AMP, which acts at cellular levels and releases free fatty acid, during lipolysis, from fat cells.

Other than aerobic exercise, this is one of the few times that something extraneous “burns” fat before carbohydrate. Don’t forget coffee also has chloragenic acid, which is like a health elixar – very good substance!

Exercise (high cardiorespiratory fitness levels slow the decline in peak heart rate with age; Ozemek, Whaley, et al, Med Sci Sports Exerc. #1, 2016, 73-81) can slow the onset of age-related decline in heart rate peak in men and women – ie., slow the decline in heart fitness, which brings us back to dieting.

Usuallys when one diets, he/she restricts kcal (calories). This causes carbohydrates (CHO) stored as glycogen to be utilized first. When they are depleted, protein, through gluconeogenesis, is “burned” to provide the needed glucose for our brain, which does not utilize fat for energy. So that is all utilized before fat is burned during “dieting” by way of restricting kcals. When the inevitable crash is over, and we regain the weight – we have actually become literally fatter, ie, the percent of body fat has increased, because we have utilized our muscle mass for energy.

The way to gain fitness, look better and feel better, ie, “drop a dress size,” is to exercise daily. You may or may not lose weight but you will be healthier.

Each year, American Dental Education Association, ADEA, collects data on topics of particular interest to dental school deans, program directors, faculty, students, residents and fellows. The resulting ADEA Snapshot of Dental Education presents findings on discrete subject areas to help the ADEA membership and related stakeholders better understand the academic dental profession and its role in health and health care.

ADEA is committed to conducting research into contemporary and emerging issues that are likely to impact decisions in the dental education and policy-making communities. Each year, the information in this report is taken from data compiled by ADEA, the American Dental Association and other sources. The associated online resources are updated regularly and are available for download at: adea.org/snapshot.

Twelve Years of Giving Kids A Smile

On Friday, February 5th, for the twelfth year, the dental practice of Dr.’s Levy, Somerville, Patel and Bhushan warmly welcomed and treated 35 children as part of the annual Give Kids A Smile program. All the children received free exams, x-rays, cleanings, fluoride, oral hygiene instructions, and goodie bags filled with both oral hygiene supplies and prizes. Some of the children also received sealants, dental restorations and other needed treatment. One child will have her treatment completed in the hospital operating room at no charge. For some children, this was their first visit to a dental office. All were invited back for next year’s GKAS. F

Fixed Detachable Hybrid Restoration Surgical and Restorative Protocols

The concept of replacing a full arch of teeth with an implant supported fixed detachable restoration has become a popular patient request. Since the introduction of the “All-on-

Four” surgical and restorative protocol, there have been many modifications presented in the literature to accomplish replacement of a full arch of teeth with a fixed detachable implant-retained prosthesis utilizing anywhere from 4 to 6 or 8 dental implants per arch. Over the past 10 years my restorative colleagues and I have gained experience in restoring a full arch with the implant-retained fixed detachable prosthesis. Patients may suffer from this due to loss of the dentition related to blast injuries from the wars in Iraq and Afghanistan, as well as from tooth loss from other sources of trauma, caries and periodontal disease.

Many patients are just not tolerant of a removable prosthesis due to negative effects related to their speech, taste or instability and poor retention of the prosthesis, and desire a fixed option. The fixed detachable prosthesis in many instances provides the treatment option patients request, and resolves their complaints of an ill-fitting and unstable prosthesis. This treatment option may be the solution for the hopeless or mutilated dentition, with an opportunity for improved speech, esthetics, function and taste in patients who desire a fixed option. This presentation will discuss the treatment planning, work-up, surgical procedures and the prosthodontic considerations and steps involved in delivering the “teeth in a day” concept through the direct approach. This presentation will focus on evidence-based outcomes reported in the literature as well as survival and success rates

Immediate Molar Implant Replacement

Molar extraction due to fractures, caries or periodontal disease is unfortunately not an uncommon presentation for the restorative dentist or the endodontist. Traditionally these non-restorable

teeth have been extracted and allowed to heal or the sockets augmented with a ridge preservation bone graft for future implant placement. Another option was to attempt to place a traditional size implant into the inter-septal bone with an attempt to achieve primary stability. This is frequently an unpredictable procedure with implants placed in less than ideal positions for the restoration or with implants being placed too deep or too shallow.

There are now molar sized implants (7-9mm in diameter) that are designed to be placed at the time of molar extraction that will occupy most of the socket and achieve a very high degree of primary stability and will allow for ideal emergence profile for the restoration. This presentation will discuss the advantages and disadvantages of immediate molar implant replacement and a technique that can provide a predictable outcome with a high degree of patient satisfaction. The surgical protocol regarding the molar extraction and immediate implant placement will be discussed in detail and examples of various molar relationships to the nerve and sinus will be reviewed utilizing clinical cases. The results of our evidence-based clinical study on immediate molar replacement implants will be disclosed and will serve as the basis for the success of this technique.

