Include the following information when ordering:

• Catalog product number(s).• For all orders which require typesetting or engraving, complete the attached form and mail or fax with your order form. This step in procedure is critical to producing your order correctly and on time. • Shipping and billing information along with your customer number.• Date products are needed.• Name and telephone number of person placing order.• For trophies or plaques, indicate the figurine desired.• Any artwork desired on an award can be supplied to MTM, or choose from a wide variety of stock art. Laser prints, faxes or copies are not camera-ready, and will require a minimum art charge of $45.00 to ready the artwork for production. The type of file required for the method of production will vary from order to order.

How to supply your artworkMTM prefers a vector EPS file for our manufacturing processes. Generally, this is an attachment of the artwork in the original application that it was created in. This can be obtained via the artist who created it, your HR Department, IT Department or Design Department. If none of these are available to you, we suggest contacting a Customer Care Associate for more information.

Send artwork via e-mail, DVD or thumb drive. PC is the preferred format. All artwork is subject to production approval. Non-electronic art should be “camera–ready". The art department will determine if artwork is camera– ready. Artwork may be altered for production processes. Include all font information as well as any PMS colors used. Fonts should be converted to outlines or points.

Electronic Art Logos and GraphicsWe require vector artwork for production in the form of Encapsulated PostScript (EPS) files, created in Adobe Illustrator. Halftones / photographs — We require images of 300dpi or greater in JPEG, TIFF, GIF or PSD format. Text — an Excel file is the preferred format. Please use 3 columns for names (First, Middle, Last). Word is also an acceptable format.

Camera-Ready ArtMTM requires black and white artwork with very clean and crisp edges. (No fades, blends, percentages or halftones) Supplied art should be at the highest possible quality for reproduction. Logos and graphics will have to be re-drawn in a vector format (this process will incur an art charge). Text sent in this format may have to be reset (this process will incur a typesetting charge). Contact your CCA for additional information on acceptable file formats.

CancellationsOrders cancelled during production will be charged for the work completed at the time of cancellation.

Voice MailTo check the status of an order, call our customer service department at 800-324-5997 x6899. MTM has a voice mail system for your convenience. If you reach your Customer Care Associate’s extension and find it busy or your Customer Care Associate is unavailable, please leave a detailed message including your phone number and any specific information desired. Your Customer Care Associate will return your call.

WarrantyAll products sold by MTM are guaranteed to be of good quality materials and workmanship. If our quality inspectors overlooked a factory defect, write or call your Customer Care Associate within ten days after delivery and we will issue a Returned Goods Authorization (RGA) number for return. All shortages in shipment must be reported within 10 days of delivery to receive credit replacement.

Drop Ship ServiceMTM can economically provide drop shipment service and/or custom packaging of products other than our standard pack. Call or write with your specific details and a price will be quoted.

ReturnsA Return Authorization (RA) number must be issuedfor all returns. Merchandise without a RA will not be accepted. Custom orders may not be returned unless a factory defect exists. A 20% restocking fee applies to all returns.

Payment InformationCREDIT TERMSAll orders must be prepaid cash, check, or credit card. If you plan to charge to VISA, MasterCard, American Express, or Discover, please have your card number and expiration date available when placing your order.

Method of PaymentCREDIT CARD: To place an order using a credit card we require the type of card, card number, name as it appears on the card, expiration date and home phone number. Please place customer account number on all correspondence. CHECK: Make payable to MTM Recognition and be sure to include shipping and handling. Add applicable sales tax. Shipping Information• Merchandise will be shipped via ground whereweight limits permit. Unless otherwise specified, shipping will be F.O.B. factory. MTM is not responsible for delays or damage in transit.

Prepaid Shipments• Priority shipping is available; however, customer is responsible for all expedited shipping charges. Ground shipping is free on orders that are more than $75.

Prepaid Orders• Prepaid orders must follow the shipping schedule on the attached order form. Oversized or extra heavy parcels will require additional charges and the customer will be billed any extra shipping costs.
