Thehoneymoon isover. In fact thereis no honeymoon at all and no restfor thewicked.This jobdescription issimply hard work and responsibility.I remembered a few years back ourimmediatepastPresidentDr.RubyLeehad mentioned to me the love andcarefromothers.AtthetimemyinitialthoughtwaseverybodylovedtheprettyRuby! Now the ugly Angus has thesamefeelingwithallthecongratulationmessagesfromhomeandabroad.Theloveandcareoriginatesfromeverybodywho wishes the College and FamilyMedicinetogoforwardandtostriveforthebest.
Myfirstoutingwasto“yumcha”atSheungWanMaxim’sPalaceforthefirstmeetingoftheHKCFPPhotographyClub.Thebigtablewasfilledupwithmembersandofcoursedimsum.ThankyoutoDr.LoLingandDr.DavidChaofororganizingthisactivity.Duringthechitchatcameramodels, lensadaptors,andlensesallcameintothediscussion.Dr.LauHoLimisthephotographyencyclopedia.Hecouldtellyouanythingandeverythingaboutphototakingandcameraprice. Iwouldnotwastemytimetogoogleanymore.Everyoneshowedofftheir“cannons”andsoondisappearedintothemistofseafoodstallsandtemplesinSheungWan.Ifyouhaveanyhobbythatcanbepromotedtomembersintheirleisure,pleaseletmeknow.
Ihadavery interestingdinnerwithDr.ChowPakChin,PresidentoftheHongKongCollegeofOphthalmologistsandhisCouncilmembers.Dr. Chow expressed his viewpoints on the interaction betweenophthalmologistsandoptometrists.HisCouncil isverykeenfor familydoctorstoengagemoreinprimaryeyecare.TheyarealsolookingintothepossibilityofFMtraineesattachedtoprivateophthalmologists forlearningsessions.WedofeelthiscanbethefirstofmanyopportunitiesforcollaborationbetweendifferentCollegestoenhancetheir trainees’experience.
On9thJanuary2015,Drs.LauHoLim,DavidChao,BillyChiu,Ms.EricaSoandIattendedabriefingonElectronicHealthRecord (eHR)byMr.SidneyChan,Head (eHR)andDr.KMChoy,Consultant (Private-PublicPartnership)of theFoodandHealthBureau.TheyupdatedusoneHRBill legislation,majorconcernsonpatientchoice,privacyprotectionandrecentdevelopmentonsafedepositboxforpatientdata.This isagreat leapforwardcomparedtowhatwehadheardtwoyearsago. Itsfirstphaseof implementation is likely tocommenceat theendof thisyearorbeginningofnextyear.Thereisnofinancialincentive,softwareorhardwaresupportfromthegovernment.However,forgoodpatientcare,thereisfoodforthoughtforeverypracticingdoctortoparticipate.Pleaselookupmoredetailfromitsofficialwebsite:http://www.ehealth.gov.hk/

He advocates a voluntary accreditation course,possiblylastingfortwoyearstoequipfreshgraduates.Dr. Donald Li, President of Hong Kong Academyof Medicine and the ex-President of our Collegehighlightstheneedtotrainthosewhodecideagainstpostgraduatestudies,but infavourofstartingaclinicwhilestill inexperienced.Their lackofexperienceandtrainingposesaproblem toqualityandefficiencyinprimarycare. I cannotagreemorewithDonald!Mandatory training forprimarycaredoctors/ familyphysicians is an unequivocal universal trend tosafeguardnot just theknowledgebut thequality,standardandattitudeof themostvaluableassetofanyhealthcaresystem,TheFamilyDoctors.ForcertaintherewillbeongoingdiscussionamongstthedifferentstakeholdersandtheMedicalCouncilofHongKong.

The37thAnnualGeneralMeeting(AGM)and37thAnnualDinneroftheHongKongCollegeofFamilyPhysiciansweresuccessfullyheldon13thDecember2014(Saturday)attheRunRunShawHalloftheHongKongAcademyofMedicine,thankstothegreatsupportbyallourparticipatingmembersandfellows.OurnewPresident,Dr.AngusChanledtheExecutives,CouncilandCollegememberskick-startedtheeveningwiththeAGMgoingthroughthevariousoperationalmattersrelatedtotheCollege.AbriefCouncilmeetingwasconductedtoconfirmthechairmanshipofvariousboardsandcommitteesforthecomingyear.
TheAnnualDinnercommencedrightafterthat.WeweremostgratefultohaveDr.MonicaWong-HeadofPrimaryCareOffice,DepartmentofHealth,HongKongSAR,Dr.TsangManWo,specialistinEndocrinology,Diabetes&Metabolism,andMr.AndrewLaufromtheSunTakDistrictMinYuenTongofHongKongjoiningusforthedinner.