Rendering Care to Patients Prior to Arrival of EMS Respondents

For nearly fifty years, we have been encouraged not to get involved in times of crisis, but to call 911 and wait for assistance by trained professional

responders. While this has been successful in reducing death, there is a period of opportunity between recognizing a threat until the arrival of first responders. What is often only minutes, provides lay rescuers a chance to render aid and reduce or mitigate the harm. Citizens willing to self-rescue or to assist others until the arrival of responders serve as the necessary bridge that spans the time from injury to definitive care. This presentation will discuss the important role of the citizen responder in reducing potentially preventable death and disability in the immediate aftermath of an active shooter or terrorist event. It will describe the common behavioral response by unprepared persons confronted with an immediate threat of harm and describe mental preparation and training that can improve your ability to act effectively and improve your changes for surviving, and helping others survive, such an event.

The first of two big yellow school buses arrived with children from northern Carroll County Elementary schools on February 5th as Mt. Airy Children’s Dental Associates hosted their 14th Annual Give Kids A Smile Day.

The first Friday in February is saved for this very special day and to make sure there are no conflicts, it is marked twelve months in advance.

Pediatric dentists Drs. Dave Hasson, Allison Green, Nate Shapiro and Mike Virts with 10 volunteer hygienists and assistants screened and cared for 51 children. Donna Pope and Charmaine Amoss, both have been with the practice for 18 years, managed a mountain of treatment permissions and treatment plans for the children. Alison Reiter, RDH who has been with the practice for over 20 years, was in charge of “Dino” the dinosaur puppet for diet and oral hygiene instructions, while the dental hygiene and pediatric assistant volunteers took over clinical duties.

“What we are most proud of with our participation in GKAS is that it’s not just a day for screening . . . all who have needs will be followed through to completion, and the doctors and staff of our practice and some of our colleagues in and around Mt. Airy help us deliver restorative, preventive and surgical care,” said Dr. Hasson. This year, 29 of the 51 children needed comprehensive care, of which, 3 were urgent.

“Give Kids A Smile Day brings a smile not only to the kids, but to all members of our office who volunteer. As soon as the school bus pulls into the parking lot, we love running out to meet the kids, bring them inside and start brushing their teeth. I love hearing from the kids how this is their first dental visit and how they had so much fun,” said Dr. Green.

The annual event, promoted by the American Dental Association, calls attention to unmet dental needs of children throughout the country. Thousands of dentists and their team members provide free oral health care services to children in need. “It was incredible to see the effort from our whole team and the participating schools in bringing oral health education and much-needed care to these children. It felt so rewarding to help so many kids in need,” said pediatric dentist Dr. Nate Shapiro. “I feel like we are making a big difference for them.” This was Dr. Shapiro’s first time volunteering for GKAS.

The Mt. Airy pediatric dental practice was thankful for a perfect-weather-day. “We’d been blown out by several snow and ice storms in our 14 years with GKAS, causing us to reschedule many times. We pretty much got it down to a science... 51 kids in 4 hours... that might be our all-time record!” explained Dr. Hasson.

The kids were transported by bus from Carroll schools in two separate groups accompanied by pupil personnel workers, teachers, guidance counselors and an assistant principal from Winfield Elementary School.

A family of 5 children was brought to the office by Frederick County Elementary School Community Liaison Dede Pew. Ms. Pew has helped coordinate and transport hundreds of kids from Frederick County over the years. “Dede has been with us since the first year. Her experience with Frederick County schools combined with her incredible dedication has forged the way for us to work with Carroll County schools who participate in our annual program,” said business associate Donna Pope.

The children most needy were identified by school nurses. Teachers and guidance counselors then passed their parents information about the program. Pediatric dentist Dr. Mike Virts said, “As pediatric health care professionals, we should feel personally obligated to give of ourselves when the need is the greatest. Through our participation in Give Kids A Smile Day, year after year, we are giving these local, deserving children the same care we give our own families. I hope that our efforts are not only playing a role in improving the children’s health and self-esteem, but also motivating other professionals to realize the importance of participating in future events.” F

About one half of the population has periodontitis ranging from slowly progressing destruction to rapid loss of periodontal support. The most susceptible individuals continue to lose support with either of these treatment modalities. Properly applying the principles of infectious disease control as developed for other infectious diseases can stop

periodontal destruction, and probably reduce or arrest systemic disease effects caused by periodontitis. Periodontal pathogenic microorganisms must be identified, monitored and effectively treated before, during and just as important, active periodontal treatment to ensure disease control. This course presents effective methods for infection monitoring and treatment of pathogenic periodontal infections by dental professionals, as well as strategies to reliably control supra- and subgingival-biofilms via patient home care procedures. License Renewal Guidelines: The Maryland State Board of Dental Examiners now requires that ALL 30 CE credits must be obtained by December 31 of the year PRIOR to the license expiring.
