    MEMORANDUM

    To: Chelsea Lhyn K. Opon

    From: Management Team

    Date: November 10, 2015

    Subject: Warning

    I want to inform you that I have been receiving complains about your conduct

    towards your co workers these last few weeks. It is said that you are talking

    impolitely to other employees and you are addressing them indecently. There is

    also a report of you having a bad habit of taking breaks during working hours and

    had been caught using office materials for your own personal use. Please be

    reminded that your actions could influence other employees behavior negatively

    and could invoke an adverse image regarding the discipline level in the

    organization.

    We will take this as a misconduct and breach of policy as written to our Employee

    Handbook. We warn to improve your bad habits and bring changes in your

    conduct. If you do not change and continue to breach our policy we will be forced

    to take further disciplinary or corrective actions. Please take this warning

    seriously and do the right actions accordingly