Variousentertaininggamesincludingaphonegameandbingowereconducted,followedbythemarvelloussingingperformancesbyDr.KennyKung,Dr.ChanHungChiu,PeterandDr.YeungWaiMan,Raymond.ForthefirsttimeinthehistoryofourCollegefunctions,wewereprivilegedtohaveDr.MayLamandherhusband,Mr.BernardYimtoperformanelegantflamencodanceandguitarplayforus.Thankstothethoughtfulgenerositiesofourcouncilmembers,friendsandrelatedorganisations,theluckydrawwascarriedoutsuccessfullytoconcludetheannualdinneronahighnote.Wewouldliketotakethisopportunitytothankallthecolleagueswhohavehelpedtomaketheeveningaveryenjoyableoccasionagain.

The HKCFP Award for the Best Research of 2014
TheResearchCommitteeoftheHongKongCollegeofFamilyPhysiciansiscallingforTheAwardfor
TheBestResearchoftheYear2014.AllmembersandfellowsoftheCollegeareinvitedtoparticipate
andsubmittheirresearchpaperstotheResearchCommitteeforselection.TheAwardwillbepresented
attheConfermentCeremonyin2015.
Entry and assessment criteria are listed below:
Entry Criteria:
1. Theprincipal investigatorhas tobeaMemberoraFellowof theHongKongCollegeof
FamilyPhysicians.
2. Theresearchmustbeoriginalworkoftheinvestigator(s).
3. TheresearchshouldbeconductedinHongKong.
4. Theresearchmusthavebeencompleted.
5. Thepaper shouldbepresentedunder the standardheadingsofAbstract, Introduction,
Methodology,Results,DiscussionandConclusion.Referencesshouldbelistedinfullatthe
endinVancouverformat.
Assessment Criteria:
1. HowrelevantarethetopicandfindingstoFamilyMedicine?
2. Howoriginalistheresearch?
3. Howwellistheresearchdesignedinthemethodology?
4. Howwellaretheresultsanalyzedandpresented?
5. Howappropriatearethediscussionandconclusion(s)drawn?

HKCFP Research Fellowship 2015
Introduction
TheHKCFPResearchFellowshipwasestablishedbytheHongKongCollegeofFamilyPhysicianstopromoteresearch inFamilyMedicine.TheGrant isup to thevalueofHK$100,000.ApplicantsareexpectedtohaveregularcontactwithanominatedsupervisorwithMasterorequivalentdegreeorabove.
Eligibility
Applicants for theHKCFPResearchFellowshipmustbeactiveFellow,FullmemberorAssociateMemberoftheHKCFP.Newandemergingresearchersareparticularlyencouragedtoapply.However,full-timeacademicstaffofUniversitieswouldnotbeeligibletoapply.
Selection criteria
Applicationswillbejudgedon*:•Trainingpotentialofapplicants•RelevancetoFamilyMedicineandcommunityhealth•Quality•Valueformoney•Completeness(incompleteorlateapplicationswillnotbeassessedfurther)

HKCFP Research Seed Fund 2015
TheResearchCommitteeofHKCFPisproudtocontinuetoofferthetworesearchfunds,TheTrainees
ResearchFundandtheResearchSeedFund.
TheTraineesResearchFundwill beopened to all registeredHKCFP trainees and ismadeof four
awards(eachuptoHK$5,000).Itisenvisageditwillhelptraineesespecially(butnotlimitedto)those
doingresearchprojectsastheirexitexamination.Thosewhohavefundingsupportelsewherewillnotbe
considered.
TheResearchSeedFundisopentoallHKCFPmemberswhenamaximumof$10,000awardwillbe
madetothesuccessfulapplicanttoassisttheconductionofaresearchproject.
Winners of the awardwill receive50%of the approvedgrant up front and the remainder 50%upon
completionoftheproject.
Assessment Criteria for both funds:
1. Academicrigorofthepaper(e.g.originality,methodology,organisationandpresentation);
2. Relevanceandimpacttofamilymedicine&primarycare(e.g.importanceofthetopicand
theimpactofthefindingsonthepracticeordevelopmentofthediscipline);and
3. Overallbudget

