Membership Agreement and Liability Disclaimer

By filling out the required fields for membership and clicking submit, the applicant acknowledges that all information provided is accurate and current. The applicant understands and is aware that any false information provided is grounds for immediate revocation of membership with the reporting to a national databank and withdrawal of all membership privileges without reimbursement. The American College of Aesthetic and Cosmetic Physicians reserves the right to withdraw membership and/or Board certification by the American Board of Cosmetic and Aesthetic Medicine if information provided is false or a member has conducted him/herself in a manner deemed unprofessional to the standards and bylaws of this organization. Members with Board certification agree that they must be in good standing without any disciplinary actions with an active license and must not have a criminal history. Member agrees to surrender membership and return certificate(s) if any license to practice medicine/nursing is revoked. Member agrees that having membership alone does not qualify member to practice cosmetic and aesthetic procedures. Member with board certification acknowledges that this is used as a criteria for promoting the basic fundamental knowledge and safety, and the physician practicing aesthetic and cosmetic medicine in no way can hold the American College of Aesthetic and Cosmetic Physicians, the American Society of Aesthetic/ Cosmetic Physicians and the American Board of Cosmetic and Aesthetic Medicine liable or responsible for the members cosmetic procedures or practicing of medicine. To clarify further, the American College of Aesthetic and Cosmetic Physicians, American Society of Aesthetic/ Cosmetic Physicians, and American Board of Cosmetic and Aesthetic Medicine will make no guarantees for nor assume any liability for the actions of any members in their practice of medicine as a result of becoming a member or earning the American Board of Cosmetic and Aesthetic Medicine certification. The member understands and acknowledges that his/ her practice of any form of aesthetic or cosmetic medicine is completely the responsibility of his/ herself and there is no liability or responsibility on this organization. The American Board of Cosmetic and Aesthetic Medicine make no guarantee of efficacy of any mode of procedure or practice of medicine on the part of its members. The Board Certification itself does not guarantee the efficacy nor safe practice of any medical practitioner in the use of aesthetic or cosmetic procedures. Treatments of patients are entirely within the control of medical practitioners and are not in any way supervised by the Board. Furthermore, membership from the American College of Aesthetic and Cosmetic Physicians and Board Certification from the American Board of Cosmetic and Aesthetic Medicine is not a license to perform aesthetic or cosmetic procedures and each member must check with rules and regulations within their own state, territory, or district from licensing authorities. Member accepts that the content of this application will be seen by individuals working for the American College of Aesthetic and Cosmetic Physicians, American Society of Aesthetic/ Cosmetic Physicians, and/ or American Board of Cosmetic and Aesthetic Medicine and he/she authorizes use of this information in a confidential and secure protected manner. American College of Aesthetic and Cosmetic Physicians, American Society of Aesthetic/ Cosmetic Physicians, or American Board of Cosmetic and Aesthetic Medicine reserves the right to post or create a list of its members on its website 

that will include medical designation and certifications. If the American College of Aesthetic and Cosmetic Physicians, American Society of Aesthetic/ Cosmetic Physicians, or American Board of Cosmetic and Aesthetic Medicine is contacted by an outside source to inquire if a physician or healthcare professional is a member and/ or holds the American Board of Cosmetic and Aesthetic Medicine certificate, confirmation shall be granted if it is accurate. Member seeking certification is aware there is no organization in Aesthetic Medicine that is currently part of the American Board of Medical Specialties (ABMS). It is the goal of the American Board of Cosmetic and Aesthetic Medicine to work with and towards eventually being recognized by the ABMS. Until then, it will remain a private Board as many other current medical specialty Boards. The logos for the American College of Aesthetic and Cosmetic Physicians, American Society of Aesthetic/ Cosmetic Physicians, and American Board of Cosmetic and Aesthetic Medicine are registered trademarks and are not available for general use. Members using the names of our affiliated organizations for business cards (i.e. Board certification) or for resume must be used in an ethical manner and must not misrepresent the integrity of the organization nor be used to mislead anyone. The applicant understands payment is for membership and is aware that terms, conditions, and pricing are subject to change without notice. Lastly, the applicant understands that membership to the American College of Aesthetic and Cosmetic Physicians and American Society of Aesthetic/ Cosmetic Physicians is a privilege not a right. The applicant has read completely and understands these terms and conditions.
