It involves 5 steps: 1) thinking about your values, wishes & beliefs; 2) learning about different medical procedures & what they can and can’t do; 3) deciding who will be your substitute decision- maker; 4) talking about your wishes with your loved ones & your doctor; 5) record- ing (writing down) your wishes.