A crow lived in the forest and was absolutely satisfied in life. But one day he saw a swan. “This swan
is so white,” he thought, “and I am so black. This swan must be the happiest bird in the world.”
He expressed his thoughts to the swan. “Actually,” the swan replied, “I was feeling that I was the hap-
piest bird around until I saw a parrot, which has two colors. I now think the parrot is the happiest bird
in creation.” The crow then approached the parrot. The parrot explained, “I lived a very happy life until
I saw a peacock. I have only two colors, but the peacock has multiple colors.”
The crow then visited a peacock in the zoo and saw that hundreds of people had gathered to see him.
After the people had left, the crow approached the peacock. “Dear peacock,” the crow said, “you are
so beautiful. Every day thousands of people come to see you. When people see me, they immediately
shoo me away. I think you are the happiest bird on the planet.”
The peacock replied, “I always thought that I was the most beautiful and happy bird on the planet. But
because of my beauty, I am entrapped in this zoo. I have examined the zoo very carefully, and I have
realized that the crow is the only bird not kept in a cage. So for past few days I have been thinking that

Weactuallyhad lesscommunicationwhenKevinwasamedicalstudentandIdidn'tpreachFamilyMedicinetohim.WebecamemuchcloserwhenKevinstartedtopracticeFamilyMedicineandweshareddifficultiesduringdailypracticeandconsultations.OurcommoninterestinFamilyMedicinehasbroughtuscloserasafatherandsonandhelpedtakenourrelationshiptoanotherlevel.
Dr. Stephen Foo, we know that one of your greatest challenges of your time was to establish the HKCFP and negotiate specialty recognition for family medicine. What do you perceive the greatest challenges for young and aspiring family doctors such as Kevin in the 21st century?
Dr. Stephen Foo: One of the greatest challenges issettingupone’sownprivateclinicandmaintainingone’sautonomyasanindependentdoctorincurrenteconomicconditions.Rental inHongKonghasbeenescalatingfastandcompetitionisprogressivelyfierce.Thismakesitmuchhardertorunone’sownindividualpractice.Oneoftheoptionsisservingasamedicalofficerinpublicoraresidentmedicalofficerinprivatehospitals.Toovercomethesechallenges,oneruleofthumbistoadheretothefundamentalprinciplesof familymedicinewhichareneverwrongirrespectiveofdifferencesingeographyortime–interpersonalcontact,care,concern,empathyandrapport.Heedlessofthechallengesinthe21stcentury,youngFamilyPhysicianshavetorememberthatourdutiesaretoourpatientsandweshouldconsidertheirproblemsfrom their stand points. Dr. Stephen Foo, how did you encourage Kevin to follow your footsteps to become not only a doctor but also a family physician?
Dr.StephenFoo:MywifeandIneverpushedourchildrenintheiracademicstudies.Myself,Iwasverybusywithmyclinicandworkandusedtogetoffworklateeverynightat9pm.KevinwasonlyinprimaryschoolthenandheusedtofinishhishomeworkonhisownoppositemeonmystudytablewhilstIwaspreparingdocumentsfortheCollege.
I think itwasasubconscious influence forhimandhebecameinterestedinmedicineaftersecondaryschoolonhisownaccord.FamilyMedicinewasalsohisownchoice.KevinwasactuallytheBestHousemanofYearinSurgeryandIwasthinkingmaybethiskidwantedtobeasurgeon.ButitturnedouthisfirstandonlychoicewasFamilyMedicine.
Kevin what were your inspirations to become a Family Physician?

(仁心仁術)BenevolenceappliessomuchmoretoFamilyMedicinethanotherbranchesofmedicine.Ialwaystellmytraineesnottobedisheartenedbytherelatively less incomeinourspecialty.Weare inamuchbetterpositiontogainrespectfromourpatients.Theworldischanging.FamilyMedicineisprogressingfast.It isgettingmoreuniversalandacceptable. In fact,asDirectorof theDiplomaofFamilyMedicineformorethanadecade,Ihavewitnessedmany other specialists apply for the diploma oncetheycontemplatestarting theirownpractice to learncommunicationandotherGPskills.
How do you perceive these challenges Kevin?
Dr.KevinFoo:Itotallyagreewithmyfather.Wecanonlyovercomethesechallengesbyprotectingourcorevalues.Therearemanyexternalfactorsthatarebeyondourcontrolandourpracticeshouldnotbegovernedbymonetaryrepaymentalone.Public awarenessand recognitionofFamilyMedicineasaspecialtyby itsownmerits isachallenge.ButIbelieveaslongaswestandfirmandfulfillourownroles,positivechangeswillgraduallytakeplacewiththehelpofgovernment’spolicytopromoteFamilyMedicine,privatehospitalstoestablishFamilyMedicineCentresandgradualpublicrecognition.WehavetoplaceconfidenceinFamilyMedicineasittrulyisaself-sustainablesystemandisbeneficialtoourhealthcaresystem.
AnotherchallengeisourcurrenttrainingsystemwithinthepublicsettingwherewecannotgenuinelypracticeFamilyMedicine.MyfatherhasbeenagreattrainerasaprivateFamilyPhysicianpassingonhislifeexperiencetohismentees. Thesystemnowisteachingtraineeshowtodispenseacasewithin3-5minutesandwecanhardlytreatourpatientswithcareandconcern.Ifoundthistypeoftrainingnotchallengingandmonotonousforme.ButIdoknowthatmanypartiesareworkingtoimprovethiscondition.
What suggestions do you have for the future of Family Medicine in Hong Kong?
Dr.KevinFoo: I thinkmoreemphasiscanbeplacedontraining in theprivatesectorwhere traineescankeepincontactwithamentorwhoisanexperiencedFamilyPhysician.FamilyMedicinetrainingintheprivatesectorismorelivelyandappealingandtraineesneedtohaveaglimpseofotherchoicestheyhaveapart fromworkingin thepublicsector.Undergraduatetraining isalsoanareawhichneedstobeworkedon. I think ithasmuchimprovedalreadysincewhenIwasamedicalstudent.ItakemedicalstudentsmyselfandtheyareintroducedFamilyMedicinesinceYear1ratherthanjustarotationduringYear4.UndergraduateteachinghasdefinitelyplacedmoreemphasisonFamilyMedicine. I thinktheycanbenefitmorefrommorefrequentvisitstoprivateGPs.Hencemoretrainersarerequiredintheprivatesector.
Dr.StephenFoo: Itwouldactuallybeverydesirabletoincorporateacertainperiodoftraininginprivateforourtraineesforexample3months.Butalotofdifficultiesareanticipated,likelimitationofresources.Trainersneedtobepaidtodothejobwell.Protectedtimehastobeassignedtotraineesapartfromservicing.KevinandIoftendiscusshowprimarypublicservicessuchasGOPDsandthevariousdifferencesamongst thepublic/privatesectorcanbeimproved.Ithinkthefrustrationsourtraineeshave,theyarenotabletopracticeinthepublicsettingswhattheyhavelearntinthetrainingperiod.
Dr.KevinFoo:A lotofcasesdealtwith inourconjointexaminationsarenotcasesoftenseeninGOPDsettingssuchastravelmedicine,HIVpostexposureprophylaxis,casespresentingwithpyelonephritis,acutefracturecasesrequiringurgentX-rayswhicharenotreadilyavailableinGOPDs.Afterdiscussingwithmyfather,Irecognizethattheseareveryrealcaseswhichareoftenencounteredintheprivatesector.IcametorealizethatIwasnotabletopracticewhatIhadlearntintheGOPDsettingandthatwaswhyIdecidedtoacquirefurthertrainingintheprivatesector.IthinkthefactmyfatherisanexperiencedprivateFamilyPhysicianhashelpedmemakethedecisiontobetrainedinprivateasIwasmoreinformedabouttheprivatemarket.
Dr.StephenFoo:Iinitiallydidn’tknowKevinhaddecidedtopracticeintheprivatesector.NowheisintheHongKongSanatorium&HospitalFamilyMedicineTrainingCentre,I thinkhehasmore learningopportunitiesandbetteranalyticaltrainingforhisdecisionmaking.
Dr. Stephen Foo, you are socially very active, professionally you hold many posts, you also exercise regularly including playing badminton as many people know, yet you have a remarkably close relationship with your family and children. How do you balance your work and family?
Dr.StephenFoo:IhavebeenveryfortunateandIowealottomywifeMariawhohasbeenunconditionallysupportiveandtoleranttomywork.Mywifealwayssaysmyrealwifeis theCollege. Iusedtofinishclinicat9o’clock intheeveningandcontinuetoperformCollegedutiesafterwork.I remember in thenonComputerAgewhenwedidn'thaveemailsandIwouldbebusyfaxingdocumentstotheCollegesecretariesat2amforpreparationoftheupcomingmeetings.Mywifetookupthemainburdentocarefortheupbringingofourfourchildren.

Kevin, how do you balance work, family and health?
Dr. Kevin Foo: One of the core values emphasizedin Family Medicine is the unity of family members.Throughoutmyupbringing,familyhasalwaysbeenveryimportanttome.Geographically,Iliveclosetomyparentssothismakes iteasytospendtimewiththem. Ispendqualitytimewithmywifeduringweekends.AlsoImakeitahabittoschedulemytimeforregularexercise–Ithinkitisimportanttopracticewhatwepreach.
Dr.StephenFoo:Sportshasalsobeenan importantaspect inmy lifeevensincesecondaryschool. Iwasasportscaptain inWahYanCollege, representing theschool invariousinterschoolsportcompetitions.Sportsandregularexerciseprovideatimeformetorelaxandrecuperate.Manypeoplecomplaintheydon’thavetimeforexercise,butmostofthetimeitisaboutself-discipline,timecontrolandprioritization.Inowhavedancingclassesthreetimesaweekwithmywife.Kevinalsolearnthowtowaltzwithussometimes.
Dr.KevinFoo:Ihavebeenmuchinfluencedbymyfather’spassionforsports.MywifeandIarenowlearningtennistogether. Ithinkwecancombinefamilytimeandsportstogethertobecomea“healthyfamily”.
Dr. Stephen Foo, you have much experience having held many important posts in the medical field and in our College in the past 30 years. How do you share this plethora of experience with Kevin?
Dr.StephenFoo: IsharemyexperiencewithKevin inaveryliteralsense–IusedtotakehimtoCollegeeventssuchasNewYeargatheringsandmeetdifferentpeopleinthemedicalfield.Throughourdiscussions,hegraduallybegan to realizewhatbeingadoctormeantand toascertaintheroleofaFamilyPhysician.AlsoIthinkKevinhasdifferentperceptions thathehas toexperiencethroughvarious levelsofhisprofessionalcareer. Forinstance,hisbeingamemberoftheorganizingcommitteeoftheHongKongPrimaryCareConference(HKPCC)isaverygoodexperienceforhim.
Dr. Stephen Foo, you have trained up many Family Specialists in the past. How do you train Kevin?
Dr. Stephen Foo: To be honest, I never train himpurposefullyorsystematically.ThebesttrainingIofferedhimwastobearolemodelmyself. Ibelieve it isnotamatterofshowingKevinhowthingsshouldbedonebuttoallowhimchanceandexperiencetoexplorebyhimself.Wedidnothaveanydeliberatetrainingsessionstogetherbut Iwouldsharemycasesandexperiencewithhim
whentheoccasionraises.AlotalsodependedonKevin’sownobservationand initiatives forself improvement.MywifeandIdidnotbelieveourchildrenwouldlearnbyreprimandingthem.Thebestmethodtotrainchildrenand indeedtotrainupasuccessfulFamilyPhysician istomotivate them. Throughouthis training,mymainemphasistoKevinwastofocusonpatientsatisfactionandnotfinancialrewards.KevinandIalsohaveanevolvingrelationshipashematuresprofessionally.Nowwealsolearnfromeachother.
Kevin, what was the most memorable experience you have with your father in the medical field? Do you share cases with your father?
Dr.KevinFoo:It’sverytruewhenmyfathermentionedourrelationship isevolving.WhenIwasamedicalstudent,Ididn'tcommunicateasmuchwithmyfathermedically.AfterstartingmytrainingasaFamilyPhysician,IfoundIhadmuchmorecommongroundwithmyfatherandsharedthedifficultiesIencounteredasaFamilyPhysician.Beinginthesamespecialtyhasdefinitelybroughtusclosertogether.Weenjoya lotofcasesharingespeciallysince IhavebecomeaFamilyPhysicianintheprivatesectorastherearesimilaritiesinpatients’diseasepatternsandexpectationsinmanagement.Basicallythereisnogenerationgapbetweenusandwebothbenefitfromcasediscussions.
Dr. Stephen Foo, what life motto do you want to pass on to Kevin?
Dr.StephenFoo:There isnodistinctmotto Ineedtopasson.Kevinneedstodevelophisownpersonalvaluesthroughthevirtueofhisownwisdomashematures. IonlywanttopassontheattitudeIholddearinmylifeandprofession.Toputitsimply,IwouldbeaveryhappyfatherseeingKevinbecomingagoodfamilydoctor.Ibelievebybeingagoodrolemodelisgoingtobethebestinfluenceonthenextgeneration.
Kevin, what do you think is the greatest legacy from your father?
Dr.KevinFoo: I thinkthemost importantthingI learntfrommyfather ishissincerityandpassion inhiswork.Hisdevotionandsensitivitytohispatients isaconstantremindertomethatourprofession’stoppriorityispatientcareandsatisfaction insteadofa financial return.Themedicalprofessionnowadays ismoreprofitdrivenwithaneglect in familymedicineprinciples.My fatherhasenjoyedan immenselysatisfactoryrelationshipwithhispatients.IrememberhimreceivingmanyChristmascardsandgifts fromgratefulpatients. AnotherthingI learntfrommyfatherisdiligenceandthecontinuouscraveforlearning.My fatheraccumulates100-200CMEpointsannually throughmultiple coursesand lectures.Weessentiallyhavenogenerationgapasmyfatherisatthetopofthegamewithceaselessmedicalupdates.Apartfromhisownlearning,heisalsoceaselesslypassingonhisknowledgeandexperiencetothenextgenerationandtrainingFamilyPhysicians.Thishasalso inspiredmetotakemedicalstudentsandpassonmyownknowledge.

Exercise, should we adjust our target for a fully sedentary patient?
The19theditionofStandardCharteredHongKongMarathonheldon25thJanuary2015attractedover70000participants.Inadditiontothefullmarathon,a10kmrunandahalfmarathonwerealsoheld.Marathonisdefinitelyagoodchoiceofexercisebutrequirescertaindegreeoftrainingandpractice.Currentguidelinessuggested150minutesormoreofmoderateactivityeachweek,butitisquiteoftenthatourpatientsfinditdifficulttoengageinlongexerciseafterbusywork.Ifwereducethetargetexercisetime,isitstillbeneficial?OnJanuary21,therearetwoarticlespublishedintheBMJthatdiscussedtheabovequestions.
Inthefirstarticle,DrBarretoarguedthat,toreduceprematuremortality,policiesshouldfocusongettingfullyinactivepeopletodoa littlephysicalactivity ratherthanstrivingfor theentirepopulationtomeetcurrentphysicalactivityrecommendations.
Inthesecondarticle,DrSparlingcommentedthatolderadultsfinditdifficulttomeetmoderateandvigorousexercise targets.Given thatadose-responseexists forphysicalactivityandhealthbenefits,achange inmessagetoreducesedentarytimeandincreaselightactivitiesmayprovetobemorerealisticandpavethewaytomoreintensiveexercise.
Inseven large longitudinal,observational studiesDrBarretoexamined, the researchersshowedthat thegreatestdifferencesinmortalityriskwerebetweenthemostsedentaryandtheslightlymoreactive,“suggestingthatpeoplewhoaresedentarycouldhavetheirriskofallcausemortalitygreatlyreducedwithrelativelyminorincrementsinphysicalactivity.”Allbutoneofthesevenstudiesshowedthatpeoplejudged“somewhatactive”wereatalowermortalityriskcomparedwithinactivepeople,andtheriskreductionsforthe“somewhatactive”grouprangingfrom14%to37%acrossthestudies.
InastudyconductedinNorwayamong56,072people,thosewhoengagedinasingleweeklyexercisesessionhadlowercardiovascularmortalitythaninactivepeople,leadingtoarelativeriskinmenof0.71(95%confidenceinterval,0.59-0.86)comparedwithinactivemen.Inwomen,thesinglesessiondroppedtherelativeriskevenmoredramatically,to0.56(95%confidenceinterval,0.44-0.71).
“Smallincrementalincreasesinphysicalactivityshouldbepromotedinaslowlyprogressivemannertomaximisehealthbenefitsandminimisepotentialadverseeffects,”DrBarretowrote.
DrSparlingadvisedthatexerciserecommendationsshouldfocusonintroducinglightactivitythroughouttheday, increasinglightactivitiesby30minutesdaily,andreducingprolongedsittingbystandingorstrolling1or2minuteseveryhour.Peoplecouldbeadvisedtogetupfromtheirchairduringcommercials,paceduringtelephoneconversations,andtake5-minutewalksthreetimesaday.
“Wearenotproposingthatthe150minuteaweekstandardbeabandoned.Rather,ourpurposeistoremindcolleaguesthatabroadperspectivetocounselling isalreadyembeddedintheguidelinesandthatawholedayapproachforoldersedentarypatientsmayhelpthemmovetowardstherecommendedactivitylevels,”DrSparlingaddressed.

Lifecycleofhairsconsistsof3phases:Anagenphase,CatagenphaseandTelogenphase.Anagenphaseisthegrowingphase.Itusuallylastsfor3yearsandincludes85%ofallhairs.Catagenphasephaseistheinvolutionphasewhichlastsforonly3weeks(only1%ofhairs).Afterwardshairsgointothetelogenphasewhichistherestingstage.Hairsareshedafter3months.Normalanagentotelogenratioisaround6:1(85%:14%).
Importantquestionsinapproachingalopecia:1. Generalizedorlocalizedtypeofalopecia?2. Scarringornon-scarringtypeofalopecia?3. Otherabnormalsignse.g.erythema, lichenification,
scaling…
Thefirstcasewasamiddle-agedmanwithscalppruritusandgradualonsetofapatchofalopecia.Hewasworriedaboutalopeciaareata(AA).Physicalexaminationshowedlocalizedandnon-scarringlesionwitherythema,lichenificationandill-definedmargin.ThediagnosiswaschronicdermatitisratherthanalopeciaareatasinceAAarenon-inflammed,non-scalywithwell-definedborders.Exclamationmarkhairsarehairswhichlookedlikeexclamationmarks.TheyareindicativeofactivehairlossinAA.
Thesecondcasewasachildwithapatchoflocalizedandnon-scarringalopecia.Thelesionwaswell-definedwitherythema,whitescalesandbrokenhairsatcentre.ThediagnosiswastineacapitiswhichcouldbeconfirmedbyWood’slight,directmicroscopyandfungalculture.Topicalantifugal(azolegroup)couldbeusedformildcases.
Thethirdcasewaslocalizednon-scarringalopeciaagainbutthelesionwasnon-inflammedwith irregularborderandbrokenhairsamong longernormalones---typicaloftrichotillomaniawhichisaformofalopeciacausedbyintentionalorunintentionalpullingofhairs. Inchildrenit isoftenatemporaryhabittic.Counsellingisthemostimportantaspectofmanagement.
Localizedscarringtypesofalopeciaare lesscommonthanthenon-scarringtypes.Morpheaanddiscoidlupuserythematosuswerediscussed.
Generalizedalopeciacanalsobedividedintonon-scarringandscarringtypes.Commoncausesofnon-scarringtypes:telogeneffluvium,androgeneticalopecia(AGA),alopecia

totalis,systemicdiseases.Scarringtypecanbeduetoburnortrauma.
Causesoftelogeneffluvium:childbirth,febrileillness/majoroperation,starvationdiet,metabolic(hypothyroidism,irondeficiency),emotionalstress,drugs,chronicsystemicdiseases.Telogeneffluviumusuallyoccurs3-4monthsafter thetriggeringeventbecausetheeventcausessuddenconversionoflargeamountofhairsfromanagentotelogenphase.Thesetelogenhairsremainonscalpfor3monthsandthentheydropoffalltogether.Spontaneousrecoveryhappensinmostcasessocounsellingcansettlepatients’anxiety.
Androgeneticalopecia(AGA)showstypicalsigns:anteriorhair-linerecession,thinningofhairsatvertex,normalhairdensityatsidesandback,normallookingscalpandhairs.
